Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem., Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. Cieľ: Cieľom našej práce bolo identifikovať kauzatívne germinálne mutácie u suspektných NF1 pacientov za účelom diferenciálnej diagnostiky a zozbieranie čo najväčšieho súboru pacientov pre prípadné genotypovo-fenotypové korelácie. Súbor a metodika: Náš súbor pozostával zo 107 nezávislych pacientov zo Slovenska, ktorý boli v období rokov 2008-2013 klinickými genetikmi posúdení ako suspektní pre NF1. Na identifikáciu mutácií bolo použité sekvenovanie cDNA NF1 génu, ktoré nám umožnilo úspešnú identifikáciu zostrihových mutácií, ako aj MLPA analýza umožňujúca identifikáciu rozsiahlejších delécií v géne. Výsledky: V súbore 94 nepríbuzných slovenských pacientov, ktorí spĺňali základné diagnostické kritériá pre neurofibromatózu, sme za použitia našich metód identifikovali germinálne mutácie v NF1 géne u 83 z nich (88,3 %). Zaznamenali sme vysoký podiel mutácií, ktoré boli zatiaľ identifikované len v slovenskej populácii (41/83, 49,4 %), a u 27/42 testovaných rodinách sme potvrdili mutáciu de novo (64,3 %). Genotypovo-fenotypové korelácie odhalili zvýšený výskyt očných gliómov u pacientov s mutáciou na 5’ konci NF1 génu. Závery: Kombinácia cDNA analýzy a MLPA je efektívnou pre identifikáciu mutácií v NF1 géne. Na základe tejto diagnostickej metódy sme odhalili aj často nesprávne klasifikované netypické zostrihové mutácie. Naše výsledky využili klinickí genetici pri diferenciálnej diagnostike ochorenia, hlavne v sporných prípadoch. Kľúčová slová: NF1 gén – kauzatívna germinálna mutácia – RNA zostrih – genotypovo-fenotypové korelácie – de novo mutácie –diferenciálna diagnostika, Aim: The aim of our study was to identify causative germline mutation in suspected NF1 patients, in order to help differential diagnostics, as well as to collect as large as possible group of patients for a possible genotype-phenotype correlations. Material and methods: Our set of patients consisted of 107 Slovak patients, recruited between 2008 and 2013, who were considered for a diagnosis NF1 by clinical genetics. In order to identify mutations, we employed NF1 cDNA sequencing that enabled us to also detect splicing mutations, as well as MLPA analysis that enables identification of larger deletions. Results: By employing the selected methods in our set of 94 unrelated Slovak patients who fulfilled the basic NF1 diagnostic criteria, we uncovered germline mutations in the NF1gene in 83 of them (88.3%). We observed a high proportion of mutations identified in Slovak population only so far (41/83, 49.4%), and we confirmed de novo mutation in 27/42 tested families (64.3%). Genotype-phenotype correlations revealed an increased incidence of optic pathway glioma in patients with a mutation in the 5´end of the NF1 gene. Conclusion: Combination of cDNA analysis and MLPA provides an effective method for identification of mutations in the NF1 gene. By employing these methods, we were able to also identify frequently incorrectly classified atypical splicing mutations. Clinical geneticists used our results in the differential diagnosis of the disease, especially in contested cases. Úvod: Tři jednonukleotidové polymorfismy (SNP) v NOD2/CARD15 genu (3020insC, R702W, G908R) jsou pro Crohnovu chorobu významným rizikovým faktorem. Jejich diagnostické a terapeutické využití v klinické praxi je však doposud limitované. Cílem studie byla analýza možné asociace těchto polymorfismů s rizikem reoperací a posouzenísouvislosti s fenotypem onemocnění. Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Anamnestická a klinická data, včetně chirurgické léčby byly retrospektivně získány ze zdravotnické dokumentace a prospektivně doplňovány podle protokolu studie. Analyzovány byly souvislosti mezi vybranými genovými polymorfismy, nutností reoperací a fenotypem nemoci. Výsledky: U 24 pacientů (32 %) se našla alespoň jedna sledovaná varianta v NOD2/CARD15 genu. 25 pacientů (33 %) bylo reoperováno, 51 (67 %) představovalo kontrolní skupinu. Trend vyšší četnosti reoperací ve skupině s genovou mutací nedosáhl statistické významnosti (p=0,2688). Dva ze čtyř homozygotů (50 %) nesoucích variantu 3020insC byli opakovaně operováni. Neprokázali jsme signifikantní rozdíly mezi soubory s wild formou a mutovanou formou NOD2/8CARD15 genu s ohledem na věk v době diagnózy (p=0,4356), formu onemocnění (p=0,6610) a lokalizaci (p=0,4747) podle Montrealské klasifikace. Závěr: Polymorfismy v NOD2/CARD genu signifikantně neovlivnily četnost reoperací, homozygoti nesoucí mutaci 3020insC genu však představovali vysoce rizikovou skupinu. Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. The purpose of study was to analyze clinical and genetic polymorphism of Duchenne/Becker progressive muscular dystrophies among patients with neuromuscular diseases in Uzbekistan. 106 male patients with progressive pseudohypertrophic forms of muscular dystrophy were retrospectively and prospectively analyzed in the period from 2004 till 2014: 93 patients with Duchenne PMD aged from 3 years to 18 years and 13 patients with Becker PMD aged from 10 years to 25 years, who had been examined in the medico-genetic consulting department of the Republican Center “Mother and Child Screening” of Tashkent city.
