Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Castlemanova choroba je vzácnou nosologickou jednotkou, která byla v literatuře poprvé popsána v roce 1954 Castlemanem a Lownovou. Jde o onemocnění primárně postihující lymfatické uzliny, u kterého se rozlišují čtyři histomorfologické podtypy a dva klinické obrazy (uni a multicentrický). Nejčastěji se klinicky projeví jako lymfadenopatie mediastina, krku či retroperitonea. Příznaky nemoci jsou výrazně heterogenní a záleží na daném histologickém podtypu. Taktéž prognóza se liší podle typu nemoci, kdy může jít jak o zcela benigní nálezy (zejména forma unicentrická), tak o potenciálně maligní formu, při které nemoc progreduje až do stadia maligní lymfoproliferace (multicentrická forma). Autoři prezentují případ 29letého muže s klinicky manifestním (ikterus, poruchy střevní pasáže) objemným tumorem retroperitonea lokalizovaným subhepatálně, který byl úspěšně radikálně exstirpován. Pooperační průběh byl nekomplikovaný. Definitivní histopatologická diagnóza potvrdila hyalinně-vaskulární typ Castlemanovy nemoci. Vzhledem k tomu, že se jednalo o multicentrickou formu onemocnění, bude následovat adjuvantní onkologická léčba. It is rare sickness which primary affects lymfatic nodes. There are four histo-morfologic subtypes and two clinic manifestations (uni and mulicentric). Comon clinical picture is simple lymfadenopathy of neck, medistinal a retroperitonal lymfatic nodes. Signs of this disease are heterogenic and depend on histological subtype. Also prognosis depends on type of illness. It should take place as benign diagnosis (unicentric form) but also can be potencionally malignant form which shlould exacerbate to malignant lymfoproliferation (multicentric form). Authors present the case of 29 years old men with clinical manifestation (jundice, intermitent bowel obstruction) of lagre retroperitoneal tumor in subhepatal localization. The treatment was radical surgical extirpation, surgery was without complications. Definitive histological diagnosis was confirmed as hyaline-vascular type of Castleman disease. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci.
