ABSTRAKT Cieľ: Zámerom štúdie bolo identifikovať štatisticky významné rozdiely v úrovni emočnej inteligencii (EI) u študentov ošetrovateľstva vzhľadom k demografi ckým údajom (vek, dĺžka štúdia) a zároveň zistiť či úroveň EI študentov ošetrovateľstva ovplyvňuje ich záujem o problematiku geriatrického ošetrovateľstva. Metódy: Výskumu sa zúčastnilo 86 univerzitných študentov denného bakalárskeho štúdia odboru ošetrovateľstvo. Na meranie úrovne EI boli využité dotazníky reprezentujúce rôzne chápania EI. Prostredníctvom SIT-EMO (Situational Test of Emotional Understanding) bola zisťovaná EI ako schopnosť. EI ako črta bola zisťovaná dotazníkom SEIS(Schutte Emotional Intelligence Scale). Dotazníkom vlastnej konštrukcie ESE-GP (Emotional self-effi cacygeriatric patient) bola zisťovaná úroveň EI študentov v procese starostlivosti o geriatrických pacientov. Výsledky: Nezistili sme štatisticky významné rozdiely v úrovni EI študentov z hľadiska demografi ckých údajov (vek a ročník štúdia). Medzi úrovňou EI (SEIS)a perspektívnym záujmom študentov venovať sa geriatrickému ošetrovateľstvu bola zistená štatisticky významná súvislosť (p < 0,05). Závery: Zistené výsledky môžu uplatniť najmä inštitúcie poskytujúce ošetrovateľské vzdelávanie, ktorým odporúčame zamerať pozornosť na rozvoj EI v procese vzdelávania študentov ošetrovateľstva., Aim: The aim of the study was to identify statistically significant differences in levels of emotional intelligence (EI) of nursing students in relation with demographic data (age and year of study) and also determine whether the EI level of nursing students affects their interest in the issue of geriatric nursing. Methods: The sample consisted of 86 full-time undergraduate university students of nursing. To measure the level of EI, questionnaires representing diff erent understanding of EI were used. EI as an ability was determined by the SIT-EMO (Situational Test of Emotional Understanding). EI as a trait was evaluated by the questionnaire SEIS (Schutte Emotional Intelligence Scale). The questionnaire of own construction ESE-GP (Emotional self-effi cacygeriatric patient) assessed the level of EI of students in the process of caring for geriatric patients. Results: There were no statistically signifi cant diff erences in the students’ level of EI in terms of demographic data (age and year of study). A statistically signifi cant association (p < 0.05) was found between the students’ level of EI (SEIS) and their promising interest to pursue geriatric nursing. Hyperurikémia v súčasnosti predstavuje novší uznávaný rizikový faktor pre kardiovaskulárne ochorenia. Údaje o jej prevalencii u nás a o jej liečbe doposiaľ v našej populácii sme presne nemali. Z literárnych údajov však vieme, že prevalencia hyperurikémie je rôzna v závislosti od rasových a geografických rozdielov a vyskytuje sa medzi 4–40 %. V hospitalizačných podmienkach jej prevalencia predstavuje približne 7 % a slúži ako dôležitý prediktor mortality, zvlášť pre zlyhanie srdca. Podľa Framinghamských údajov jej relatívne riziko predstavuje až 25 % pre kardiovaskulárne ochorenia, koronárnu chorobu srdca a mortalitu pre všetky príčiny. Z epidemiologického sledovania Zrkadlo Slovensko sme hodnotili hyperurikémiu zo vzorky 20 000 pacientov z ambulancií praktických lekárov na získanie približného obrazu o tomto novšom rizikovom faktore. Prevalence data and its treatment in our patient´s population are still missing. Literature data shows, that its prevalence differs in various populations significantly from 4 % up to 40 % with race and geographical means. In the hospital population its prevalence is about 7 % and represents the important predictor of hospital mortality, e.i with heart failure. From the Framingham data relative risk was estimated of 25 % for cardiovascular diseases, coronary heart disease and all-course mortality. From the epidemiologic survey Mirror Slovakia hyperuricaemia was evaluated from the sample of 20 000 patients from the primare care physicians in order to see the picture on this newer risk factor. Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Cíl: Hlavním cílem výzkumného šetření bylo zjistit, které faktory ovlivňují pouţívání ošetřovatelských diagnóz v klinické praxi. Dalším cílem bylo zjistit rozdíly v hodnocení faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi podle věku, vzdělání a typu pracoviště. Metodika: Pro získání dat byl sestaven nestandardizovaný dotazník zaměřený na zjištění faktorů ovlivňujících pouţívání ošetřovatelských diagnóz. Výzkumný soubor tvořilo 253 registrovaných všeobecných sester z 9 zdravotnických zařízení v České republice. Sestry hodnotili na Likertově škále od 1 do 5, jak jednotlivé faktory ovlivňují pouţívání ošetřovatelské diagnostiky v praxi. Výzkumné šetření probíhalo od června 2011 do listopadu 2011. Výsledky: Podle našeho výzkumného souboru nejvíce ovlivňují pouţívání ošetřovatelských diagnóz v praxi následující faktory: počet pacientů na 1 sestru, administrativní zátěţ, syndrom vyhoření, nedostatek času, rozdíly v teorii a praxi a nedostatek personálu. Statisticky významné rozdíly v hodnocení faktorů podle věku byly zjištěny v hodnocení 2 faktorů: rozdíly v teorii a praxi, nedostatek motivace. Podle dosaţeného vzdělání byly statisticky významné rozdíly zjištěny v hodnocení 4 faktorů: nejednotnost ošetřovatelských diagnóz, terminologie diagnóz, rozdíly v teorii a praxi, rutinní práce. V závislosti na typu pracoviště byly statisticky významné rozdíly nalezeny v hodnocení 5 faktorů: terminologie diagnóz, rozdíly v teorii a praxi, nedostatek praktických zkušeností s ošetřovatelskou diagnostikou, rutinní práce, syndrom vyhoření. Závěr: Rozeznání faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory můţe ulehčit implementaci ošetřovatelské diagnostiky v klinické praxi, Aim: The main aim of the study was to identify which factors influenced the use of nursing diagnoses in clinical practice. Another aim was to ascertain whether age, education and type of workplace affected the relative importance accorded to these factors. Methods: A non-standardized questionnaire on factors influencing the use of nursing diagnoses was administered to a sample consisting of 243 registered general nurses from 9 healthcare facilities in the Czech Republic. Using Likert´s 5-point scale, the nurses rated the importance of several factors in influencing their use of nursing diagnoses in practice. The research was conducted between June 2011 and November 2011. Results: Based on our sample, there are several factors which influence the use of nursing diagnoses in practice. Some of them are: number of patients per nurse, administrative strain, burnout syndrome, lack of time, discrepancies between theory and practice, lack of staff. Statistically significant age-related differences in rating factors were found for the following: discrepancies between theory and practice, lack of motivation. Statistically significant differences in ratings relative to education were found for four factors: disunity of nursing diagnoses, diagnostic terminology, discrepancies between theory and practice, workplace routine. Based on type of workplace, statistically significant differences in ratings were found for five factors: diagnostic terminology, discrepancies between theory and practice, lack of practical experience in nursing diagnostics, workplace routine, burnout syndrome. Studie se zabývá rozhodováním matek o vakcinaci svých dětí proti pneumokokovým nákazám. Danou otázku zkoumá pomocí kvantitativního on-line dotazníkového šetření (n=180) vycházejícího z teoretického rámce teorie plánovaného chování, již také aplikuje jako teoretický model formou logistické regrese. Teorie plánovaného chování se ukazuje jako dobrý prediktivní nástroj, přičemž nejvýznamnější determinantou záměru očkovat je postoj matek vůči očkování proti pneumokokům, jeden ze tří základních konstruktů teorie. Zbylé dva se jako vlivné neprojevily s určitostí (subjektivní norma) či vůbec (vnímaná kontrola chování). Jako vlivné se také ukazuje vnímání hrozby vedlejších účinků vakcíny, jedna z proměnných rozšiřujících původní teoretický model. Výsledky testovaných modelů podporují závěr, že postoj k očkování představuje základní determinantu rozhodnutí, a to zejména pro matky se záměrem neočkovat., The study deals with the issue of mothers' decision-making regarding their child's vaccination against pneumococcal infections. An online survey of mothers (n = 180) was based on the theory of planned behaviour (TPB). A hierarchical logistic regression was used to test the theoretical model with the intention to vaccinate as the dependent variable. The theory proved itself as a strong predictive instrument. The results suggest that attitude is the main predictor of the decision. The subjective norm, as the second construct of the TPB, contributes only in certain cases and the perceived behavioural control was found to have no influence at all. The perception of vaccine-related contradictions is an important factor alongside that of attitude. Cieľ: Cieľom štúdie bolo analyzovať faktory ovplyvňujúce dentálnu hygienu, medzi ktoré patrí konzumácia sladkostí a sladených nápojov u školskej mládeže z hľadiska veku a pohlavia a zároveň analyzovať výskyt dentálnej hygieny počas dňa. Metodika: Súbor respondentov tvorilo 1187 žiakov (620 dievčat a 567 chlapcov) vo veku 11, 13 a 15 rokov v oblasti stredného Slovenska. Zber údajov prebiehal v období máj 2010 až jún 2010. Na zber údajov bol použitý štandardizovaný dotazník vytvorený pre potreby HBSC štúdie (Health Behaviour in School – aged Children). Výsledky boli vyhodnotené Chí-kvadrát testom. Výsledky: Sladkosti a kolu v jednotlivých vekových kategóriách konzumuje každý deň alebo viac krát za deň približne tretina respondentov. Výskyt umývania zubov viac ako raz denne je približne rovnaký vo všetkých vekových kategóriách od 53% u 11 ročných chlapcov do 64% u 15 ročných dievčat. Vo všetkých vekových kategóriách dievčatá udávajú častejšie umývanie zubov v porovnaní s chlapcami. Záver: Výsledky poukazujú na zvýšenú konzumáciu sladkostí a koly počas dňa u školskej mládež stredného Slovenska, čo môže spôsobovať zvýšenú kazivosť zubov. Dentálna hygiena je v našom súbore respondentov nedostačujúca. Na základe získaných výsledkov je možné konštatovať, že potreba zvyšovania úrovne vedomostí nielen detí, ale aj rodičov o zásadách správneho stravovania a o zásadách dentálnej hygieny je neodmysliteľnou súčasťou výchovy k zdraviu a preventívnych opatrení v školskom veku., Aim: The aim of the study was to analyse the factors affecting dental hygiene such as consumption of sweets and sweet soft drinks in school-aged pupils in terms of their age and gender and at the same time to analyse the occurrence of dental hygiene during the day. Methods: The set of respondents consisted of 1187 pupils (620 girls and 567 boys) at the age of 11, 13 and 15 years in the area of central Slovakia. The data collection was conducted in the period of May 2010 to June 2010. For data collection was used standardized questionnaire for HBSC studies (Health Behaviour in School–aged Children). The results were analysed by Chi-square test. Results: Approximately one third of respondents in each category consumes sweets and Coca-cola every day or several times a day. The occurrence of cleaning the teeth more than once a day was equally proved in all age groups ranging from 53% in 11 years old boys to 64% in 15 years old girls. The girls of all age groups indicate more frequent cleaning of teeth in comparison with boys. Conclusion: The results show increased consumption of sweets and Cocacola during the day in the school youth from central Slovakia, which may cause an increase in tooth decay. Dental hygiene in our set of respondents is insufficient. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Sleep disturbances are one of the most common non-motor symptoms in patient with Parkinson’s disease (PD) with community‑based studies reporting prevalence data of 60%. Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05.
