In this paper a longitudinal study is presented of age-dependent speech acoustic characteristics from the utterances of Czech actresses, acquired from movie databases recorded in the years from 1985 to 2014. The age dependence of phonation is analyzed using the parameters fundame ntal frequency, jitter, and shimmer. For assessment of articulation, the formant-based voice area index of vocals and the length of the burst from apalatal stop consonant isused. For prosody, no characteristics were found that were applicable to the analysis of film archives. The results of age dependencies described by the phonetic categories are consistent with the assumptions and conclusions presented in the literature. Cíl: Některé kategorie migrantů ze třetích zemí mají v České republice omezený přístup ke zdravotní péči. Cílem této studie je popsat a analyzovat zkušenosti migrantů ze třetích zemí s jejich přístupem ke zdravotní péči a využíváním alternativních strategií k zajištění zdraví. Metody: Kvalitativní design za využití semistrukturovaných rozhovorů. Bylo realizováno dvacet čtyři rozhovorů s migranty ze třetích zemí různých národností a s různými typy povolení k pobytu na území České republiky. Výsledky: Migranti v praxi čelí četným problémům v přístupu ke zdravotní péči. Byly identifikovány hlavní bariéry přístupu ke zdravotní péči: omezený právní nárok na veřejné zdravotní pojištění, nedůvěra a špatná informovanost pojištění, nedůvěra a špatná informovanost zdravotnických pracovníků, přístup komerčních zdravotních pojišťoven založený na tržních principech, jazyková bariéra a nedostatek znalostí cizinců o českém zdravotnictví. Tyto překážky byly příčinou následujících alternativních strategií migrantů k zajištění zdraví: kontaktování nebo využívání lékařů v zemích původu, platby v hotovosti, odkládání zdravotní péče, návštěvy známých lékaře nebo lékařů s migračním původem a samoléčba. Závěr: Některé kategorie migrantů ze třetích zemí čelí formálním a neformálním překážkám využívání českého zdravotnického systému, které jsou příčinou alternativních strategií zajišťování zdraví. Ty mohou vést k nepříznivým účinkům na jejich zdraví. Tato studie ukazuje, že v souladu s mezinárodními lidskými právy je potřeba implementovat změny v rámci veřejné ev. zdravotní politiky a zajistit tak přístup ke zdravotní péči pro všechny legální migranty ze třetích zemí v České republice., Background: Some categories of third countries´ migrants in the CR have restricted access to healthcare. The aim of this study is to describe and analyse migrants´ experiences of access to health- -care. Methods: Qualitative design using semi-structured interviews with twenty four migrants in the Czech Republic. Results: Some categories of third countries‘ migrants face formal and informal barriers to the Czech healthcare system, which leads to alternative health-seeking strategies such as visiting doctors in countries of origin (so called salmon bias), payments in cash, postponing the health care, visiting the known physicians or physicians with migration background and self-medication. Background: In evidence-based education, experts are supposed to possess skills and content knowledge. Scientific skills provide the tools and ways of thinking that enable researches to build the robust conceptual frameworks needed to gain expertise. Science education should not only provide broad content knowledge base but also develop analytical thinking, understanding of scientific research processes, and inspire curiosity of researchers. Objectives: is paper discusses the key results of subjective evaluation of attained outcomes of a 2,5-year project focused on enhancing research competences in a faculty of health sciences. Methods: e data was gathered from project members by four consequent semi-structured online questionnaires. Total number of respondents was 78 (N = 78) with 75% response rate. e data was analysed by a summative content analysis combining quantitative and qualitative analyses. Results: e findings present evident progress in personal achievements, team working, and scientific practice in the faculty. Based on identified needs the concrete measures supported learning and lead to enhancement in the target group’s research competences. Conclusions: Capacity building approach was successful in laying a robust foundation for consistent development in research-based practice both in academic and professional levels. Hodnocení a sledování průběhu veřejně politických opatření (včetně politiky podpory zdraví) je, zejména v rámci diskurzu na poli veřejné politiky jako vědní disciplíny, chápáno jako nedílná součást tvorby a realizace veřejné politiky, která by měla přispívat k naplňování demokratických principů ve veřejné správě. Vlastní evaluační praxe, a tedy i konečný smysl hodnocení, se však může značně lišit od odborných akademických metodologických doporučení a deklarací v rámci strategických veřejně politických dokumentů. Cílem předkládaného článku je na základě systematické rešerše vytvořit teoretický rámec pro exploraci a porozumění evaluační praxi v politice podpory zdraví na národní úrovni (včetně podmínek, které na ni mohou působit) a představit zjištění některých zahraničních autorů, kteří zkoumali jednotlivé případy evaluační praxe., Public policy (including health promotion policy) evaluation and monitoring is considered, especially in the discourse on the ground of public policy as an academic discipline, to be an integral part of public policy creation and implementation, which should contribute to promotion of democratic principles in public administration. The particular evaluation practice and hence the very meaning of evaluation, may differ greatly from academic methodological recommendations and declarations included in strategic public policy documents. Stárnutí populace má ekonomické, politické i sociální důsledky. Na demografické změny reaguje plánování potřeb dlouhodobé péče včetně úvah o počtu profesionálních pečujících. Ačkoli v řadě zemí Organizace pro hospodář skou spolupráci a rozvoj (OECD) včetně ČR převládá péče rodinných pečujících, množství profesionálů se má v těchto zemích do roku 2050 zdvojnásobit (v ČR dokonce ztrojnásobit). Potřeba existence kvalitních institucí zajišťujících dlouhodobou péči je provázána s nastavením příznivých pracovních podmínek a podporou dobré zdravotní kondice jejich zaměstnanců. Prevence a podpora zdraví jak stárnoucí populace, tak formálních i neformálních pečujících, představují významná témata v politice dlouhodobé péče. V rámci reflexe současného stavu podpory zdraví a pracovních podmínek formálních pečujících v českém prostředí jsme provedli kvalitativní sondu (focus groups), jejímž cílem bylo zjistit subjektivní názory této cílové skupiny na snahy zaměstnavatelů o podporu jejich zdraví formou zkvalitňování pracovních podmínek. Klíčovým tématem byla úroveň podpory zdraví na pracovišti a s ní související pozitivní a negativní faktory ovlivňující pracovní výkon., Population aging has economic, political and social consequences. Planning needs of long term care (LTC) and reflecting the number of professional caregivers responds to demographic changes. Although family caregivers dominate in many OECD countries including the Czech Republic, a number of professionals in these countries will double until 2050 (even triple in the Czech Republic). The need for the existence of quality institutions providing LTC setting is linked with good working conditions and health promotion of their employees. Prevention and health promotion of the aging population and of formal and informal caregivers are important topics in LTC policy. Within reflection of the current state of health promotion and working conditions of formal caregivers in the Czech environment, we conducted a qualitative probe (focus groups). The objective was to determine subjective opinions of this target group of employers’ efforts to promote their health by improving working conditions. V České republice je od roku 2008 zaveden celoplošný screening gestačního diabetes mellitus u těhotných žen. Od roku 2014 jsou však výsledky orálního glukózového tolerančního testu (oGTT), který je součástí screeningu, hodnoceny podle dvojích kritérií. Zatímco diabetologové a biochemici nově postupují podle doporučení Mezinárodní asociace pro studium diabetu v těhotenství (IADPSG), přejatého v řadě zemí a také WHO v roce 2013, čeští gynekologové zůstávají u původních diagnostických kritérií z roku 2008. Článek rozebírá nejdůležitější argumenty proti přijetí nových kritérií a vysvětluje důležitost jednotného postupu při screeningu a sledování gestačního diabetes mellitus., Since 2008 there have been screened all pregnant women for gestational diabetes in the Czech Republic. Nevertheless since 2014 the results of the oral glucose tolerance test (which is a part of the screening) have been evaluated by two different sets of diagnostic criteria. While diabetologists as well as biochemists newly use the criteria recommended by the International Association of Diabetes and Pregnancy Study Groups (IADPSG), accepted by various national medical societies and WHO in 2013 as well, czech gynecologists still use the former criteria accepted in 2008. Czech health and social services have undergone significant changes related to the reform of public administration including the transfer of certain social and health services to the competence of municipalities and regions. Regions also became legally responsible for social services ́ planning process including their registration and since 2015 financing. The findings in the article are based on a qualitative methodological approach, which was used as a combination of public policy documents analyses and primary data analysis. The empirical research was based on focus groups when respondents represented persons caring for the elderly and disabled people as well as providers of health-social services in the Central Bohemian Region. Based on the survey we can state that some types of services are underdeveloped, particularly the domiciliary services, social services for older children and adults with disabilities, respite care and other support services for caregivers. The great problem is the absence of interconnection between health and social services in the region. Pluralitní versus unitární systém po - jištění – tedy více či jedna jediná pojišťovna? Tyto otázky vedly autorku k identifikaci rolí, které by na základě teorie pojišťovny měly hrát a ke zjištění, že tyto role se pojišťovnám teoreticky lépe plní v systému pluralitním. Tato tvrzení otevírají prostor pro další výzkum a vybízejí k jejich aplikaci na realitu českého systému. Autorka si klade otázku, zda se český systém přibližuje „optimálně“ fungujícímu pluralitnímu systému, a zda dává pojišťovnám možnost, aby své role plnily. Následně studuje dopady tohoto stavu na zdravotní systém a jeho efektivitu., Plural versus unitary health insurance system – one or more health insurance funds? These questions led the author to the identification of roles that the health insurance funds should play and to the conclusion that those roles will be easily achieved in the plural system. These claims challenge further research of the Czech reality. The author examines to what extent the Czech system resembles to “optimal” plural system and if it enables the health insurance funds to fulfil their roles. The management of chronic diseases shall be considered a priority in the work of the global international institutions, which are related to health policies. In the search for effective and efficient solutions for the treatment of chronic diseases, scientists are developing different programs. In Bulgaria the continuous monitoring of chronically ill people is defined with the term dispensarization. The objective of the research is to analyze the place and the role of patients in the monitoring of their chronic diseases and how the concept of quality of life relates to this process. Method The research in focus groups is planned within a research project on the monitoring of chronic diseases. Five group discussions were held. The number of participants in all focus groups was sixty. Results The two discussed directions - the active role of the patient and the focus on the results are weak links in the current organization of the care for the chronically ill people. The topic of creating and adopting an Act of the patient was also presented, in which to be paid attention to the evaluation of the quality of life. In general was brought the need for a clear delineation of the roles and the competencies of everyone involved in the monitoring process of chronic diseases. The chronic disease cannot be defined only as a medical problem, as there are economic and social, including political consequences. Conclusions The management of the chronic disease requires coordination of doctor-patient interaction, a model of partnership and trust in the relations, self-management of the condition by the patients and their inclusion in the health team.
