Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma. Pudendální neuralgie je špatně snesitelný bolestivý stav způsobený postižením (většinou kompresí) pudendálního nervu. Klinicky je charakterizován progresivní bolestí v pánvi, která se horší sezením, bývá spojena se sexuální dysfunkcí, potížemi s močením a defekací. Ke stanovení jisté diagnózy jsou používána nanteská kritéria. V této práci prezentujeme soubor šesti pacientů, kteří byli od roku 2009 léčeni pro toto onemocnění ve FN Brno, a popisujeme současné terapeutické možnosti a hodnotíme jejich efekt., Pudendal neuralgia is a very painful condition caused by compression or entrapment of the pudendal nerve. Clinical characteristics include progressive pelvic pain, worsened by sitting, sexual dysfunction and difficulty with urination and defecation. Nantes criteria are used to confirm pudendal neuralgia. We present a group of six patients treated for pudendal neuralgie at the University Hospital in Brno since 2009 and discuss the options for the treatment of this syndrome. Poruchy kortikálního vývoje a benigní tumory mozku jsou častou příčinou dlouhotrvající farmakorezistentní epilepsie u dětských i dospělých nemocných. Tito nemocní jsou operačně řešeni a následně sledováni v epileptochirurgických centrech. Progrese gradingu mozkových tumorů u nemocných s farmakorezistentní epilepsií je vzácným problémem a rozvoj maligního tumoru mozku u nemocných s malformacemi kortikálního vývoje mozku je publikován zcela výjimečně. Ve sdělení prezentujeme dvě kazuistiky nemocných s farmakorezistentní epilepsií s neočekávaným rozvojem maligního tumoru mozku. U jedné nemocné s chronickou epilepsií došlo k velmi časné progresi gradingu opakovaně histologicky verifikovaného nízkostupňového gliomu s finálním nálezem glioblastomu. U druhého nemocného byl popsán výjimečný rozvoj glioblastomu v terénu hemimegalencefalie., Malformations of cortical development and benign brain tumors are frequent causes of long lasting pharmacoresistant epilepsy in paediatric and adult patients. Such patients are surgically treated and subsequently followed in epileptosurgical centers. Tumor upgrading in patients with pharmacoresistant epilepsy is only rarely encountered and the development of malignant brain tumor in a patient with malformation of cortical development is an exception. The aim of the paper is to present two cases of patients suffering from intractable epilepsy with unexpected development of malignant brain tumor. There was upgrading of low grade glioma (verified by repeated biopsy sampling) in one patient with chronic epilepsy with final diagnosis glioblastoma multiforme. Sarkoidóza je multisystémové onemocnění granulomatózního charakteru neznámé etiologie, které velmi vzácně postihuje nervový systém. Granulomatózní proces může postihovat všechny struktury centrální i periferní nervové soustavy. Neurosarkoidóza má špatnou prognózu a léčbu je nutno zahájit v časných stadiích. Uvádíme kazuistiku muže středního věku s diagnostikovanou sarkoidózou nitrohrudních uzlin a plicního parenchymu, u kterého s časovou latencí 10 let došlo k rozvoji kardiálního a neurologického postižení. Bylo diagnostikováno postižení periferního nervového systému charakteru periferní neuropatie a symptomatika zadních míšních provazců asociované se základním onemocněním sarkoidózou. Byla nasazena léčba kortikoidy, která vedla ke zmírnění neuropatických obtíží. Pacient je v současnosti na udržovací dávce kortikoidů, vykazuje nízkou aktivitu plicního onemocnění a neurologický nález se během doby sledování nezměnil. Sdělení obsahuje přehled základních diagnostických a léčebných algoritmů u pacientů se stanovenou diagnózou neurosarkoidózy, vč. prvních zkušeností s novými biologickými preparáty při léčbě tohoto raritního, ale velmi závažného onemocnění., Sarcoidosis is a multisystem granulomatous disease of unknown origin that rarely affects the nervous system. Granulomatous process may affect all structures of the central and peripheral nervous system. Neurosarcoidosis is associated with bad prognosis and the treatment should be started at early stages of the disease. We present a case of a middle-aged man with sarcoidosis of the peripheral lung lymph nodes and lung parenchyma. After 10 years of follow-up, the disease progressed with cardiac and neurological involvement. The peripheral nervous system was affected with peripheral neuropathy associated with sarcoidosis; other reasons for peripheral neuropathy were excluded. Treatment with corticosteroids led to improvements in neurological symptoms. At present, the patient is on a maintenance dose of corticosteroids, shows low activity of lung disease and neurological findings have been stable throughout the follow-up period. A brief literature review of the latest diagnostic and treatment options in patients with neurosarcoidosis is presented, including the first experience with novel biological treatments of this rare, very serious disease. Spinální epidurální lipomatóza (SEL) je stav, při kterém dochází k patologickému nahromadění tuku v epidurálním prostoru páteřního kanálu. Za možnou příčinu bývá považováno užívání kortikosteroidů, obezita, endokrinologické onemocnění (nejčastěji endogenní nadprodukce kortizolu), u části případů však zůstává příčina neobjasněna. SEL se stává symptomatickou zřídka, a to ve chvíli, kdy způsobí kompresi míchy nebo nervových kořenů. Klinická manifestace závisí na úrovni postižení páteřního kanálu. Pro terapii SEL je určující závažnost komprese nervových struktur a tomu odpovídající tíže klinického syndromu. Při hodnocení šíře epidurálního tuku na zobrazovacích vyšetřeních, při volbě terapeutického postupu a při stanovení prognózy mohou být nápomocné nové klasifikace, které vytvořili Borré (pro bederní páteř) a Quint (pro hrudní páteř). Terapeutické možnosti zahrnují konzervativní terapii (redukce tělesné hmotnosti, redukce dávky kortikoidů, léčba endokrinologického onemocnění, analgetika, rehabilitace) a chirurgickou dekompresi. Samotná SEL je však velmi vzácná. U pacientů se často vyskytuje v koincidenci s jinými, hlavně degenerativními změnami páteře. V naší práci uvádíme tři kazuistiky, na kterých demonstrujeme možnou příčinu, průběh a terapii tohoto onemocnění. U prvního pacienta se lipomatóza manifestovala syndromem kaudy, v druhém případě radikulárním syndromem a u třetího pacienta SEL způsobila kompresi hrudní míchy., Spinal epidural lipomatosis (SEL) is a condition associated with pathological fat accumulation in the epidural area of the spinal canal. The disorder is likely caused by the use of corticosteroids, obesity, endocrinal disorders (especially endogenous overproduction of cortisol), although, in some cases, no cause is immediately evident. SEL rarely becomes symptomatic unless it leads to compression of the spinal cord or nerve roots. Clinical manifestation depends on the level at which the spinal canal is affected. Severity of compression of nerve structures and the corresponding intensity of clinical syndrome are the most important factors in the choice of SEL therapy. Recent classifications for evaluation of epidural fat layer on radiographic images by Borré (for lumbar spine) and Quint (for thoracic spine) can help in guiding the diagnostic and treatment approaches. Therapeutic options include conservative therapy (reduction of body weight, reduction of any corticoid dosage, treatment of the endocrinal disorder, analgesics, rehabilitation) and surgical decompression. However, SEL itself is rare and is frequently found together with other (e.g. degenerative) spinal changes. We report three cases from our own patient base through which we demonstrate possible causes, course and therapy of this disorder. Lipomatosis manifested as cauda equina syndrome in the first patient, as radicular syndrome in the second, and SEL led to compression of the thoracic spinal cord in the third. Spinocerebelární ataxie typu 6 (SCA6) je relativně vzácné hereditární neurodegenerativní onemocnění patřící do skupiny autozomálně dominantních spinocerebelárních ataxií. Příčinou je expanze nestabilních CAG tripletů v genu kódujícím podjednotku napěťově řízeného kalciového kanálu na 19. chromozomu. Onemocnění je charakteristické postupným rozvojem paleo- a neocerebelární symptomatiky s počátkem ve středním a vyšším věku. V následujícím textu prezentujeme případ nyní 63leté pacientky s náhlým rozvojem nespecifických potíží při chůzi, zprvu hodnocených jako stav po proběhlé cévní mozkové příhodě, s postupným vývojem klinického obrazu do výrazné trupové ataxie a instability chůze při zcela němé rodinné anamnéze. Po vyloučení častějších příčin obtíží byla na základě genetického vyšetření diagnostikována SCA6. Kazuistika ukazuje na nutnost pomýšlet i na možnost výskytu vzácné diagnózy, což v tomto případě přináší ukončení opakovaných vyšetření a v neposlední řadě i možnost prediktivního testování u příbuzných v riziku., Spinocerebellar ataxia 6 (SCA6) is a rather rare neurodegenerative disorder, one of autosomal dominant spinocerebelar ataxias. It is caused by expansion of unstable CAG triplet repeats in the gene responsible for the voltage-dependent calcium channel mapped to the chromosome 19. The disease is characterized by progressive paleo- and neocerebellar symptomatology. The onset of the disease is typically in middle- and older-age. The authors describe a case of 62-year-old women with sudden development of gait deterioration. This was first diagnosed as a post-ischemic stroke state. However, clinical symptoms progressed into gait ataxia. No analogical case was found in family history. Genetic testing, performed after all other potential causes of the condition were excluded, indicated SCA6. Our case report emphasises the importance of considering rare conditions as part of differential diagnosis. In this case, this protected the patient from further diagnostic testing as well as, importantly, enabled predictive testing in relatives in risk. Stiff-person syndrome (SPS) is a rare disorder characterized by muscle stiffness and painful spasms. Misdiagnosis may occur due to the fact that the clinical picture of SPS is often atypical. The main pathophysiologic mechanism underlying the development of SPS is insufficient inhibition at the cortical and spinal levels. There is good evidence for a primary autoimmune etiology. A 61-year-old man was admitted to a neurological department due to muscle hypertonia with episodic attacks of painful spasms predominantly affecting axial muscles. The symptoms developed shortly after tickborne meningoencephalitis. Electromyography (EMG) revealed signs of continuous motor unit activity. Antibodies against glutamate decarboxylase (anti-GAD) were highly elevated. We present a case of a man who developed clinically severe anti-GAD positive SPS, provoked by tick-borne encephalitis. After therapeutic plasma exchange (TPE) a rapid, temporary improvement of the clinical and neurophysiological findings was noted. Only after being placed on long-term immunosuppression did the patient achieve stable recovery. Pacient s trombózou mozkových splavů a současným intracerebrálním hematomem parieto-okcipitálně byl neúspěšně léčen plnými dávkami intravenózní antikoagulace. Vzhledem k postupné progresi klinického stavu vedoucí ke ztrátě vědomí byla indikována endovaskulární intervence. Pro časté ucpávání aspiračního systému Penumbra byl originálně použit guiding katétr Neuron (současně se Separator 3D) zavedený přímo do trombu namísto Reperfusion katétru se současnou aspirací trombu. Tento postup byl bezpečný a úspěšný a může být použit u pacientů s rozsáhlým trombotickým postižením. A common endovascular aspiration technique was indicated because of the progressive severe deterioration, the level of consciousness had failed. As a novel approach, we used the 6F Neuron guiding catheter (together with The Separator 3D) inserted directly into the thrombus instead of the Reperfusion Catheter, for mechanical disruption of the thrombus mass together with continual aspiration. Zánětlivý pseudotumor je vzácná patologie neznámé etiologie vyskytující se nejčastěji v oblasti orbity a plic. Zcela raritně můžeme identifikovat ložisko zánětlivého pseudotumoru intrakraniálně. Predilekční lokalizace v nitrolebí je baze lební a kavernózní sinus. V naší kazuistice prezentujeme případ 43leté pacientky, u které jsme diagnostikovali konvexitární, extraaxiální, kontrastem se sytící expanzi. Pacientka podstoupila operační zákrok s extirpací procesu. Histologické vyšetření stanovilo diagnózu zánětlivého pseudotumoru. Nemocná je půl roku po operačním výkonu bez obtíží., Inflammatory pseudotumour is a rare pathology of unknown aetiology, located predominantly in the orbita and lungs. Intracranial localisation is infrequent. The most frequent intracranial localisations include the cranial base and cavernous sinus. In the present paper, we present a case report of a 43-year-old woman diagnosed with a contrast-enhancing extra-axial lesion of the convexity. The patient underwent a surgery and the lesion was extirpated. The diagnosis of inflammatory pseudotumour was verified by histology. The patient is free of any symptoms one year after surgery.
