AIMS: Our aims were to isolate and cultivate mesenchymal dental pulp stem cells (DPSC) in various media enriched with human blood components, and subsequently to investigate their basic biological properties. METHODS: DPSC were cultivated in five different media based on α MEM containing different concentrations of human plasma (HP), platelet-rich plasma (PRP), or fetal calf serum (FCS). The DPSC biological properties were examined periodically. RESULTS: We cultivated DPSC in the various cultivation media over 15 population doublings except for the medium supplemented with 10% HP. Our results showed that DPSC cultivated in medium supplemented with 10% PRP showed the shortest average population doubling time (DT) (28.6 ± 4.6 hours), in contrast to DPSC cultivated in 10% HP which indicated the longest DT (156.2 ± 17.8 hours); hence this part of the experiment had been cancelled in the 6th passage. DPSC cultivated in media with 2% FCS+ITS (DT 47.3 ± 10.4 hours), 2% PRP (DT 40.1 ± 5.7 hours) and 2% HP (DT 49.0 ± 15.2 hours) showed almost the same proliferative activity. DPSC's viability in the 9th passage was over 90% except for the DPSC cultivated in the 10% HP media. Metodika: Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (IM), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Ve větvi se simvastatinem v monoterapii byl cílový LDL‑c < 70 mg/dl. Předpokladem bylo, že ezetimib sníží LDL‑c o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo nonST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října 2005 do července 2010. Pacienti s nonSTEMI/NAP měli častěji diabetes mellitus, hypertenzi a předchozí IM. Medián LDL‑c při randomizaci byl 100 mg/dl u STEMI a 93 mg/dl u nonSTEMI/NAP. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %) (p = 0,016) léčených kombinací. Methods: The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, re‑hospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postran‑domization). The simvastatin monotherapy arm's LDL‑C target was < 70 mg/dL. Ezetimibe was assumed to further lower LDL‑C by 15 mg/dL and produce an estimated ~8% to 9% treatment effect. The targeted number of events was 5,250. Heart attacks alone were reduced by 13 %, and non-fatal stroke by 20 %. Deaths from cardiovascular disease were statistically the same in both groups. Patients were followed an average of approximately six years, and some as long as 8.5 years. Karotická endarterektomie byla dlouho považována za zlatý standard v léčbě a prevenci ischemické cévní mozkové příhody u pacientů s významnou stenózou vnitřních karotických arterií. Karotický stenting, i přes počáteční limitace, dosahuje v současné době výsledků srovnatelných s karotickou endarterektomií a u selektovaných pacientů je považován za rovnocennou volbu. Bezpečnost obou výkonů je dána zejména rizikem periprocedurálních komplikací, které jsou významně ovlivněny zkušenostmi operatéra. Transkraniální dopplerovská ultrasonografie je zavedenou diagnostickou a monitorovací metodou, jejíž využití přispívá ke zkvalitnění péče o pacienty podstupující karotické intervence. Tento článek shrnuje znalosti týkající se invazivní léčby karotických stenóz se zaměřením na možnosti optimalizace pomocí transkraniální dopplerovské ultrasonografie., Surgical carotid endarterectomy was for a long time considered the standard approach to the treatment of atherosclerotic carotid artery disease. Despite its initial limitations and slow acceptance, carotid artery stenting has evolved into an elaborate method currently considered to be equivalent and in some patients even preferable to carotid endarterectomy. Given the fact that the long‑term outcomes of carotid artery stenting and carotid endarterectomy are comparable, the major risk of both procedures is derived from the periprocedural complications, which are highly operator dependent. Transcranial Doppler ultrasound is a well‑established diagnostic and monitoring tool allowing optimising the care of patients with carotid artery disease. Carnosine is a dipeptide formed of the amino acids β-alanine and histidine. Only a limited number of studies have examined the effects of carnosine on sympathetic nerve activation and anxiety. The present study was undertaken to determine the dose-related effects of carnosine on anxiety in the elevated T-maze test (ETM) with electrodermal activity (EDA). Carnosine was injected in three groups of rats with doses of 10 (low dose), 100 (medium dose) and 1000 (high dose) mg/kg i.p. Physiological saline was injected in the sham group. The anxiety scores of the rats were measured with ETM 20 minutes after injection. Then, SCL was measured. The decreased number of entries into the open arm (NEOA), the percentage of time spent in the open arm (% TSOA) and higher EDA [shown by skin conductance level (SCL)] indicate higher anxiety. The NEOA and % TSOA were lower in the high-dose group than in the other groups. SCL was lower in the medium-dose carnosine group than in the high-dose carnosine and sham groups. SCL was higher in the high-dose group than in the medium-dose and sham groups. Our results suggest that high-dose carnosine produced anxiety-like effects as assessed in the SCL and ETM. Inzulinovou rezistenci (IR) definujeme jako stav, při němž normální koncentrace volného plazmatického inzulinu vyvolává sníženou odpověď organizmu. V užším slova smyslu IR chápeme jako poruchu účinku inzulinu v cílové struktuře, která může vzniknout na kterékoli úrovni inzulinové signalizační kaskády. V klinických podmínkách ji obvykle definujeme jako poruchu účinku inzulinu v metabolizmu glukózy, i když platí, že porucha se může se týkat různých účinků inzulinu a různých buněčných struktur. Charakteristickým rysem IR svázané s metabolickým syndromem či diabetem 2. typu je defektní signalizace, která postihuje PI3-kinázovou větev inzulinové signalizační kaskády. Další účinky inzulinu, které závisí na signalizaci vedoucí přes Ras komplex a MAP-kinázu, nemusí být postiženy. Vlivem kompenzatorní hyperinzulinemie mohou být dokonce zvýšeny. Článek shrnuje některé novější poznatky týkající struktury a regulací inzulinové signalizační kaskády a rozebírá vybrané primární a sekundární příčiny IR, které zahrnují faktory genetické a epigenetické, regulační roli mikroRNA a metabolické, humorální a imunologické faktory. Detailní poznání příčin IR nabízí možnosti její racionální léčby. Ta se v současné době opírá o léčbu odstranitelných příčin IR, tj. důslednou kompenzaci diabetu, redukci hmotnosti, režimová opatření (dieta, fyzická aktivita), přehodnocení potřeby léčby kortikoidy, léčbu přidružených onemocnění, a případně podání metforminu či pioglitazonu. In the narrower sense we understand IR as the impairment of insulin action in the target structure which may arise at any level of the insulin signalling cascade. In the clinical conditions we usually define it as the impairment of insulin action in glucose metabolism, although it is true that the impairment may concern different effects of insulin and different cell structures. The characteristic feature of IR linked to the metabolic syndrome or Type 2 diabetes is defective signalling which affects PI3-kinase branch of insulin signalling cascade. Other insulin actions depending on the signalling through the Ras complex and MAP-kinase, may not be affected. Due to compensatory hyperinsulinemia they may be even increased. The article summarizes some recent findings regarding the structure and regulation of insulin signalling cascade and analyses selected primary and secondary causes of IR which include genetic and epigenetic factors, the microRNA regulation role, metabolic, humoral and immunological factors. The detailed knowledge of the causes of IR opens possibilities of its rational treatment. This is currently based on the treatment of curable causes of IR, i.e. consistent compensation of diabetes, weight reduction, regimen arrangements (diet, physical activity), re-assessment of the need to use corticosteroids in therapy, treatment of coexisting conditions and possibly administration of metformin or pioglitazone. Syndrom renálních cyst a diabetu (RCAD), zvaný také HNF1B-MODY či MODY5, je způsoben delecí nebo bodovou mutací genu HNF1B, která vede k depleci transkripčního faktoru HNF1B. Hlavními klinickými součástmi RCAD jsou cystické onemocnění ledvin nebo jiné vývojové anomálie ledvin a diabetes mellitus, který se projeví nejčastěji ve 2. dekádě života nebo později. Poruchy ledvin mohou vést k rozvoji chronické renální insuficience už v dětství nebo mladé dospělosti. K dalším příznakům patří poruchy funkce jater – cholestatický ikterus ve středním věku, ale někdy už novorozenecká cholestáza, atrofie pankreatu s poruchou exokrinní sekrece a některé vrozené anomálie pohlavního ústrojí. Na rozdíl od ostatních forem MODY diabetu rodinná anamnéza nemusí být pozitivní, protože většina odchylek HNF1B vzniká de novo. Na RCAD pomýšlíme zejména u dospělých osob s diabetem a cystickým onemocněním ledvin nebo cholestatickým ikterem a u dětí s cystickým onemocněním ledvin nejasné etiologie, i bez přítomnosti diabetu. Podpůrným nálezem může být hypomagneziemie, která se vyskytuje až u 70 % nositelů poruchy genu HNF1B, a hyperurikemie. Klíčová slova: diabetes mellitus – cholestatický ikterus – HNF1B – MODY – RCAD, The renal cysts and diabetes syndrome (RCAD), also known as HNF1B-MODYor MODY5, is caused by the deletion or point mutation of HNF1B gene which leads to the depletion of HNF1B transcription factor. The main clinical components of RCAD include cystic kidney disease or other developmental anomalies of the kidneys and diabetes mellitus which typically manifests in the second decade of life or later. Renal disorders may lead to the development of chronic renal insufficiency already in childhood or young adulthood. The other symptoms include hepatic impairment - cholestatic jaundice in middle-aged patients, sometimes even neonatal cholestasis, atrophy of the pancreas with the impairment of exocrine pancreatic secretion and some congenital anomalies of the genital tract. As opposed to the other forms of MODY diabetes, the family history may not be positive because most of the deviations of HNF1B appear de novo. We associate RCAD in particular with adults suffering from diabetes and cystic kidney disease and/or cholestatic jaundice and children with cystic kidney disease of unclear etiology, even without the presence of diabetes. A supportive finding may be hypomagnesemia which occurs in up to 70 % of patients diagnosed with HNF1B related disease and hyperuricemia. Srdeční selhání patří mezi závažné zdravotní problémy tohoto století vedoucí zejmena k častým rehospitalizacím, snižuje též kvalitu života a zhoršuje prognózu nemocných. Při progresi chronického srdečního selhání dochází často k narůstající neschopnosti srdce splňovat metabolické nároky většiny orgánů. Zatímco důsledky srdečního selhání na renální funkce (kardiorenální syndrom) byly v literatuře mnohokrát popsány, komplexní popis dopadu srdečního selhání na jaterní funkce (kardiohepatální syndrom) včetně klinického průběhu, biochemických profilů a histologických nálezů v naší literatuře stále chybí, podobně jako popis významu užití laboratorní kontroly jaterních funkcí jakožto prognostického ukazatele u pacientů se srdečním selháním., Heart failure is a major health problem of this century, with frequent repeated hospitalizations, deteriorated quality of life and worsened prognosis. Úvod: Kardiovaskulární rehabilitace je v současné době standardní součástí léčby nemocných po akutní koronární příhodě. Intervenční tréninkový kardiovaskulární rehabilitační program je součástí II. fáze kardiovaskulární rehabilitace, která je klíčovým bodem v celém sekundárně‑preventivním procesu u nemocných s ischemickou chorobou srdeční. Dochází zde k hemodynamické adaptaci pacienta na běžnou fyzickou zátěž, k postupnému zvyšování aerobní kapacity a k osvojování si principů pravidelného aerobně‑odporového tréninku. Cíl: Předložená práce se zaměřuje na posouzení vlivu modifikovaného aerobně‑odporového tréninku na kardiorespirační ukazatele u nemocných po akutní koronární příhodě. Soubor a metodika: Do studie bylo zařazeno 106 pacientů (85 % mužů) průměrného věku 60,4 ? 10,9 let s ejekční frakcí levé komory 57,4 ? 7,2 %. Jednalo se o pacienty po akutním koronárním syndromu. Doba od vzniku akutní koronární příhody do zahájení tréninkového programu byla 35 ? 8 dnů, u nemocných po aortokoronárním bypassu 50 ? 16 dnů. Všichni pacienti podstoupili dvouměsíční aerobně‑odporový trénink s frekvencí 3× týdně. Tréninková jednotka trvala 100 min (z toho 60 min vlastní aerobní trénink). Výsledky: Absolvování intervenčního tréninkového programu vedlo k signifikantnímu nárůstu pracovní tolerance (1,8 ? 0,3 vs 2,0 ? 0,4 W/kg; p < 0,001) a vrcholové spotřeby kyslíku (22,8 ? 4,5 vs 25,9 ? 5,5 pVO2; p < 0,001), taktéž byl zaznamenán nesignifikantní pokles klidových hodnot tepové frekvence a systolického a diastolického krevního tlaku. Závěr: Modifikovaný intervenční tréninkový program vede ke zlepšení aerobní kapacity, která je jedním z významných prognostických ukazatelů u nemocných po akutní koronární příhodě., Introduction: Cardiovascular rehabilitation is currently part of standard therapy in patients after acute coronary syndrome. The interventional cardiovascular rehabilitation training program is a part of the second rehabilitation phase, which is a key point in all secondary‑preventive processes in patients with coronary artery disease. Patients are hemodynamically adapted to normal physical exercise, their aerobic capacity is gradually increased, and they learn about the principles of regular aerobic‑resistance training. Design: This study is focused on assessing the impact of modified aerobic‑resistance training on cardiorespiratory parameters in patients after an acute coronary event. Methods: The study included 106 patients (85% men), mean age 60.4 ? 10.9 years, with a left ventricular ejection fraction 57.4 ? 7.2%. The time form acute coronary event to the start of the training program was 35 ? 8 days; in patients after coronary artery bypass graft this was 50 ? 16 days. All patients completed a 2-month aerobic‑resistance training program with 3 sessions a week. A training session lasted 100 min (including 60 min of aerobic training). Results: The completion of the intervention training program led to a significant increase in work tolerance (1.8 ? 0.3 vs. 2.0 ? 0.4 W/kg; p < 0.001) and peak oxygen consumption (22.8 ? 4.5 vs. 25.9 ? 5.5 pVO2; p < 0.001). Other results observed were a non‑significant decrease in resting heart rate, systolic and diastolic blood pressure values. Karotická endarterektomie (CEA) je s ohledem na svou dlouhou historii a přiznivé vysledky tradičně zvažována jako metoda první volby v léčbě a prevenci cévní mozkové příhody u pacientů s významnou stenózou vnitřních karotických tepen. Díky pokroku katetrizačních technik je však možno mnohým z těchto pacientů nabízet karotický stenting, který je minimálně invazivní a při dobré selekci nabízí srovnatelné dlouhodobé výsledky s velmi nízkým rizikem periprocedurálních komplikací.
