Mnohočetný myelom (MM) a solitární plazmocytom (SP) jsou nejčastější primární nádory páteře. Léčba MM a SP páteře je komplexní proces, který zahrnuje léčbu kauzální s cílem potlačit nádorový klon a dále léčbu podpůrnou, jejíž součástí je i chirurgická terapie. Operace přispívají k lepší kvalitě života a nabývají na významu a smysluplnosti zejm. díky zlepšené prognóze onemocnění. Navrhli jsme algoritmus operační léčby MM a SP a ten jsme dále prospektivně ověřovali a jeho vyhodnocení bylo cílem této studie. Pacienti byli operováni v průběhu 10 let, minimální doba sledování byl jeden rok. Soubor tvořilo 32 pacientů (20 mužů a 12 žen) s průměrným věkem 61 let. Operováno bylo celkem 61 obratlů. Důvodem k operaci byly projevy útlaku míchy a progredující neurologický deficit (hodnoceno Frankelovou škálou) nebo na konzervativní léčbu nereagující axiální bolest (hodnoceno VAS; Visual Analogue Scale) páteře. Provedena byla některá z těchto operací: vertebroplastika (VP), laminektomie, transpedikulární fixace, somatektomie a jejich kombinace, nebo okcipito‑cervikální fixace. Pooperační VAS a Frankelova škála byly vyhodnoceny za jeden rok, skiagramy a MR jsme provedli a zhodnotili každý jeden rok. U žádného pacienta nenastala lokální recidiva tumoru ani selhání stabilizace na operovaném úseku v celém sledovaném období. Průměrný předoperační VAS byl 6,8 a jeden rok po operaci se průměrný VAS snížil na 1,1. Všechny operační výkony vedly nejen ke kontrole bolesti, ale také k prevenci nebo zlepšení neurologického postižení po celou dobu sledování. Hodnotili jsme náš postup – algoritmus operací, míru radikality a jejich načasování jako optimální pro MM i SP., Multiple myeloma (MM) and solitary plasmacytoma (SP) are the most frequent primary tumors of the spine. Management of the spinal MM and SP is a complex process involving causal treatment to suppress the tumor clone, as well as supportive therapy, including surgery and radiotherapy. Surgery should be considered because of its favourable effect on disease prognosis. We proposed a surgical treatment algorithm in patients with spinal MM and SP and the aim of this article is to present prospective evaluation of this algorithm. Patients undergoing surgical treatment during the past 10 years were included in this study, with the minimum follow-up of one year. A total of 32 patients (20 males and 12 females) with the mean age of 61 years were included. Surgery of 61 vertebrae was performed. The procedures were indicated for progressing neurological deficit (Frankel score) and for axial spinal pain (VAS classification). We performed the following procedures: vertebroplasty, laminectomy, transpedicular fixation, somatectomy, and their combination, or occipito-cervical fixation. Frankel score and VAS was assessed one year after the surgery and X-ray and spinal MRI was performed every year. No local relapses of the tumor or stabilization failure were detected. The mean preoperative VAS was 6.8 and improved to 1.1 one year after the surgery. During the follow up period, we observed positive effect of surgery on pain control and on prevention or improvement of neurological dysfunction. The authors concluded that all surgical procedures, the extent of resection and timing were adequate in all subjects. V posledních letech jsme svědky narůstajícího zájmu o onemocnění plicní cirkulace. Je to dáno jednak rozvojem na poli specifické farmakoterapie plicní hypertenze (PH), zlepšením její dostupnosti, a také zvládnutím techniky endarterektomie plicnice, jež se etablovala jako metoda první volby pro nemocné s chronickou tromboembolickou plicní hypertenzí (CTEPH). Klíčová je ale časná indikace léčby, a tedy časně stanovená diagnóza a určení typu plicní hypertenze. S moderními terapeutickými prostředky můžeme tak našim pacientům nabídnout kromě zlepšení prognózy a kvality života dokonce naději na úplné uzdravení, obzvláště nemocným s CTEPH. Následující text si klade za cíl upozornit na problematiku CTEPH., In recent years, we have witnessed a growing interest in diseases of pulmonary circulation. It is due to the development of specific drug therapy for pulmonary hypertension (PH), improving the availability and performance of endarterectomy techniques of pulmonary artery. This technique has been established as method of choice for patients with chronic thromboembolic pulmonary hypertension (CTEPH). However, the crucial issue is early indication of treatment, right diagnosis and determination of exact type of pulmonary hypertension. Modern therapeutic approaches provide better prognosis, quality of life and even possibility to cure the patients, especially suffering from CTEPH. Předkládaný text sumarizuje přístup k pacientovi s akutním závrativým stavem. Hlavním cílem prvního vyšetření je rozhodnout, zda jde o pacienta se závažným onemocněním – rozvíjející se cévní mozkovou příhodou (CMP) nebo o pacienta s vestibulární neuronitidou. Toto rozhodnutí je nutné ve většině případů učinit pod časovým tlakem a s vědomím, že v akutní fázi CMP nemusí neuroradiologické vyšetření poskytnout potřebné informace. Pečlivě provedené klinické vyšetření vestibulárního a okulomotorického systému postačuje k rozhodnutí, o jaký typ závrativé poruchy se jedná. Při sumarizaci klinických nálezů a stanovení pracovní diagnózy je nutné vycházet ze základní znalosti funkce vestibulárního systému, reflexních okruhů, do kterých je zapojen, a též principu organizace multisenzorického řízení rovnováhy., The following text summarizes management of patients with sudden vertigo. The main aim of the initial examination is to decide whether we are facing a patient with life-threatening disease, e.g. stroke, or a benign disease such as vestibular neuritis. This decision is usually made under a time pressure and knowing that neuroradiology examination may not provide sufficient information. Clinical examination of the vestibular and oculomotor function is sufficient to make the initial decision. Vestibular examination requires basic knowledge of the vestibular system function and reflex circuits involved in the organization and control of the multisensoric balance system. Tauopatie jsou neurodegenerativní onemocnění, která se vyznačují hromaděním abnormně modifikovaných forem proteinu zvaného tau především v kůře frontálních, temporálních a parietálních laloků, v bazálních gangliích a v mezencefalu. Molekulárně biologicky a biochemicky dobře definovaná skupina chorob má však dosti heterogenní klinické obrazy a je podstatou několika různých neurodegenerativních onemocnění tvořících frontotemporální lobární degenerace. Společným klinickým znakem těchto entit je ve větší či menší míře vyjádřený syndrom frontotemporální demence. Klinické jednotky, jež se řadí mezi klasické frontotemporální demence, však nejsou vždy podmíněny jen tauopatiemi, mnohdy je jejich etiopatogenetická podstata jiná. Účelem tohoto stručného přehledu je utřídění současné úrovně poznání vztahů mezi skupinou tauopatií a frontotemporálních lobárních degenerací. Článek obsahuje i návrh praktického postupu k upřesnění klinické diagnózy jednotlivých nozologických jednotek v rámci tauopatií., Tauopathies are neurodegenerative disorders characterized by accumulation of abnormally modified forms of the tau-protein, predominantly in frontal, temporal and parietal cortical regions, basal ganglia and in the midbrain. Tauopathies are well defined from the molecular biological and biochemical point of view; clinical symptoms may be, however, heterogeneous. Common signs of frontotemporal lobar degenerations include a more or less prominent syndrome of frontotemporal dementia. On the other hand, the clinical picture of frontotemporal dementia may not be caused by a tauopathy only; in many cases the underlying etiopathogenic cause is different. The aim of our review is to classify the relationship between tauopathies and frontotemporal lobar degenerations. This review contains a proposal of a practical approach to refining clinical diagnosis of the different tauopathies. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Tento přehledový článek vychází z kurzu prezentovaného na kongresu Americké urologické asociace v červnu 2014, který byl zaměřen na problematiku intersticiální cystitidy a syndromu bolestí v močovém měchýři. Autoři uvádějí nejnovější úpravy v taxonomii a názvosloví těchto syndromů včetně aktuálních definic a nabízejí pohled na bolestivé syndromy z gynekologické i urologické perspektivy. Článek je doplněn přehlednými diagramy a algoritmy uznávaných mezinárodních společností, které mohou sloužit jako vodítko při diagnostice a léčbě těchto komplexních syndromů., This review is based on the course presented on AUU meeting held in July 2014 focusing on the conditions known as interstitial cystitis and bladder pain syndrome. The authors are introducing last updates in taxonomy and nomenclature of these syndromes, including current definitions and offering an overview of painful syndromes from both gynaecologic and urologic perspective. Diabetes mellitus 2. typu léčíme dnes postupným přidáváním antidiabetik od monoterapie k dvojkombinacím, trojkombinacím i vícekombinacím léků. Metformin zůstává zatím lékem první volby, pokud je snášen a pokud není kontraindikován. V dalším kroku máme širokou možnost volby nejméně 5 skupin léků. Nové algoritmy používané ve Spojených státech zavádějí určitou preferenci inkretinové léčby (gliptiny a inkretinová analoga) a nové skupiny gliflozinů před tradičními postupy. Indikaci antidiabetik tak čekají v nejbližších letech buď velké změny nebo minimálně diskuse o preferenci jednotlivých lékových skupin včetně skupin v diabetologii nových. Klíčová slova: diabetes mellitus 2. typu – gliptiny – glifloziny – inkretinová léčba – metformin – perorální antidiabetika, Today we treat Type 2 diabetes mellitus by gradually adding antidiabetics from monotherapy to double, triple and multi combinations of medicines. Metformin still remains the first-line medicine, if well tolerated and not contraindicated. In the next step, we have a wide choice of 5 groups of medicines at least. The new algorithms used in the U.S.A. introduce some degree of preference for the incretin therapy (gliptins and incretin analogues) and the new groups of gliflozins over the standard treatments. V cerebrovaskulární neurologii vůbec poprvé existují nezpochybnitelné důkazy o klinické efektivitě mechanické rekanalizace u akutního uzávěru mozkové tepny. Pět randomizovaných studií publikovaných v roce 2015 dokumentovalo nebývalý prospěch a bezpečnost mechanické rekanalizace. Jednalo se o studie: MR CLEAN, ESCAPE, SWIFT PRIME, EXTEND-IA a REVASCAT. Bylo prokázáno, že endovaskulární léčba výrazně snižuje morbiditu a mortalitu pacientů. Počet pacientů, kteří musí být léčeni, aby jeden pacient byl kompletně vyléčen, je ohromující – jen 3–7 pacientů. Věk a tíže deficitu nepředstavují vylučující kritérium (MR CLEAN, ESCAPE a EXTEND-IA bez věkového omezení; SWIFT PRIME od 18 do 80 let a REVASCAT od 18 do 85 let). Základním zobrazením byly dominantně nativní výpočetní tomografie (CT) a CT angiografie. Perfuzní metody byly využity pouze v EXTEND-IA a SWIFT PRIME. Cílem výkonů bylo dosažení reperfuze do 60 min od vpichu do třísla. Nezbytnou součástí studií byl systém hodnocení kvality výkonů. V tomto článku přinášíme výsledky uvedených studií, shrnujeme management během endovaskulární léčby (korekce krevního tlaku, použití anestézie, konkomitantní medikace) a navrhujeme možná indikační kritéria., For the first time in cerebrovascular neurology there is an indisputable evidence of clinical effectiveness of mechanical recanalization in acute cerebral artery occlusion. Five randomized trials published in 2015 documented an unprecedented benefit and safety of endovascular thrombectomy. The particular trials were: MR CLEAN, ESCAPE, SWIFT PRIME, EXTEND-IA and REVASCAT. It has been proven that endovascular treatment reduces morbidity and mortality of patients significantly. The number needed to treat to result in one patient with good functional outcome was staggeringly low – only 3–7 patients. Age and deficit severity do not constitute exclusionary criteria (MR CLEAN, ESCAPE and EXTEND-IA without age restrictions; SWIFT PRIME between 18–80 years and REVASCAT between 18–85 years of age). The principal imaging methods were predominantly native CT and CT angiography. Perfusion methods were used in EXTEND-IA and SWIFT PRIME. The objective of endovascular treatment was to achieve reperfusion within 60 min after groin puncture. An essential part of the trials was a performance evaluation system. We provide information on the results of thrombectomy trials, summarize management during thrombectomy (correction of blood pressure, use of anesthesia, concomitant medication) and propose indication criteria. Tranzitorní ischemická ataka (TIA) a minor stroke patří mezi akutní cévní onemocnění mozku, jež sama o sobě nemají ve většině případů invalidizující dopad na pacienta. Jejich podcenění však může vést k recidivě cévní mozkové příhody (CMP) s fatálními následky. Cílem tohoto přehledového článku je upozornit na hlavní problémy současné klinické praxe v České republice a navrhnout racionální algoritmus péče o pacienty s TIA/minor stroke. Článek podává informace o nové definici TIA, která se opírá o fakt, že až u 50 % pacientů po TIA je na magnetické rezonanci (MR) přítomen infarkt, o stratifikaci rizika a jejím využití při managementu pacientů, o významu zobrazovacích vyšetření a o možnostech terapie. Poukazuje na skutečnost, že stratifikace rizika není v České republice dostatečně používána, což vede k neadekvátnímu poměru mezi rizikem pacienta a zvoleným diagnosticko-terapeutickým postupem. Urgentní péči potřebují obzvláště pacienti s vysokým rizikem recidivy CMP – tj. pacienti s poruchou řeči nebo motoriky trvající ≥ 5 min, se skóre ABCD2 ≥ 4, s fibrilací síní a pacienti s infarktem na difuzně vážených sekvencích (DWI) MR či CT (počítačové tomografii) a s okluzí nebo významnou ≥ 50% stenózou extra-/intrakraniální velké mozkové tepny. Přehledový článek zdůrazňuje nutnost nejen okamžitého zahájení medikamentózní terapie, ale i provedení časné endarterektomie a shrnuje nové možnosti léčby, např. duální antiagregace, jejíž přínos byl v časné fázi po TIA a minor stroke nedávno dokumentován., Transient ischemic attack (TIA) and minor stroke are acute cerebrovascular diseases that are usually non-disabling. However, if not followed adequately, they may lead to potentially fatal stroke recurrence. The aim of this review is to summarize the main challenges of the current clinical practice (in the Czech Republic) and suggest a rational algorithm for TIA/minor stroke management. This review informs about the new TIA definition based on the fact that up to 50% of TIA patients have a brain infarction on MRI (magnetic resonance imaging), about risk stratification and its use in patients management, about the importance of imaging studies and about the available treatment options. Risk stratification for TIA is underused in the Czech Republic and this affects risk-benefit ratios for the available management plans. In particular, follow-up of high-risk patients need urgent attention - patients with speech and/or motor symptoms lasting ≥ 5 min, with ABCD2 score ≥ 4, with atrial fibrillation, and patients with an infarction on DWI MRI or CT (computed tomography), and with an occlusion or significant ≥ 50% stenosis of a precerebral/cerebral large vessel. The review highlights the importance of an immediate medical intervention with early endarterectomy and provides an overview of new therapeutic options, e.g. dual antiplatelet therapy, the benefit of which has recently been documented for the first time.
