BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Dna je zánětlivé, metabolicky podmíněné onemocnění indukované krystaly. Prevalence dny stále vzrůstá. V klinické praxi je často špatně diagnostikována a terapie především akutního záchvatu není adekvátní. V první části publikace jsou rozebírány diagnostické možnosti dnavé artritidy. Především je zdůrazněna výhodnost analýzy synoviálního výpotku a přímého průkazu krystalů v polarizačním mikroskopu. Při nedostupnosti materiálu na krystalografickou analýzu je nutné použít kombinaci klinických kritérií tak, jak např. uvádí ve svých doporučeních Evropská liga proti revmatizmu (EULAR). V druhé části je rozebírána terapie dny, která je rozdělena na období asymptomatické hyperurikemie, akutního dnavého záchvatu, interkritického období a chronické tofózní dny. Asymptomatickou hyperurikemii léčíme pouze při opakovaně vyšších urikemiích než 540 ?mol/l nebo při přítomnosti dalších rizikových faktorů a komorbidit. Pro léčbu akutního dnavého záchvatu je důležitější včasné zahájení léčby než výběr preparátu. Alternativou jsou NSA, kolchicin nebo glukokortikoidy. Nově registrovaným lékem pro léčbu refrakterního akutního zánětu je inhibitor IL1 kanakinumab. Při léčbě hyperurikemie se uplatňují režimová opatření, dietní opatření, abstinence a hypourikemická léčba. K dispozici jsou inhibitory xantinoxidázy alopurinol a febuxostat, který je vhodnější u pacientů se středně závažnou renální insuficiencí. Novým lékem pro léčbu těžké refrakterní tofózní dny je peglotikáza. In clinical practice it is frequently wrongly diagnosed and the therapy of acute attacks in particular is not adequate. The first part of the publication discusses diagnostic possibilities of gouty arthritis. First of all the advantage of the analysis of synovial exudate and of direct evidence of crystals in the polarization microscope is emphasized. If the material for crystallographic analysis is not available, it is necessary to use a combination of clinical criteria as specified e.g. in the recommendations of the European League Against Rheumatism (EULAR). The second part focuses on the therapy of gout which is divided into the periods of asymptomatic hyperuricemia, acute gouty attack, intercritical and chronic tophaceous gout. Asymptomatic hyperuricemia is only treated when uricemia greater than 540 ?mol/l occur repeatedly, or when other risk factors and comorbidities are present. In the treatment of acute gouty attack its early start is more important than the choice of a preparation. Alternatives are NSA, colchicine or glucocorticoids. A newly registered medicine for the treatment of refractory acute inflammation is the IL-1 inhibitor canakinumab. The treatment of hyperuricemia involves regimen and diet measures, abstinence and hypouricemic therapy. Available are the xanthine oxidase inhibitors, allopurinol and febuxostat; the latter is better suited for patients with moderate renal insufficiency. Tauopatie jsou neurodegenerativní onemocnění, která se vyznačují hromaděním abnormně modifikovaných forem proteinu zvaného tau především v kůře frontálních, temporálních a parietálních laloků, v bazálních gangliích a v mezencefalu. Molekulárně biologicky a biochemicky dobře definovaná skupina chorob má však dosti heterogenní klinické obrazy a je podstatou několika různých neurodegenerativních onemocnění tvořících frontotemporální lobární degenerace. Společným klinickým znakem těchto entit je ve větší či menší míře vyjádřený syndrom frontotemporální demence. Klinické jednotky, jež se řadí mezi klasické frontotemporální demence, však nejsou vždy podmíněny jen tauopatiemi, mnohdy je jejich etiopatogenetická podstata jiná. Účelem tohoto stručného přehledu je utřídění současné úrovně poznání vztahů mezi skupinou tauopatií a frontotemporálních lobárních degenerací. Článek obsahuje i návrh praktického postupu k upřesnění klinické diagnózy jednotlivých nozologických jednotek v rámci tauopatií., Tauopathies are neurodegenerative disorders characterized by accumulation of abnormally modified forms of the tau-protein, predominantly in frontal, temporal and parietal cortical regions, basal ganglia and in the midbrain. Tauopathies are well defined from the molecular biological and biochemical point of view; clinical symptoms may be, however, heterogeneous. Common signs of frontotemporal lobar degenerations include a more or less prominent syndrome of frontotemporal dementia. On the other hand, the clinical picture of frontotemporal dementia may not be caused by a tauopathy only; in many cases the underlying etiopathogenic cause is different. The aim of our review is to classify the relationship between tauopathies and frontotemporal lobar degenerations. This review contains a proposal of a practical approach to refining clinical diagnosis of the different tauopathies. Huntingtonova nemoc (HD) je neurodegenerativní porucha způsobená elongací CAG repetic v genu kódující protein huntingtin (Htt). U pacientů jsou v postižených tkáních přítomny vedle monomerní formy hlavně N‑koncové fragmenty, oligomery a polymery mutovaného huntingtinu (mtHtt), oproti tomu samotná monomerní forma mtHtt je exprimována v podstatě ve všech buňkách. Nejvíce postižené tkáně jsou bazální ganglia a mozková kůra. V této studii jsme analyzovali přítomnost N‑koncových fragmentů a oligomerů Htt v různých tkáních 24- a 36měsíčních transgenních (TgHD) miniprasat exprimujících N‑koncovou část lidského mutovaného huntingtinu a jejich zdravých sourozenců. Zjistili jsme, že mozková kůra a varlata na rozdíl od svalu a srdce TgHD miniprasat obsahují kromě monomerní formy i N‑koncové fragmenty a oligomerní smíry. Ve svalech z 36 měsíčních TgHD miniprasat však již začíná mírná fragmentace. Tato zjištění napodobují časnou progresi onemocnění u lidí, a proto miniprase poskytuje slibný model pro terapeutické testování HD. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Huntington’s disease (HD) is a neurodegenerative disorder caused by the the elongation of CAG triplet repeat in the gene encoding the huntingtin protein (Htt). In patients, in addition to the monomeric form of huntingtin, N‑terminal fragments, oligomers, and polymers are present mostly in the affected tissues, even though the mutated huntingtin (mtHtt) is expressed basically in all cells. The most affected tissues are basal ganglia and cerebral cortex. In this study we analyzed the presence of N‑terminal fragments and oligomers of Htt in different tissues of 24 and 36 months old experimental animals. This was done in our large animal model of HD, which uses transgenic (TgHD) minipigs expressing N‑terminal part of human mtHtt. Among all the tissues tested, we found cortex and testes to contain N‑terminal fragments as well as oligomeric smears in TgHD minipigs compared to wild type siblings. On the other hand, we did not detect any fragments or oligomers in muscle and heart of TgHD minipigs, only starting fragmentation in muscles of 36 months old animals. V současné době jsou pro prevenci a léčbu tromboembolické nemoci k dispozici nová perorální antikoagulancia (NOAC), která jsou registrována pro vybrané indikace i v ČR. Jedná se o přímé perorální inhibitory trombinu (dabigatran etexilát) a přímé inhibitory faktoru Xa (apixaban a rivaroxaban), jejichž výhodou jsou některé vlastnosti, které nevykazují dosavadní „klasická“ antikoagulancia. I v souvislosti s novými antikoagulancii však řešíme některé problémy, jako jsou laboratorní monitoring v urgentních situacích účinné léčby a u krvácivých komplikací dosavadní absenci specifického antidota. Níže přiložený text předkládá indikace, dávkování, způsob eliminace, sledování účinnosti léčby či rizika krvácení a léčbu krvácivých komplikací., There are presently new oral anticoagulants (NOAC) for prevention and the treatment of thromboembolic diseases and they are registered in CZ. It concerns of orally direct inhibitors of thrombin (dabigatran etexilate), inhibitors of factor Xa (apixaban, rivaroxaban), respectively, with advantage of some properties not being seen in „classical” anticoagulants. In the use of new anticoagulants, however, are some problems – such as laboratory monitoring in urgent situations of effective treatment and the absence of specific antidote – resolved. Invazivní mykotické infekce (IFI) jsou spojeny u imunokompromitovaných dětí s významnou morbiditou a mortalitou. Včasné zahájení cílené léčby je klíčovým faktorem pro úspěšný výsledek. Nicméně, možnosti léčby IFI u dětí jsou omezené. Ne vždy je snadné včas a přesně potvrdit diagnózu IFI a správně identifikovat etiologické agens. Použití několika dostupných účinných antimykotik u dětí je komplikováno nedostatkem farmakokinetických a bezpečnostních dat. V našem sdělení bychom chtěli shrnout současná doporučení v cílené antimykotické léčbě v dětské hemato-onkologii na základě vlastních zkušeností a doporučení publikovaných během posledních let., Invasive fungal infections (IFI) in immunocompromised children are associated with significant morbidity and mortality. Early targeted therapy is the key factor for successful outcome. However, treatment options against IFI in children are limited. Not always it is easy to timely and to accurately confirm the diagnosis of IFI and properly identify the etiologic agent. Use of several available potent antifungals in children is complicated by the lack of pharmacokinetic and safety data. Závislost na tabáku je příčinou každého šestého úmrtí v ČR a souvisí s nemocemi celého těla. Léčba závislosti na tabáku by měla být samozřejmou součástí léčebně preventivní péče v rozsahu dle časových možností – od krátké intervence při každém klinickém kontaktu s pacientem až po intenzivní léčbu. Zahrnuje psychosociobehaviorální podporu a farmakoterapii. Měla by se týkat všech profesí v klinické medicíně – dle doporučení WHO především lékařů, sester, lékárníků a dentistů, a měla by být hrazena v rámci zdravotnických systémů. Z ekonomického hlediska je to jedna z nejvýhodnějších intervencí v medicíně. V naší populaci kouří asi 30 % populace nad 15 let (asi 2 200 000 osob), diagnóza F17 se týká asi 1,75 milionu osob (zhruba 80 % kuřáků)., Tobacco dependence causes every sixth death in the Czech Republic and is associated with diseases of the whole body. Treatment of tobacco dependence should be a standard part of clinical care to the extent of time available – from brief intervention at each clinical contact with patients up to intensive treatment. It includes psycho-socio-behavioural support and pharmacotherapy. It should apply to all professions in clinical medicine – as recommended by WHO mainly doctors, nurses, pharmacists and dentists, and should be covered within health care systems. Economically, it is one of the most cost-effective interventions in medicine. Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research.
