Stenóza cervikálního kanálu může vést ke kompresi krční míchy a představuje nejvýznamnější mechanický faktor v patofyziologii cervikální spondylogenní myelopatie. Míšní tkáň je však vůči kompresi poměrně odolná a k rozvoji manifestní myelopatie dochází až při vyšším stupni komprese a v kombinaci s dalšími patofyziologickými faktory, zejm. dynamickým faktorem a zevním traumatem. Asymptomatická spondylogenní komprese krční míchy (ASCCC) je velmi častým nálezem zvláště v populaci vyššího věku nad 50 let. Objektivizace a kvantifikace cervikální stenózy a komprese krční míchy jsou však nejednotné a nejsou jasně stanoveny prediktory progrese do manifestní myelopatie a s tím související indikace případné preventivní chirurgické dekomprese. Přehled obsahuje nejčastěji používané metody stanovení cervikální stenózy a cervikální míšní komprese pomocí zobrazovacích metod. Nativní radiogram a výpočetní tomografie mají význam v objektivizaci cervikální stenózy, zatímco dominantní metodou objektivizace míšní komprese je magnetická rezonance (MR). Z parametrů, které nejvíce korelují s klinickou manifestací komprese, jsou uváděny příčná plocha míchy v místě komprese a přítomnost intenzitních změn míšního signálu. Z novějších zobrazovacích modalit je nejcitlivější MR zobrazení tenzorů difúze. Z prognostických faktorů rozvoje manifestní myelopatie u ASCCC jsou obecně akceptovány přítomnost manifestní radikulopatie a abnormita motorických a senzomotorických evokovaných potenciálů. Význam zobrazovacích metod v predikci rozvoje manifestní myelopatie a benefit provedení preventivní dekomprese ve skupině nemocných s asymptomatickou kompresí a vyšším rizikem rozvoje manifestní myelopatie bude nutné ověřit dalšími studiemi., Cervical spinal stenosis may lead to cervical cord compression and represents the most important mechanical factor in pathophysiology of cervical spondylotic myelopathy. Medullar tissue is, however, rather resistant to compression and development of symptomatic myelopathy occurs only when higher degree stenosis is present and in combination with other pathophysiological factors, mainly dynamic compression and trauma. Asymptomatic spondylotic cervical cord compression (ASCCC) is a quite frequent finding in older population above the age of fifty. However, reliability of the methods used to verify and quantify cervical stenosis and cervical cord compression is low and clear predictors of development of symptomatic myelopathy and related indication of potential preventive surgical decompression in ASCCC have not been determined yet. The overview discusses the most frequently used methods to establish cervical spinal stenosis and cervical cord compression using imaging methods. Radiogram and especially computed tomography are important for verification of cervical spinal stenosis, while magnetic resonance imaging (MRI) is a preferable method to detect cervical cord compression. The cross‑sectional spinal cord area and T2 MRI spinal cord hyperintensity are among the parameters considered to be the most closely correlated with clinical manifestation of spinal cord compression. Among newly introduced imaging modalities, MRI diffusion tensor imaging seems to be the most promising one. The presence of symptomatic radiculopathy and abnormality of motor and somatosensory evoked potentials are among generally accepted predictors of symptomatic myelopathy. The importance of imaging methods as predictors of symptomatic myelopathy development as well as benefits of preventive surgical decompression in ASCCC individuals with high risk of developing symptomatic myelopathy is to be established in future studies. Kyselina močová je u človeka biologicky aktívny koncový produkt metabolizmu purínov. Zvýšené koncentrácie kyseliny močovej sprevádzajú metabolický syndróm a jeho komponenty už u detí a mladistvých. K hyperurikémii prispieva konzumácia nápojov prisládzaných fruktózou, nakoľko je jediným sacharidom, pri ktorého metabolizme vzniká kyselina močová. Súčasné vedomosti neumožňujú odlíšiť, či je hyperurikémia kauzálnou príčinou komponentov metabolického syndrómu, alebo len sprievodným znakom naznačujúcim zvýšené riziko vzniku kardiovaskulárnych ochorení. V súčasnosti neexistuje konsenzus či a ako liečiť asymptomatickú hyperurikémiu u detí a mladistvých. Zaujať zásadný postoj k týmto otázkam umožnia len výsledky randomizovaných kontrolovaných prospektívnych štúdií, ktoré poskytnú dôkazy o prospešných účinkoch tejto liečby na prevalenciu metabolického syndrómu a incidenciu kardiovaskulárnych chorôb., In humans, uric acid represents a biologically active end-product of purine nucleotides. Several studies in children and adolescents documented an association between hyperuricaemia and the components metabolic syndrome. High intake of fructose-sweetened beverages might increase uricaemia, since fructose is the only saccharide which metabolism results in the formation of uric acid. Current knowledge does not allow distinguishing whether hyperuricaemia is causally related to the components of metabolic syndrome, or rather represents a marker of an enhanced risk, and poor outcome. No guidelines exist whether or not to treat asymptomatic hyperuricaemia in the adolescents. Ateroskleróza jako zánětlivé postižení cévní stěny má více forem, které se vyskytují většinou současně. Klasická aterosklerotická léze charakterizovaná akumulací lipidů v subendoteliálním prostoru bývá často provázena změnami v hlubších vrstvách arteriální stěny, které jsou typické zmnožením extracelulární matrix a aktivací hladkosvalových buněk. Vlivem skladby rizikových faktorů může dominovat jeden nebo druhý typ postižení. Zatímco v patogenezi klasické formy aterosklerózy hrají zásadní roli aterogenní lipoproteiny (především třídy LDL), při rozvoji změn arteriální medie to jsou jiné rizikové faktory, např. hyperaktivita systému renin-angiotenzin-aldosteron (RAS). Ovlivněním těchto dvou základních mechanizmů prokazatelně zpomalujeme progresi cévních změn a příznivě ovlivňujeme prognózu nemocných. Důležitý je i fakt, že současné působení na tyto faktory má synergické působení doložené na úrovni experimentální i klinické. Důsledné využití možností nabízené intenzivním snižováním hladin aterogenních lipidů i nadměrné aktivity systému RAS snižuje riziko typických aterotrombotických komplikací (akutní koronární syndrom) i příhod podmíněných větší měrou změnami arteriální medie či hypertrofií levé komory srdeční (maligní arytmie, srdeční selhání). Tyto dva směry tak představují předpoklady úspěchu kardiovaskulární prevence. Classical atherosclerotic vascular lesion characterised by lipid accumulation in the subendothelial space is frequently accompanied by changes in deeper layers of arterial wall, in which increased extracellular tissue mass and smooth muscle cells activation represent the most prominent feature. Due to a specific constellation of risk factors the first or second pathology may be more expressed. While initiation and progression of classical atherosclerosis are mostly driven by lipoproteins (especially of LDL class) the most important factor of arterial media changes seem to be different risk factors e.g. hyperactivity of renin-angiotensin-aldosterone system (RAS). Influencing these two basic pathogenic mechanisms undoubtedly slows down the course of vascular changes and impacts positively on the prognosis of the patients. It is noteworthy, that simultaneous targeting of both of these mechanisms yields synergistic effects as evidenced both by experimental and clinical works. Using the opportunities offered by intensive lowering of atherogenic plasma lipids and over activation of the RAS system reduce not only the incidence of typical atherotromobotic complications (e.g. acute coronary syndrome) but also the events caused by changes of medial part of arterial wall or left myocardial ventricle (malignant arrhythmia, heart failure). These two strategies represent necessary conditions for successful cardiovascular prevention. Autoimunitní encefalitidy jsou akutní či subakutní monofázická nebo progresivní zánětlivá onemocnění CNS podmíněná autoimunitními mechanizmy (působením patogenních autoprotilátek nebo autoagresivních efektorových buněk). Mezi autoimunitní encefalitidy lze řadit klasické paraneoplastické syndromy s postižením CNS, limbické encefalitidy, anti‑NMDAR encefalitidu, Morvanův syndrom, Rasmussenovu encefalitidu, onemocnění z okruhu Stiff‑Person Syndromu včetně progresivní encefalomyelitidy s rigiditou a myoklonem, akutní autoimunitně podmíněné extrapyramidové poruchy hybnosti, encephalitis lethargica, neurologické syndromy se zvýšenou senzitivitou vůči glutenu a další. V posledních letech prodělala tato oblast neuroimunologie prudký rozvoj. Byly popsány nové syndromy, upřesněny klinické projevy a etiopatogenetické mechanizmy některých již známých chorob a identifikována řada nových typů protilátek. Tato minimonografie si klade za cíl podat ucelený přehled současných znalostí v oblasti autoimunitních encefalitid a umožnit tak klinikům snazší orientaci v této oblasti neurologie., Autoimmune encephalitis is a term used to describe a group of acute or subacute monophasic or progressive inflammatory CNS disorders with autoimmune pathogenesis (i.e. induced by pathogenic autoantibodies or autoreactive effector cells). This group includes such diverse conditions as paraneoplastic CNS syndromes, limbic encephalitis, anti-NMDAR encephalitis, Morvan’s syndrome, Rasmussen encephalitis, stiff-person syndrome spectrum disorders including progressive encephalomyelitis with rigidity and myoclonus, autoimmune movement disorders, encephalitis lethargica, neurological syndromes with increased sensitivity to gluten, etc. This area of neuroimmunology has evolved dynamically during the past 10 years. Many new syndromes have been described, understanding of pathogenesis of the already known syndromes has improved and new antibodies were identified. This review aims to provide an overview of current knowledge in the field of autoimmune encephalitis in order to help clinicians navigate the rough waters of this exciting area of neurology. Autoimunitní forma pankreatitidy patří mezi specifickou formu onemocnění slinivky břišní s významným podílem autoimunitní složky v etiopatogenezi. V současné době existují 2 formy onemocnění, které jsou definované klinicky i histomorfologicky a jsou laboratorně popsány. Přes řadu společných rysů histomorfologických se obě formy zásadně odlišují přítomností tzv. endoteliálních granulárních lézí (GEL) u typu 2, resp. absencí GEL u typu 1. Abundantní přítomnost gamaglobulinu a imunoglobulinu G4 charakterizuje 1. formu nemoci, podobně jako častá přítomnost IgG4 pozitivních extrapankreatických lézí. Subtyp 2 bývá typicky spojen s nálezem idiopatických střevních zánětů, především ulcerózní kolitidy. Oba typy autoimunitní pankreatitidy se vyznačují rychlou odpovědí na terapii steroidy. Vzhledem k různým diagnostickým kritériím autoimunitní formy pankreatitidy byl v roce 2011 zveřejněn diagnostický konsenzus sjednocující stávající kritéria a vycházející z klinických symptomů, biochemických nálezů, nálezů při použití zobrazovacích metod, histomorfologie žlázy a pozitivní odezvy na podání steroidů. Stálým problémem zůstává včasná diferenciální diagnostika mezi fokální formou autoimunitní pankreatitidy a karcinomem hlavy pankreatu. MRI/CT, MRCP a v Japonsku ERCP, s cílenou biopsií žlázy pod EUS kontrolou, jsou doporučeny jako metody volby., The autoimmune type of pancreatitis represents the specific disease of pancreas, with significant contribution of autoimmune processes in its etiopathogenesis. Currently, there are two proved subtypes of this particular pancreatopathy, which are defined clinically, histomorphologically and serologically. They have many histomorphological signs in common, but differ in the presence of so-called granulocytic epithelial lesions (GEL), which are absent in subtype 1. The subtype 1 is characterized by the presence of gammaglobulines, esp. immunoglobuline G4 and IgG4 positive extrapancreatic lesions. The subtype 2 is typically associated with the inflammatory bowel diseases, esp. ulcerative colitis. But the common characteristic of both subtypes is the fact response to applied steroid treatment. Due to diverse diagnostic criteria in the past, in 2011 the consensus for the diagnosis of autoimmune pancreatitis was announced. It is based on clinical symptoms, biochemical results, the results got by using of imaging methods, histomorphology and positive response to steroid treatment. The matter to be solved is the question of early differential diagnosis between focal autoimmune pancreatitis and adenocarcinoma of pancreatic head. Dysfunkce autonomního nervového systému patří k častým klinickým projevům roztroušené sklerózy (RS). Incidence autonomní dysfunkce (AD) u pacientů s RS se v různých studiích pohybuje mezi 16 a 80 % a jednoznačně se zvyšuje s délkou trvání RS a progredující disabilitou. Může se však projevit v kterékoli fázi demyelinizačního onemocnění a dokonce může být jeho prvním klinickým projevem. AD významně ovlivňuje kvalitu života pacientů s RS a přispívá k celkové disabilitě. Přesto postižení autonomního nervového systému patří mezi stále relativně poddiagnostikované symptomy tohoto onemocnění. Nejčastějším a nejzávažnějším typem AD u pacientů s RS je narušení funkce kardiovaskulárního a/nebo urogenitálního systému. Méně častá je dysfunkce gastrointestinální, sudomotorická či narušení pupilomotoriky. Autonomní (především kardiovaskulární) dysfunkce u pacientů s RS také úzce souvisí s rozvojem únavy. Cílem tohoto sdělení je poskytnout přehled jednotlivých typů AD u pacientů s RS a možností jejich diagnostiky., Autonomic nervous system dysfunction (AD) represents a frequent clinical presentation of multiple sclerosis (MS). According to published studies, the incidence of AD in MS patients ranges between 16 and 80% and gradually increases with the length of the demyelinating disease and with the progression of disability. However, AD can occur in any phase of MS and can even represent its first symptom in some patients. Autonomic dysfunction has a significant negative impact on the quality of life in MS patients and contributes to overall disability. Even so, autonomic nervous system dysfunction is frequently underdiagnosed in patients with multiple sclerosis. Cardiovascular and urogenital dysfunction are the most frequent types of AD in MS patients with the highest impact on their clinical status and the quality of life. Less frequently, gastrointestinal, sudomotor or pupilomotor dysfunction can be found. Autonomic (in particular cardiovascular) dysfunction in multiple sclerosis is closely related to fatigue, another frequent clinical symptom in MS patients. The aim of this paper is to sum up the spectrum of symptoms of autonomic dysfunction in multiple sclerosis and its diagnostic tools. Centrální a periferní nervový systém jsou odděleny od krevního řečiště bariérami, které brání volnému přechodu ve vodě rozpustných molekul prostřednictvím těsných spojů, jež propojují endoteliální buňky a epiteliální buňky plexus chorioideus. Tyto bariéry též hrají roli v influxu esenciálních molekul a odstraňování xenobiotik. V posledních letech jsou objasňovány rozdíly a společné rysy jednotlivých bariérových systémů. Laparoscopic cholecystectomy (LC) is the treatment of choice for gall stones but is associated with an increased risk of bile duct injury (BDI). A BDI detected during LC may be repaired if a biliary surgeon is available but the easiest and safest option for a general surgeon is to place drains in the subhepatic fossa to covert the acute BDI into a controlled external biliary fistula (EBF). Most BDIs are diagnosed in the postoperative period and result in bile leak. Treatment is with percutaneous catheter drainage and endoscopic stenting of the bile duct; early repair is not recommended. Repair, in the form of hepatico-jejunostomy (HJ), should be performed by a biliary surgeon after a delay of 4−6 weeks when the EBF has closed. BDI is a common cause of medico-legal suit and a large burden on healthcare costs. Most BDIs can be prevented by adhering to the principles of safe cholecystectomy., Laparoskopická cholecystektomie (LC) je léčbou volby u žlučových kamenů, avšak je spojená se zvýšeným rizikem poranění žlučových cest (bile duct injury − BDI). Pokud je BDI detekováno během LC, může se řešit ihned, pokud je dostupný hepatobiliární chirurg, ale nejjednodušší a nejbezpečnější postup pro všeobecného chirurga je umístění drénů do podjaterní krajiny a převedení akutní BDI na řízenou zevní biliární píštěl (external biliary fistula − EBF). Většina BDI jsou diagnostikované až v pooperačním období, kdy vzniká biliární leak. Léčbou je drenáž perkutánním katetrem a endoskopické zavedení stentu do žlučovodu; časná reparace se nedoporučuje. Reparace ve formě hepatikojejunostomie (HJ) by měla být provedena hepatobiliárním chirurgem s odstupem 4−6 týdnů poté, co se uzavře EBF. BDI jsou častou příčinou zdravotnicko-právních žalob a významně zatěžují náklady ve zdravotnictví. Většině BDI se dá předejít dodržováním principů bezpečné cholecystektomie., and V. K. Kapoor
