Neurologické postižení je poměrně vzácná komplikace hepatitidy E. Uvedená kazuistika popisuje 52letého imunokompetentního muže s aseptickou meningitidou a mnohočetným postižením periferních nervů na horních končetinách typu mononeuropatia multiplex, u kterého byla sérologickým vyšetřením prokázána akutní virová hepatitida E. Navíc byl virus hepatitidy E zjištěn polymerázovou řetězovou reakcí v séru i likvoru (genotyp 3f). Pacient byl léčen pouze symptomaticky, po půl roce přetrvával mírný neurologický deficit. Infekci virem hepatitidy E je vhodné zvažovat u pacientů se současným neurologickým a jaterním postižením., Neurological complications of acute hepatitis E are relatively rare. The case report describes mononeuropatia multiplex with aseptic meningitis in a 52‑year‑old immunocompetent man. Viral hepatitis E was confirmed serologically and by polymerase chain reaction from the blood and cerebrospinal fluid (genotype 3f). Symptomatic treatment was given. Residual neurological deficit persisted six months after the onset of the disease. Diagnostic of hepatitis E should be considered in patients with concurrent neurological symptoms and liver cytolysis. Autoři uvádějí kazuistiku 41leté pacientky se subakutním vývojem kmenové a pyramidové symptomatologie a bolestí hlavy. Vývoj nálezu na magnetické rezonanci mozku odpovídal zánětlivým změnám mozkového parenchymu v oblasti kmene s rozvojem vícečetných ložiskových ischemií v této lokalizaci a v oblasti levého thalamu a zadního raménka capsula interna. V krvi a likvoru byly přítomny známky vysoké zánětlivé aktivity bez prokázaného infekčního agens. Podrobná anamnéza odhalila dlouhodobé projevy systémového postižení: recidivující orální a genitální ulcerace, postižení oční, kloubní, kožní a gastrointestinální. Je popisována diferenciální diagnóza přítomných ložiskových změn mozku, především s ohledem na nekonzistentní echokardiografické nálezy. Klinický průběh se systémovým postižením, paraklinická vyšetření a odpověď na léčbu jsou konzistentní s diagnózou Behçetovy nemoci s neurologickými komplikacemi., The authors present a case report of a 41‑years‑old woman with subacute onset of brainstem and pyramidal symptoms, and a headache. Progression of the finding on magnetic resonance suggested brainstem parenchymal inflammation, and formation of multiple focal infarcts in the brainstem and the left thalamus and posterior branch of capsula interna. Serum and cerebrospinal fluid tests showed high inflammatory markers and no evidence of any infectious agent. Detailed history revealed long‑term systemic involvement presenting as recurrent oro‑genital ulcers accompanied with eye, joint, skin and gastrointestinal disorders. Differential diagnosis of focal brain changes is discussed, including inconsistent echocardiographic findings. The clinical course with systemic involvement, laboratory findings, and response to therapy support the diagnosis of Behçet’s disease with neurological manifestation. Úvod: Protilátky proti membránovým a synaptickým antigenům jsou přímo patogenní protilátky sdružené s pestrou paletou neurologických syndromů. Různorodost příznaků komplikuje stanovení diagnózy, a tím i včasné zahájení imunoterapie. Cíl: Cílem práce bylo popsat klinické charakteristiky pacientů s pozitivitou uvedených protilátek. Metodika a soubor: V období dvou let jsme zjišťovali přítomnost protilátek proti membránovým a synaptickým antigenům u 224 pacientů. Metodou nepřímé imunofluorescence byly vyšetřeny protilátky proti N‑metyl‑D‑aspartátovému receptoru (anti‑NMDAR), α‑amino‑3-hydroxy‑5-metyl‑4-izoxazolepropionátovému receptoru (anti‑AMPAR 1 a 2), B‑receptoru kyseliny γ‑aminomáselné (anti‑GABABR), proti leucin‑rich‑glioma‑inactivated proteinu‑1 (anti‑LGI1) a proti contactin‑associated proteinu‑2 (anti‑CASPR2). U pozitivních pacientů byly retrospektivně zhodnoceny klinické projevy, nálezy pomocných metod a vývoj onemocnění. Pacienti s pozitivitou anti‑NMDAR protilátek nebyli do hodnocení zahrnuti. Výsledky: Autoprotilátky byly prokázány v séru 11 pacientů (medián 58 let, sedm mužů) – u šesti pacientů anti‑LGI1, u čtyř anti‑CASPR2, u dvou anti‑AMPAR1 (u jednoho pacienta současně anti‑AMPAR1 a anti‑CASPR2). Příznaky zahrnovaly epilepsii (n = 5), subakutní encefalopatii (n = 5), doprovázenou ve čtyřech případech epileptickými záchvaty, a mozečkový syndrom s doprovodným kognitivním deficitem (n = 1). U dvou pacientů byly současně přítomny dobře charakterizované onkoneurální protilátky (anti‑Hu, anti‑Ma2). Nádor (malobuněčný karcinom plic) byl zjištěn u jedné pacientky (anti‑AMPAR1). U osmi pacientů došlo po podání imunosuprese k remisi nebo částečnému zlepšení klinického stavu. Závěr: Přítomnost protilátek proti membránovým antigenům je sdružena nejčastěji s limbickou encefalitidou nebo fokální epilepsií, vzácněji s jinými neurologickými syndromy. Efekt imunoterapie závisí na jejím časném podání., Background: Neuronal surface antibodies are associated with numerous neurological symptoms. Better knowledge of these symptoms may improve identification of potential candidates for immunotherapy. Aim: Characterize clinical signs in patients with neuronal surface antibodies positivity. Methods: We detected neuronal surface antibodies in 11/2011–12/2013 in 224 patients (224 in serum and 37 in cerebrospinal fluid). We investigated anti-NMDAR, anti-AMPAR1, anti-AMPAR2, anti-GABABR, anti-LGI1, anti-CASPR2 using cell-based assays for indirect immunofluorescence (Euroimmun AG). We retrospectively analyzed clinical characteristics of patients with positive neuronal surface antibodies in serum or cerebrospinal fluid other than anti-NMDAR positive patients. Results: Neuronal surface antibodies were detected in 11 patients (seven males, median age 58). Six patients had anti-LGI1, four anti-CASPR2 and two anti-AMPAR1 antibodies (one patient had both anti-CASPR2 and anti-AMPAR1 antibodies). Clinical symptoms included chronic epilepsy (n = 5), acute encephalopathy (n = 5) accompanied by epileptic seizures in four patients and one patient presented with cerebellar syndrome and cognitive deficit. Two patients had coincidence of paraneoplastic antibodies (anti-Hu, anti-Ma2). Tumor (small cell lung carcinoma) was diagnosed in one patient (anti-AMPAR1). Eight patients improved following immunotherapy (corticosteroids, IVIG). Early immunotherapy was associated with better outcome. Conclusion: NS-Abs were mostly associated with limbic encephalitis and chronic temporal lobe epilepsy. Immunotherapy had better effect when applied early in the disease course. Neuromedin beta (NMB) is a member of the bombesin-like peptide family expressed in brain, gastroin testinal tract, pancreas, adrenals and adipose tissue. The aim of our study was to compare the frequency of P73T polymorphism in overweight and obese patients (37 men: age 50.6±11.7 years, BMI 41.1±7.8 kg/m2; 255 women: age 49.0±11.9 years, BMI 37.9±6.8 kg/m2) with that of healthy normal weight subjects (51 men: age 28.2±7.1 years, BMI 22.3±2.0 kg/m2; 104 women: age 29.1±9.1 years, BMI 21.5±1.9 kg/m2) and to investigate the polymorphism’s influence on anthropometric, nutritional and psychobehavioral parameters in overweight/obese patients both at the baseline examination and at a control visit carried out 2.5 years later, regardless of the patient́s compliance with the weight reduction program. No significant differences in the genotype distribution were demonstrated between normal weight and overweight/obese subjects. Male T allele non-carriers compared to T allele carriers had higher energy (p=0.009), protein (p=0.018) and fat (p=0.002) intakes and hunger score (p=0.015) at the beginning of treatment. Male T allele non-carriers had a more favorable response to weight management at the follow-up, as they exhibited a significant reduction in waist circumference, energy intake and depression score as well as a significant increase in dietary restraint. No significant differences between carriers and non-carriers were demonstrated in women at the baseline examination. Both female T allele carriers and non-carriers demonstrated similar significant changes in nutritional parameters and in restraint score at the follow-up. We summarize the main results on probabilistic neural networks recently published in a series of papers. Considering the framework of statistical pattern recognition we assume approximation of class-conditional distributions by finite mixtures of product components. The probabilistic neurons correspond to mixture components and can be interpreted in neurophysiological terms. In this way we can find possible theoretical background of the functional properties of neurons. For example, the general formula for synaptical weights provides a statistical justification of the well known Hebbian principle of learning. Similarly, the mean effect of lateral inhibition can be expressed by means of a formula proposed by Perez as a measure of dependence tightness of involved variables.
Neuromyelitis optica (NMO), také známá jako Devicova choroba, je devastující autoimunitní onemocnění centrálního nervového systému klinicky charakterizované záněty zrakového nervu, míchy a méně často mozkového kmene. Objev protilátek proti akvaporinu‑4 (NMO‑IgG/AQP4-IgG) definitivně odlišil tuto chorobu od roztroušené sklerózy. Testování AQP4-IgG je klíčové v diferenciálně diagnostickém procesu, ve kterém se dále neobejdeme bez magnetické rezonance mozku a míchy. Typickým nálezem jsou míšní ložiska přesahující délku tří míšních segmentů. Změny na magnetické rezonanci mozku diagnózu NMO nevylučují, jsou naopak relativně časté a mají poměrně typický charakter. Důležité informace přináší i optická koherentní tomografie, která kvantifikuje postižení sítnice a zrakového nervu. Neurologický deficit u této choroby je následkem relapsů onemocnění, a proto je zcela zásadní časné stanovení diagnózy a nasazení adekvátní terapie, která může změnit nepříznivou prognózu pacienta. Mezi hlavní léčebné strategie patří agresivní léčba relapsů (vysokodávkované steroidy či plazmaferéza) následované zahájením chronické terapie (mezi léky první volby dnes řadíme azathioprin a rituximab)., Neuromyelitis optica (NMO), also known as Devic‘s disease, is a devastating autoimmune disorder of the central nervous system and is clinically characterized by inflammation of the optic nerve, spinal cord, and, less frequently, brainstem. The discovery of antibodies against aquaporin‑4 (NMO‑IgG/AQP4-IgG) has definitively distinguished this disease from multiple sclerosis. AQP4-IgG testing is crucial in differential diagnosis that also includes magnetic resonance imaging of the brain and spinal cord. Typically, spinal cord lesions exceeding the length of three vertebral segments are found. Changes in magnetic resonance imaging of the brain do not exclude the diagnosis of NMO; by contrast, they are relatively common and are fairly typical. Optical coherence tomography provides important information, and quantifies the involvement of the retina and optic nerve. Neurologic deficit in this disease is a result of disease relapse, and it is therefore crucial to establish the diagnosis early and initiate an appropriate treatment that can change the unfavorable prognosis. The main therapeutic strategies include aggressive treatment of relapses (with high‑dose corticosteroids or plasmapheresis), followed by chronic therapy (first choice drugs include azathioprine and rituximab). Cíl: Cílem této práce je popis klinického obrazu pacientů s neuromyelitis optica a poruch jejího širšího spektra (NMO Spectrum Disorders; NMO SD) v České republice, včetně srovnání pacientů se začátkem onemocnění před 50. rokem života a po něm v kontextu výskytu longitudinálně extenzivní myelitis (LETM) a poškození zrakového nervu. Dalším cílem je hodnocení likvorových nálezů a séropozitivity protilátek proti akvaporinu‑4 (AQP4-IgG). Soubor a metodika: Byla provedena systematická retrospektivní analýza klinických a paraklinických nálezů 32 pacientů s NMO SD. Tloušťka nervových vláken sítnice (RNFL) každého oka byla měřena metodou optické koherentní tomografie. Tíže neurologického deficitu byla vyjádřena pomocí EDSS skóre. Výsledky: NMO SD častěji postihují ženy (8 : 1). Medián věku počátku onemocnění je 36 let. Medián délky trvání onemocnění je 10,5 roku. Medián EDSS skóre je 4,5 (rozmezí 1,0–8,5) a délky onemocnění 10,5 let (rozmezí 0,5–24). U 31 z 32 pacientů nacházíme AQP4-IgG v séru a jejich titr pozitivně koreluje s EDSS skóre. RNFL oka pravého (medián 63 ?m) a oka levého (medián 58 ?m) je sníženo pod dolní hranici normy (97 ?m) u 57 %, resp. 64 % pacientů. RNFL koreluje s délkou choroby. Myelitida jako první manifestace onemocnění je častější u pacientů starších 50 let (p < 0,05). Ve srovnání s remisí nacházíme v likvoru během relapsů vyšší koncentraci bílkoviny a počet elementů (p < 0,001; pro oba parametry). Závěr: LETM je typicky první klinickou manifestací NMO u pacientů starších 50 let a je spojena s těžkým neurologickým deficitem již v úvodu onemocnění., Aim: The aim of this study was to describe the clinical picture of patients with NMO spectrum disorders (NMO SD) in the Czech Republic, including comparison of patients with disease onset before and after 50 years of age in the context of longitudinally extensive transverse myelitis (LETM) and optic nerve involvement. Another objective was to evaluate cerebrospinal fluid samples and aquaporin-4 antibody (AQP4-IgG) seropositivity. Methods: A systematic retrospective analysis of clinical and paraclinical findings in 32 patients with NMO SD was performed. The retinal nerve fiber layer (RNFL) thickness in each eye was measured with optical coherence tomography. Severity of neurological deficit was expressed by the EDSS score. Results: NMO SD are more frequent in women (8 : 1). Median age of disease onset was 36 years. Median disease duration was 10.5 years. Median EDSS score was 4.5 (range 1.0–8.5) and median disease duration 10.5 years (range 0.5–24). AQP4-IgG were found in the serum of 31 out of the 32 patients and their titer positively correlated with the EDSS score. The RNFL thickness of the right eye (median 63 ?m) and the left eye (median 58 ?m) was below the lower limit of normal (97 ?m) in 57% and 64% of patients, respectively. The RNFL thickness correlated with the disease duration. Myelitis as the first manifestation of the disease was more frequent in patients older than 50 years (p < 0.05). Compared to remission, patients with a relapse had increased protein levels and element count in the cerebrospinal fluid (p < 0.001 for both parameters). Conclusion: LETM is typically the first clinical manifestation of NMO patients older than 50 years and is associated with severe neurological deficit at the early stages of the disease. We investigated neuronal activity of the medulla oblongata
during gastroesophageal reflux-related cough (GERC). A rat
model of GERC was generated by perfusing HCl into lower
esophagus and inducing cough with citric acid. The HCl group rat
was received HCl perfusion without citric acid-induced cough.
The saline control rat was perfused with saline instead and cough
was induced. Citric acid-induced cough rat was only induced by
citric acid. Blank group rats were fed normally. Fos expressions
were observed in medulla oblongata nuclei using
immunohistochemistry. Manganese-enhanced magnetic
resonance imaging (MEMRI) was performed to detect the
Mn2+ signal following intraperitoneal injection of MnCl2. HCl
perfusion and citric acid-induced cough caused Fos expressions in
the nucleus of solitary tract (nTS), dorsal motor nucleus of the
vagus (DMV), paratrigeminal nucleus (Pa5), and intermediate
reticular nucleus (IRt), which was higher than HCl group, saline
control group, citric acid-induced cough group, and blank group.
A high Mn2+ signal was also observed in most of these nuclei in
model rats, compared with blank group animals. The Mn2+ signal
was also higher in the HCl, saline and citric acid-induced cough
group animals, compared with blank group animals. The study
showed medulla oblongata neurons were excited in a HCl
perfusion and citric acid-induced cough rat model, and nTS, DMV,
Pa5 and IRt neurons maybe involved in the cough process and
signal integrate.
The maintenance of plasma sodium concentration within a narrow limit is crucial to life. When it differs from normal physiological patterns, several mechanisms are activated in order to restore body fluid homeostasis. Such mechanisms may be vegetative and/or behavioral, and several regions of the central nervous system (CNS) are involved in their triggering. Some of these are responsible for sensory pathways that per ceive a disturbance of the body fluid homeostasis and transmit information to other regions. These regions, in turn, initiate adequate adjustments in order to restore homeostasis. The main cardiovascular and autonomic responses to a change in plasma sodium concentration are: i) changes in arterial blood pressure and heart rate; ii) changes in sympathetic activity to the renal system in order to ensure adequate renal sodium excretion/absorption, and iii) the secretion of compounds involved in sodium ion home ostasis (ANP, Ang-II, and ADH, for example). Due to their cardiovascular effects, hypertonic saline solutions have been used to promote resuscitation in hemorrhagic patients, thereby increasing survival rates following trauma. Poruchy kortikálního vývoje a benigní tumory mozku jsou častou příčinou dlouhotrvající farmakorezistentní epilepsie u dětských i dospělých nemocných. Tito nemocní jsou operačně řešeni a následně sledováni v epileptochirurgických centrech. Progrese gradingu mozkových tumorů u nemocných s farmakorezistentní epilepsií je vzácným problémem a rozvoj maligního tumoru mozku u nemocných s malformacemi kortikálního vývoje mozku je publikován zcela výjimečně. Ve sdělení prezentujeme dvě kazuistiky nemocných s farmakorezistentní epilepsií s neočekávaným rozvojem maligního tumoru mozku. U jedné nemocné s chronickou epilepsií došlo k velmi časné progresi gradingu opakovaně histologicky verifikovaného nízkostupňového gliomu s finálním nálezem glioblastomu. U druhého nemocného byl popsán výjimečný rozvoj glioblastomu v terénu hemimegalencefalie., Malformations of cortical development and benign brain tumors are frequent causes of long lasting pharmacoresistant epilepsy in paediatric and adult patients. Such patients are surgically treated and subsequently followed in epileptosurgical centers. Tumor upgrading in patients with pharmacoresistant epilepsy is only rarely encountered and the development of malignant brain tumor in a patient with malformation of cortical development is an exception. The aim of the paper is to present two cases of patients suffering from intractable epilepsy with unexpected development of malignant brain tumor. There was upgrading of low grade glioma (verified by repeated biopsy sampling) in one patient with chronic epilepsy with final diagnosis glioblastoma multiforme.
