Cieľ: Cieľom štúdie bolo identifikovať úroveň emočnej inteligencie (EI) u sestier a zistiť rozdiely v úrovni EI vzhľadom na vek, dĺžku praxe a vzdelanie sestier. Metodika: Výskumný súbor tvorili sestry (n = 280) poskytujúce starostlivosť pacientom štyroch nemocníc Trnavského kraja. Priemerný vek sestier bol 39,64 (SD = 9,44), dĺžka ich profesijnej praxe bola 18,7 (SD = 10,64). Na meranie úrovne EI boli použité dotazníky, ktoré reprezentujú súčasné prístupy chápania EI. Dotazník SITEMO (Situational Test of Emotional Understanding), zisťuje úroveň EI ako schopnosti, výsledkom je celkové skóre EI, ktoré je dané súčtom správnych odpovedí. Dotazník SEIS (Schutte EI Scale) zisťuje úroveň EI sestier ako črty. Sestry vyjadrovali mieru súhlasu resp. nesúhlasu prostredníctvom 5bodovej Likertovej škály. Pri spracovaní dát boli využité metódy deskriptívnej štatistiky a induktívnej štatistiky. Údaje boli spracované v štatistickom softvéri SPSS 15.0. Výsledky: Zistili sme štatisticky významný rozdiel (p < 0,05) v úrovni EI sestier vzhľadom k ich veku. Pri ostatných sociodemografických znakoch (dĺžka profesijnej praxe, vzdelanie) nebol zaznamenaný štatisticky významný rozdiel (p > 0,05). Sestry s Mgr. stupňom vzdelania dosiahli najlepší výsledok v priemernom poradí mier EI v oboch metodikách. Záver: Zistené výsledky umožňujú upriamiť pozornosť na rozvoj EI u sestier prostredníctvom možností vzdelávacích inštitúcií. Môžeme podporovať zmeny, ktoré budú prispievať ku zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti. Využívanie EI je nutnou a zároveň modernou požiadavkou ošetrovateľstva., Aim: The aim of this study was to identify the level of emotional intelligence (EI) of nurses and to identify differences in the level of EI in relation to their age, length of work experience and education. Methods: The sample consisted of nurses (n = 280) providing care to patients in four hospitals in the Trnava Region. The mean age of the nurses was 39.64 years (SD = 9.44) and the length of their work experience was 18.7 years (SD = 10.64). The level of EI was measured using questionnaires representing current approaches to understanding of EI. Emotional intelligence as an ability was determined by the Situational Test of Emotional Understanding (SIT-EMO) questionnaire, yielding the overall emotional intelligence score given by the sum of the correct answers. EI as a feature, on the other hand, was assessed by the Schutte EI Scale (SEIS) in which the nurses expressed the extent to which they agree or disagree with each statement on a five-point Likert scale. To analyze the data, methods of descriptive statistics and inductive statistics were used. The data were processed using the SPSS 15.0 statistical software. Results: A statistically significant difference (p < 0.05) was found between the level of nurses’ EI level and their age. In terms of other social demographic data (length of work experience, education), a statistically significant difference (p > 0.05) was not found, although the nurses with a master’s degree ranked highest as for their mean level of EI, using both methods. Conclusion: The results enable to focus attention on the development of nurses’ EI through educational institutions. Changes may be encouraged that will contribute to an increase in the quality of nursing care provided. ABSTRAKT Cieľ: Zámerom štúdie bolo identifikovať štatisticky významné rozdiely v úrovni emočnej inteligencii (EI) u študentov ošetrovateľstva vzhľadom k demografi ckým údajom (vek, dĺžka štúdia) a zároveň zistiť či úroveň EI študentov ošetrovateľstva ovplyvňuje ich záujem o problematiku geriatrického ošetrovateľstva. Metódy: Výskumu sa zúčastnilo 86 univerzitných študentov denného bakalárskeho štúdia odboru ošetrovateľstvo. Na meranie úrovne EI boli využité dotazníky reprezentujúce rôzne chápania EI. Prostredníctvom SIT-EMO (Situational Test of Emotional Understanding) bola zisťovaná EI ako schopnosť. EI ako črta bola zisťovaná dotazníkom SEIS(Schutte Emotional Intelligence Scale). Dotazníkom vlastnej konštrukcie ESE-GP (Emotional self-effi cacygeriatric patient) bola zisťovaná úroveň EI študentov v procese starostlivosti o geriatrických pacientov. Výsledky: Nezistili sme štatisticky významné rozdiely v úrovni EI študentov z hľadiska demografi ckých údajov (vek a ročník štúdia). Medzi úrovňou EI (SEIS)a perspektívnym záujmom študentov venovať sa geriatrickému ošetrovateľstvu bola zistená štatisticky významná súvislosť (p < 0,05). Závery: Zistené výsledky môžu uplatniť najmä inštitúcie poskytujúce ošetrovateľské vzdelávanie, ktorým odporúčame zamerať pozornosť na rozvoj EI v procese vzdelávania študentov ošetrovateľstva., Aim: The aim of the study was to identify statistically significant differences in levels of emotional intelligence (EI) of nursing students in relation with demographic data (age and year of study) and also determine whether the EI level of nursing students affects their interest in the issue of geriatric nursing. Methods: The sample consisted of 86 full-time undergraduate university students of nursing. To measure the level of EI, questionnaires representing diff erent understanding of EI were used. EI as an ability was determined by the SIT-EMO (Situational Test of Emotional Understanding). EI as a trait was evaluated by the questionnaire SEIS (Schutte Emotional Intelligence Scale). The questionnaire of own construction ESE-GP (Emotional self-effi cacygeriatric patient) assessed the level of EI of students in the process of caring for geriatric patients. Results: There were no statistically signifi cant diff erences in the students’ level of EI in terms of demographic data (age and year of study). A statistically signifi cant association (p < 0.05) was found between the students’ level of EI (SEIS) and their promising interest to pursue geriatric nursing. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Cíl práce: Analýza výsledků extrakčních vaginálních operací ve FN Motol v období let 2007 – 2011. Typ studie: Retrospektivní analýza Soubor a metodika: Analýza 341 vaginálních extrakčních porodů – frekvence využití jednotlivých instrumentů, indikace k jednotlivým extrakčním operacím, mateřské a neonatologické komplikace. Výsledky: V daném období stouplo využití extrakčních metod z 0,81 % na 3,55 %. Tento stoupající trend byl dán zejména zvyšující se frekvencí využití vakuumextraktoru (VEX). Na našem pracovišti se VEX do roku 2007 nepoužíval vůbec, v roce 2011 již tvořil 92 % všech extrakčních operací (nárůst z 0 % na 3,27 %, forceps pokles z 0,81 % na 0,28 %). Indikační schéma k využití jednotlivých instrumentů se v průběhu let také změnilo, dominantou k využití forcepsu byla hypoxie, v roce 2011 při vzrůstající oblibě vakuumextraktoru tvořila právě hypoxie již 50 % indikací k jeho využití. Četnost poranění matky i novorozence byla nižší při použití vakuumextraktoru, kefalhematom novorozence v 7 % u VEXu a 13 % u forcepsu. Závěr: V daném období došlo na našem pracovišti k celkovému zvýšení četnosti extrakčních porodů, a to zejména vlivem použití vakuumextraktoru. Vakuumextraktor je dle našich dat spojen s nižší mateřskou a novorozeneckou morbiditou. Neonatologické výsledky obou metod jsou srovnatelné, mírně však ve prospěch vakuumextraktoru. Výjimkou je jeho neúspěšné použití., Objective: To analyze the results of assisted vaginal deliveries at the Motol University Hospital for the period 2007–2011. Type of study: A retrospective analysis Material and Methods: Analysis of 341 vaginal assisted deliveries - frequency of individual instrument use, indications for assisted vaginal deliveries, maternal and neonatal complications. Results: During the given time period the frequency of assisted vaginal deliveries increased from 0,81 % to 3,55 %. This increasing trend was mainly due to the increasing frequency of vacuum extractor (VEX) use. The first vacuum assisted vaginal delivery at our hospital was recorded in 2007. In 2011 this method already accounted for 92 % of all assisted vaginal deliveries (an increase from 0 % to 3,27 %; the frequency of forceps deliveries decreased from 0,81 % to 0,28 %). Over the years, indications for the use of instruments have also changed. Main indication for forceps delivery used to be hypoxia, but in 2011, with increasing popularity of vacuum extractions, it accounted only for 50 % of indications for its use. Frequency of maternal and neonatal injuries was lower when using vacuum extractor, newborn cephalhaematoma was recorded in 7 % when vacuum extractor was used and 13 % when forceps was used. Conclusion: During the given time period there was an overall increase of assisted vaginal deliveries at our department, mainly because of the vacuum extractor use. According to our data, vacuum extraction is associated with lower maternal and neonatal morbidity. Cíl: Hlavním cílem výzkumného šetření bylo zjistit, které faktory ovlivňují pouţívání ošetřovatelských diagnóz v klinické praxi. Dalším cílem bylo zjistit rozdíly v hodnocení faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi podle věku, vzdělání a typu pracoviště. Metodika: Pro získání dat byl sestaven nestandardizovaný dotazník zaměřený na zjištění faktorů ovlivňujících pouţívání ošetřovatelských diagnóz. Výzkumný soubor tvořilo 253 registrovaných všeobecných sester z 9 zdravotnických zařízení v České republice. Sestry hodnotili na Likertově škále od 1 do 5, jak jednotlivé faktory ovlivňují pouţívání ošetřovatelské diagnostiky v praxi. Výzkumné šetření probíhalo od června 2011 do listopadu 2011. Výsledky: Podle našeho výzkumného souboru nejvíce ovlivňují pouţívání ošetřovatelských diagnóz v praxi následující faktory: počet pacientů na 1 sestru, administrativní zátěţ, syndrom vyhoření, nedostatek času, rozdíly v teorii a praxi a nedostatek personálu. Statisticky významné rozdíly v hodnocení faktorů podle věku byly zjištěny v hodnocení 2 faktorů: rozdíly v teorii a praxi, nedostatek motivace. Podle dosaţeného vzdělání byly statisticky významné rozdíly zjištěny v hodnocení 4 faktorů: nejednotnost ošetřovatelských diagnóz, terminologie diagnóz, rozdíly v teorii a praxi, rutinní práce. V závislosti na typu pracoviště byly statisticky významné rozdíly nalezeny v hodnocení 5 faktorů: terminologie diagnóz, rozdíly v teorii a praxi, nedostatek praktických zkušeností s ošetřovatelskou diagnostikou, rutinní práce, syndrom vyhoření. Závěr: Rozeznání faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory můţe ulehčit implementaci ošetřovatelské diagnostiky v klinické praxi, Aim: The main aim of the study was to identify which factors influenced the use of nursing diagnoses in clinical practice. Another aim was to ascertain whether age, education and type of workplace affected the relative importance accorded to these factors. Methods: A non-standardized questionnaire on factors influencing the use of nursing diagnoses was administered to a sample consisting of 243 registered general nurses from 9 healthcare facilities in the Czech Republic. Using Likert´s 5-point scale, the nurses rated the importance of several factors in influencing their use of nursing diagnoses in practice. The research was conducted between June 2011 and November 2011. Results: Based on our sample, there are several factors which influence the use of nursing diagnoses in practice. Some of them are: number of patients per nurse, administrative strain, burnout syndrome, lack of time, discrepancies between theory and practice, lack of staff. Statistically significant age-related differences in rating factors were found for the following: discrepancies between theory and practice, lack of motivation. Statistically significant differences in ratings relative to education were found for four factors: disunity of nursing diagnoses, diagnostic terminology, discrepancies between theory and practice, workplace routine. Based on type of workplace, statistically significant differences in ratings were found for five factors: diagnostic terminology, discrepancies between theory and practice, lack of practical experience in nursing diagnostics, workplace routine, burnout syndrome. Cieľ: Cieľom štúdie bolo analyzovať faktory ovplyvňujúce dentálnu hygienu, medzi ktoré patrí konzumácia sladkostí a sladených nápojov u školskej mládeže z hľadiska veku a pohlavia a zároveň analyzovať výskyt dentálnej hygieny počas dňa. Metodika: Súbor respondentov tvorilo 1187 žiakov (620 dievčat a 567 chlapcov) vo veku 11, 13 a 15 rokov v oblasti stredného Slovenska. Zber údajov prebiehal v období máj 2010 až jún 2010. Na zber údajov bol použitý štandardizovaný dotazník vytvorený pre potreby HBSC štúdie (Health Behaviour in School – aged Children). Výsledky boli vyhodnotené Chí-kvadrát testom. Výsledky: Sladkosti a kolu v jednotlivých vekových kategóriách konzumuje každý deň alebo viac krát za deň približne tretina respondentov. Výskyt umývania zubov viac ako raz denne je približne rovnaký vo všetkých vekových kategóriách od 53% u 11 ročných chlapcov do 64% u 15 ročných dievčat. Vo všetkých vekových kategóriách dievčatá udávajú častejšie umývanie zubov v porovnaní s chlapcami. Záver: Výsledky poukazujú na zvýšenú konzumáciu sladkostí a koly počas dňa u školskej mládež stredného Slovenska, čo môže spôsobovať zvýšenú kazivosť zubov. Dentálna hygiena je v našom súbore respondentov nedostačujúca. Na základe získaných výsledkov je možné konštatovať, že potreba zvyšovania úrovne vedomostí nielen detí, ale aj rodičov o zásadách správneho stravovania a o zásadách dentálnej hygieny je neodmysliteľnou súčasťou výchovy k zdraviu a preventívnych opatrení v školskom veku., Aim: The aim of the study was to analyse the factors affecting dental hygiene such as consumption of sweets and sweet soft drinks in school-aged pupils in terms of their age and gender and at the same time to analyse the occurrence of dental hygiene during the day. Methods: The set of respondents consisted of 1187 pupils (620 girls and 567 boys) at the age of 11, 13 and 15 years in the area of central Slovakia. The data collection was conducted in the period of May 2010 to June 2010. For data collection was used standardized questionnaire for HBSC studies (Health Behaviour in School–aged Children). The results were analysed by Chi-square test. Results: Approximately one third of respondents in each category consumes sweets and Coca-cola every day or several times a day. The occurrence of cleaning the teeth more than once a day was equally proved in all age groups ranging from 53% in 11 years old boys to 64% in 15 years old girls. The girls of all age groups indicate more frequent cleaning of teeth in comparison with boys. Conclusion: The results show increased consumption of sweets and Cocacola during the day in the school youth from central Slovakia, which may cause an increase in tooth decay. Dental hygiene in our set of respondents is insufficient. Předmětem této pilotní studie je verifi kace vlivu pozitivních emocí na motorické učení prostřednictvím vizuálního feedbacku formou hry ve virtuálním prostředí u pacientů po iktu. Pilotního měření se zúčastnili 3 pacienti po cévní mozkové příhodě, kteří absolvovali dvoutýdenní terapii formou hry ve virtuálním prostředí. U každého pacienta byla hodnocena před a po terapii posturální stabilita a hrubá motorika paretické horní končetiny pomocí standardizovaného klinického testování (Berg Balance Scale, Action Research Arm Test) i přístrojového vyšetření (dynamická počítačová posturografi e). Z výsledků hodnocení pacientů je zřejmý pozitivní efekt terapie na posturální stabilitu a koordinaci. Hru ve virtuálním prostředí považujeme za vhodný doplněk komplexního fyzioterapeutického přístupu u vybraných pacientů po cévní mozkové příhodě. Vzhledem k výsledkům naší pilotní studie jsou žádoucí další měření zabývající se touto problematikou., This pilot study aims to objectively verify the infl uence of positive emotions on motor learning through visual feedback in the form of games in a virtual environment in individuals aft er a stroke. In this pilot study, we evaluated three stroke patients. They completed a two-week intensive therapy by means of playing virtual games. Postural stability was assessed in every patient before and aft er the therapy, as well as gross motor skills of paretic upper limb by both clinical (Berg Balance Scale, Action Research Arm Test) and instrumental (Dynamic Computed Posturography) testing. The evaluation of patients showed signifi cant positive eff ect of game based therapy on postural stability and coordination. Therefore, playing games in a virtual environment might be considered a suitable addition to the complex of physiotherapeutic approach in patients aft er stroke. Od objavenia mimobunkovej fetálnej DNA v krvi matky uplynulo už 16 rokov. Avšak až v priebehu posledných 5 rokoch prebieha čoraz viac výskumov za účelom stanovenia jej presnej charakteristiky, koncentrácie a molekulárnej veľkosti, ktoré sa neskôr premietli do jednotlivých testov. Tie sú ale dostupné len v posledných 2 rokoch. Pri aktuálnom trende preferovania neinvazívnych metód prenatálnej diagnostiky a ich posúvania do obdobia prvého trimestra sa stanovenie mimobunkovej fetálnej DNA v krvi matky ukazuje ako veľmi perspektívnou metódou diagnostiky chromozómových aberácií u plodov. Jej širšie uplatnenie je limitované malým počtom štúdií na nerizikových skupinách pacientok a v nemalej miere aj ekonomickou náročnosťou vyšetrenia. Aj napriek tomu sa však táto metóda čoraz viac rozširuje zo Spojených Štátov Amerických do celého sveta, čo znižuje náklady na ňu, a stáva sa dostupnejšou pre čoraz väčší počet pacientok. Ukazuje sa, že táto metóda by v rámci podrobnejšieho vyšetrovania u rizikových pacientok mohla perspektívne minimalizovať riziko fetálnych strát, ktoré sú závažnou komplikáciou invazívnych metód prenatálnej diagnostiky., It has been 16 years since there has been a discovery of cell-free fetal DNA (cffDNA) in mother‘s blood. However in the last 5 years there has been more researching done in the purpose of its specific diagnosis, concentration and molecular size, which have later on reflected into particular tests. These have been available only in the past 2 years. Within the actual trend in preference of non-invasive methods of prenatal diagnosis and its shifting into first trimester the examination of mother’s blood has been showing as a very perspective method for early cell-free fetal DNA discovering of chromosome aberration diagnosis of fetus. Its wider enforcement is unfortunatelly limited by a small amount of studies within non-risk pregnancy groups of patients as well as its higher financial cost. Despite of this information this method is extending further more from United States of America into whole world which lowers its costs and its becoming more accesible for a wider amount of patients. Fyziologická gravidita je sprevádzaná zmenami v lokálnej a systémovej koagulácii a fibrinolýze. Hemostáza zasahuje do procesu implantácie blastocysty a placentácie. Udržiava optimálne prekrvenie placenty, zabezpečuje rýchle a adekvátne zrážanie počas pôrodu. Mnohé koagulačné proteíny sú exprimované na začiatku embryonálneho vývoja a hrajú úlohu mimo koaguláciu v proliferácii a diferenciácii buniek. Mechanizmus regulácie v placente nie je dostatočne chápaný. Poruchy hemostázy sa podieľajú na peripartálnom krvácaní, gestačnom tromboembolizme a považujú sa za rizikový faktor neúspešnej reprodukcie a tehotenských komplikácií., Physiological pregnancy is accompanied by changes in local and systemic coagulation and fibrinolysis. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Mechanism regulating haemostasis within the placenta remain poorly understood. Cíl: cílem našeho šetření bylo vyhodnotit moţnou přítomnost bolesti u klientů s demencí, kteří byli přeloţeni na gerontopsychiatrické lůţko psychiatrické léčebny v Kroměříţi z jiného zdravotnického zařízení. Metodika: studium překladové ošetřovatelské dokumentace z odesílajících zdravotnických zařízení, škála pro hodnocení bolesti u pacientů s demencí (PAINAD). Soubor: klienti přeloţení do psychiatrické léčebny v Kroměříţi, obojího pohlaví, starší 65 let, pro demenci jakéhokoliv typu. Tíţe demence byla středně těţká a těţká (bylo hodnoceno při přijetí na gerontopsychiatrické lůţko celkovým skóre dosaţeným v Mini Mental State Examination - MMSE - v rozmezí 0 - 17). Výsledky: pomocí škály PAINAD byla zachycena bolest u 26,7 % přeloţených klientů se středně těţkou nebo těţkou demencí. Bolest však byla řádně zadokumentována v překladových ošetřovatelských zprávách jen u 8,96 % klientů. Téměř třetina ošetřovatelské překladové dokumentace (29,9 %) měla zcela nevyplněný, nebo nedostatečně vyplněný formulář pro hodnocení bolesti. Téměř pětina (19,4 %) ošetřovatelských překladových zpráv fyzicky vůbec neobsahovala ţádný takový formulář. Závěr: přítomnost nepodchycené nebo nedostatečně vyhodnocené bolesti u klientů s kognitivními poruchami je závaţným problémem moderního ošetřovatelství. Nutná je potřeba neustálé edukace zdravotnického personálu, kvalitního vedení ošetřovatelské dokumentace, ale také potřeba důslednějšího systému kontrol z řad vedoucích pracovníků., Aim: to evaluate presence of pain in clients of both sexes, older than 65, who have been admitted to Mental hospital Kroměříţ due to dementia of any type. Methods: study of nursing documentation from dispatching health care facilities, PAINAD scale (Pain assessment in advanced dementia). Severity of dementia was moderate of severe (this was evaluated by Mini-Mental State Examination total score during admission process to our hospital, scores ranged from 0 to 17). Results: pain was detected in 26.7% of clients with moderate or severe dementia by means of PAINAD scale, compared to 8.96% of clients with pain which has been reported in nursing documentation. Almost in one third of nursing documentation (29.9%) no data or incomplete data about pain were found. Almost one fifth (19.4%) of nursing documentation have not contained any tool for pain screening. Concusion: undiagnosed pain in cognitively impaired clients is a severe problem of nursing care.
