Dráždivý tračník (IBS) spolu s funkční dyspepsií jsou nejčastějšími funkčními poruchami trávicího traktu. Předmětem článku je souhrnný pohled na etiologii a patogenezi této choroby, diagnostiku a léčbu. Dráždivý tračník je charakterizován zejména bolestmi břicha a změnou tvaru a frekvence stolice. Je doporučováno definovat a klasifikovat jej podle „římských kritérií III“. Současný výzkum je zaměřen zejména na studium viscerální hypersenzitivity a na teorii tzv. minimálního zánětu a neuroimunní interakce. Etiologe a patogeneze jsou však stále neobjasněné a v klinické praxi zůstává farmakologická léčba dráždivého tračníku na úrovni léčby symptomatické, tj. zmírnění nebo odstranění bolesti, průjmu nebo zácpy, a v podávaní farmak s antianxiózním, event. antidepresivním účinkem. Lékem první volby by dnes měl být sulprid, antagonista D1 a D3 receptorů, který je u nemocných s IBS předepisován v dávce třikrát denně 50–100 mg, a sice pro anxiolytický a bolest potlačující účinek. V poslední části článku jsou diskutovány vedlejší účinky léků IBS na kardiovaskulární systém, Irritable bowel syndrome (IBS) and functional dyspepsia are the most common functional gut disorders. The main topic of the article is a summary review of the aetiology and pathogenesis of this disease as well as its diagnostic procedures and treatment. Irritable bowel syndrome is characterized by abdomen pain with altered stool frequency or consistency. It is recommended that it be defined and classified according to the Rome III classification. The symptoms are induced by physiological aberrations (like dysmotility and visceral hypersensitivity) or by the mechanisms of the initial inflammation and neuro‑immune interaction. This is also the focus of contemporary research. The aetiology and pathogenesis, however, remain unclear and the therapy of the day remains at the level of decreasing the main symptoms: abdominal pain, constipation, diarrhoea and psychological factors. Sulpiride, an antagonist of the D1 and D3 receptors, at a dose of 50 or 100 mg 3 times daily should be a first–choice treatment of IBS due to its anxiolytic and analgesic effects. Extrakardiální patologie u CT srdce prováděného z jakékoliv indikace nalézáme u 10 až 60 % pacientů v závislosti na populaci, průměru zobrazované oblasti (FOV), podání kontrastní látky, dále na tom, zda vyšetření interpretuje radiolog nebo kardiolog a zda je v dané geografické oblasti praktikována defenzivní medicína. U vyšetření provedených se širokým FOV nalézáme oproti úzkému FOV extrakardiální patologii u dalších zhruba 10% pacientů. Přibližně 19-26% z těchto nálezů vyžaduje další vyšetření. Výhodou identifikace extrakardiálních nálezů je možnost stanovení alternativní diagnózy, která vysvětluje obtíže pacienta (např. pleuritida, hiátová hernie). Hlavní nevýhodou je nízká výtěžnost, finanční náklady a radiační zátěž při následných vyšetřeních. Na druhou stranu u značné části pacientů není i potenciálně významný nález podroben dalšímu vyšetření. Otázkou zůstává, zda riziko přehlédnutí extrakardiální patologie (riziko sporu) je větší než potenciální benefit pro pacienta - to je však společné téma i pro další vyšetření (např. CT páteře)., Non-cardiac pathology in CT imaging of the heart can be found in 10-60% of patients depending on the population, field of view (FOV), administration of contrast material, practicing of defensive medicine, and whether the examination is interpreted by a radiologist or a cardiologist. When large FOV is used, non-cardiac findings can be found in additional 10% of patients compared to small FOV. Around 19-26% of secondary findings require further workup. Identification of non-cardiac findings has its merits: alternative diagnosis that may explain patients complaints (eg. pleurisy, hiatal hernia). Its major drawbacks include low yield of clinically significant pathology, cost and radiation burden of subsequent examinations. On the other hand, in many patients even a potentially significant finding is not subject to further workup. Familial Mediterranean Fever is an autosomal recessive inherited disease with a course of autoinflammation, which is characterized by the episodes of fever and serositis. It affects the populations from Mediterranean basin. Genetic mutation of the disease is on MEFV gene located on short arm of Chromosome 16. The disease is diagnosed based on clinical evaluation. Amyloidosis is the most important complication. The only agent that decreases the development of amyloidosis and the frequency and severity of the episodes is colchicine, which has been used for about 40 years. Úvod: Správné zhodnocení jaterních pseudolézí pomocí multidetektorového CT či MR je velmi důležité z důvodu odlišení těchto benigních změn od primárních či sekundárních malignit v jaterním parenchymu. Metoda: Autoři provedli systémový přehled literatury v databázi PubMed s vyhledáváním slov játra, pseudoléze v angličtině a češtině od roku 2000 do roku 2014. Výsledky: Autoři předkládají přehled literatury. Dále autoři retrospektivně zhodnotili soubor pacientů léčených pro jaterní onemocnění ve FN Ostrava, kdy se setkali s touto abnormalitou celkem v 7 případech. U 3 pacientů byla provedena diagnostická laparoskopie s vizuální kontrolou léze, s upřesněním diagnostiky pomocí peroperační ultrasonografie (IOUS) a s excizí části ložiska pro následné histologické vyšetření. Histologický nález u všech 3 pacientů odpovídal steatotickému postižení jater. Další 2 pacienti byli operováni pro jiná ložiska metastazujícího kolorektálního karcinomu a revize suspektní léze byla provedena během operačního výkonu otevřenou cestou. Kromě vizuální kontroly a peroperačního IOUS byla provedena excize k histologickému vyšetření, které prokázalo známky velkokapénkové i malokapénkové steatózy a nepřítomnost maligních změn. Poslední dva pacienti jsou dále sledováni v hepatologické poradně a jsou v rámci dispenzarizace kontrolováni v 6měsíčních intervalech. Závěr: Autoři prezentují vlastní zkušenosti získané v rámci mezioborové spolupráce na multidisciplinárních konferencích a připojují literární údaje o této neobvyklé problematice. Správným vyhodnocením nálezů této variety zejména u onkologických pacientů je možno zabránit zbytečným biopsiím, duplicitním vyšetřením a diagnostickým laparoskopiím či explorativním laparotomiím., Introduction: Accurate detection of hepatic pseudolesions using multi-detector CT and MRI examinations is crucial for the differentiation of benign alterations from primary and secondary malignant lesions in hepatic parenchyma. Method: The authors conducted a systematic literature review in PubMed. “Liver“ and “pseudolesion“ were used as keywords in English and Czech, and papers/articles published from 2000 to 2014 were retrieved. Results: The authors presented a literature review. In addition, the authors performed a retrospective evaluation of a group of patients treated for liver disease at University Hospital Ostrava where this anomaly was encountered in 7 cases. In 3 of the patients, diagnostic laparoscopy was done, with visual examination of the lesion accompanied by intraoperative ultrasound exam (IOUS) and partial excision, to establish the diagnosis. Subsequent histological assessment of the specimens confirmed the diagnosis of a steatotic lesion in each of these 3 patients. Additional 2 of the 7 patients underwent liver surgery for concurrent metastatic lesions of colorectal cancer and an open-access revision of the suspected lesions was performed. Visual inspection and intraoperative ultrasound (IOUS) was followed by excisional biopsy. The histology revealed macro- and micro-vesicular steatosis and excluded malignant changes. The last 2 patients still continue to be followed–up regularly on a 6-month routine check-up basis at our hepatology unit. Conclusion: The authors presented their own experience gained through inter-disciplinary cooperation at Multidisciplinary conferences. A literature overview of this unusual subject is also included. Hepatorenální syndrom je život ohrožující komplikace jaterních chorob. Jedná se o funkční postižení ledvin, které se vyskytuje jak u akutních (typ I), tak i chronických (typ II) onemocnění jater a je spojeno s vysokou mortalitou. Léčba má omezené možnosti, ale podávání vazopresorů (terlipresin), albuminu a portosystémový shunt dokážou prognózu zlepšit. Transplantace jater je léčbou nejefektivnější. Autoři prezentují aktuální data o této problematice včetně doporučení pro léčebnou praxi., Hepatorenal syndrome (HRS) is a life-treating complication of liver diseases. This functional kidney impairment is classified into acute (type I) and chronic (type II) types and is connected with high mortality. Treatment options are limited, but administration of vasoconstrictors (terlipressin), albumin and portosystemic shunt may improve their prognosis. Liver transplantation is the most effective method for these patients. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Jako incidentalom se označuje náhodně zjištěná expanze nadledviny. Vzhledem k rozvoji zobrazovacích technik se s ní setkáváme stále častěji. Její prevalence je totiž více než 1 % dospělých a roste s věkem. Při vyšetření je nutno vyloučit malignitu. K tomu slouží klasicky vyšetření CT, ev. MRI. Nověji se stále více uplatňuje PET-CT, která má senzitivitu asi 97 % a specificitu 91 %. Hormonální vyšetření má vyloučit i subklinickou nadprodukci kortizolu (optimální je dexametazonový supresní test). Aldosteron je nutno vyšetřovat jen u nemocných s hypertenzí a katecholaminy u nádorů s vyšší nativní denzitou než 10 HU. Při sledování je nutnost opakovaných CT vyhrazena jen pro vybrané případy a rutinní kontrolní laboratorní vyšetření jsou také zpochybněna., Incidentaloma is an adrenal mass discovered serendipitously. Because of increasing use of imaging techniques it is a common finding, being present in more than 1 % of adults. During work-up malignancy has to be ruled out. Classically imaging using CT or MRI is used. Recently PET-CT with FDG has been used more often as its sensitivity for diagnosis of malignancy is about 97 % and specificity 91 %. Hormonal evaluation should diagnose subclinical hypercortisolism. (Dexamethasone Suppression Test is the method of choice). Aldosterone and Renin should be measured only in patients with hypertension and catecholamines in tumours with higher native density than 10 HU. Infekce kůže a měkkých tkání (IKTM) zahrnují široké spektrum zánětlivých onemocnění postihujících kůži, podkoží, fascie a svaly. V posledním období pozorujeme nárůst množství pacientů ošetřených pro tyto diagnózy na chirurgických ambulancích, ale zvyšují se i počty pacientů hospitalizovaných v nemocnici se závažnými formami IKMT. Smrtnost nejzávažnějších forem je v literatuře uváděna až 90 %. Příčinou je podcenění počáteční symptomatologie a z ní pramenící pozdní zahájení radikální léčby až ve fázi septického šoku. Text tvoří mezioborový přehled základních principů diagnostiky a léčby IKTM, který odráží současné poznatky opřené o medicínu důkazů., Skin and Soft Tissue Infections (SSTIs) encompass a wide spectrum of inflammatory diseases of the skin, subcutis, fascia and muscles. Recent national trends indicate increasing rates of patients treated for these diagnoses at outpatient surgical department, but also of patients hospitalized with severe forms of SSTIs. Up to 90% fatality rates are reported in the literature for the most severe forms, resulting from underestimation of initial symptoms and therefore delayed onset of radical therapy only the phase of septic shock. Infekce močových cest (IMC) se řadí k nejčastějším onemocněním u dětí. Diagnostika je založená na klinických symptomech a laboratorních nálezech. Neméně významné postavení v diagnostickém algoritmu má i vyšetření zobrazovacími metodami. Zlatým standardem je ultrazvukové vyšetření ledvin, močovodů a močového měchýře. Z dalších zobrazovacích metod je možné využít mikční cystouretrografii buď klasickou rentgenovou, nebo v radionuklidové modifikaci, statickou scintigrafii ledvin, CT nebo MR vyšetření. V tomto článku je přehledně popsán ultrazvukový obraz jednotlivých typů infekcí močových cest a je doplněný obrazovou dokumentací., Urinary tract infections (UTI) are one of the most often disease in children. The diagnosis is based on clinical signs and value of laboratory results. Nevertheless imaging methods plays also very important role in the diagnostic algorithm. The gold standard is ultrasonography of kidneys, ureters and urinary bladder. Other usable imaging methods are a voiding cystourethrography which can be modified as a radionuclide examination, static renal scintigraphy, CT or MR investigation. Běžný variabilní imunodeficit (CVID) patří k nejběžnějším primárním imunitním deficitům a je charakterizován poruchou tvorby imunoglobulinů. Hypogamaglobulinemie se u CVID manifestuje v každém věku, obvykle ale v dospělosti, a není známa závislost výskytu nemoci na pohlaví. Prevalence se odhaduje na 1 : 25 000–1 : 50 000. Patogeneze CVID není plně známa. Základním klinickým respiračním příznakem CVID jsou opakující se infekce dýchacích cest, převážně bakteriální: sinusitidy, bronchitidy, pneumonie, se vznikem bronchiektázií a plicní fibrózy. Intersticiální plicní procesy a granulomatózy se obvykle manifestují již v době diagnózy CVID a jsou většinou špatným prognostickým faktorem onemocnění., Common variable immunodeficiency disorder belongs to the most common primary human immunodeficiencies and it is characterized by primary defective immunoglobulin production. The main clinical respiratory symptoms include recurrent respiratory infects, especially bacterial etiology, sinusitis, bronchitis, pneumonia, leading to bronchiectasis and lung fibrosis.
