Aminoglykosidy patří mezi nejstarší antibiotika, a i přes své relativně toxické účinky (nefrotoxicita a ototoxicita) jsou v současné klinické praxi cennou léčebnou skupinou. Jsou účinné zejména proti aerobním gramnegativním infekcím a v kombinaci s antibiotiky poškozujícími buněčnou stěnu (např. betalaktamy) také proti grampozitivním kokům. Vyznačují se koncentračně závislým baktericidním účinkem a dlouhým postantibiotickým efektem. Existuje mnoho důkazů o tom, že jedna velká denní dávka (respektive prodloužený dávkovací interval) aminoglykosidů je spojena s nižším rizikem nefrotoxicity a ototoxicity a srovnatelnou, ne-li lepší klinickou účinností v porovnání se stejnou celkovou denní dávkou podanou v několika menších dávkách během dne. Účinnost a toxicita aminoglykosidů vykazuje silný vztah k jejich sérové koncentraci. Klíčovou úlohu v minimalizaci toxicity a optimalizaci dávkování má terapeutické monitorování., Aminoglycosides constitute one of the oldest classes of antimicrobials. Despite their relative toxicity, mainly nephrotoxicity and ototoxicity, aminoglycosides are valuable in current clinical practice. They are bactericidal agents with activity against aerobic gram-negative infections and against gram-positive cocci when added to a cell wall active antimicrobial-based regimen (e.g. betalactams). There is accumulating evidence to show that large, single, daily doses (or more correctly, extended interval dosing) of aminoglycosides is associated with less nephrotoxicity and ototoxicity and comparable, if not superior clinical outcomes than the same total dose administered in small, multiple doses. Chronická žilní choroba (v originálním anglickém znění chronic venous disorder – CVD) je postižení žilního systému s vysokou incidencí, která se vyskytuje především Evropě a severní Americe. Hlavním cílem mikronizovné purifikované flavonoidní frakce (MPFF) diosminu a hesperidinu je eliminovat symptomy CVD (žilní bolest, tíže v nohou apod). Avšak MPFF diosminu a hesperidinu může být též použita k léčbě žilního otoku a žilního bércového vředu. Existuje mnoho studií, které dokazují jeho efekt v experimentu a též jeho efekt na úrovni mikrocirkulace. Další indikací MPFF diosminu a hesperidinu je hemoroidální choroba a jako doplňková léčba lymfedému. Je též prokázáno, že tato látka může být požita jako vhodná doplňující léčba zákroků na žilním systému. Pouze MPFF diosminu a hesperidinu obdržela nejlepší doporučení – 1B v posledních směrnicích pro léčbu venoaktivními látkami., Chronic venous disorder (CVD) is a common illness with high incidence existing especially in Europe and North America. The main goal of micronized purified flavonoid fraction (MPFF) of diosmin and hesperidin is to eliminate the symptoms of CVD (venous pain, fatigue, etc). But MPFF of diosmin and hesperidin has good effectiveness for treatment of venous oedema and venous ulcer too. There are many papers that prove its effectiveness in the experiment and in the microcirculation too. The other indications for MPFF of diosmin hesperidin is hemorrhoidal disease and the accessory treatment of lymphedema. It is proved that this substance could be used as an effective supplementary treatment of symptoms after venous intervention. Diabetes mellitus a zejména diabetes mellitus 2. typu představuje jeden z nejvýznamnějších rizikových faktorů kardiovaskulárních onemocnění. Z hlediska ovlivnění KV rizika má enormní význam nejen pozitivní ovlivnění glykemie, ale také léčba diabetické dyslipidemie a arteriální hypertenze. Předložená práce poskytuje analýzu terapie antihypertenzivy a hypolipidemiky u všech diabetiků registrovaných u VZP v období let 2010–2013. V uvedeném období bylo s diagnózou diabetes mellitus léčeno 866 570 pojištěnců VZP, přičemž většinu tvořili diabetici nezávislí na inzulinu. Přibližně polovina pacientů byla dispenzarizována diabetology a polovina lékaři jiných specializací. Z antihypertenziv byli pacienti nejčastěji léčeni léky ovlivňujícími systém renin-angiotenzin, dále pak betablokátory a diuretiky. Preskripce hypolipidemické léčby, zejména preskripce statinů, v souladu s platnými doporučeními, pozvolna narůstá, nicméně i nadále zůstává nedostatečná (v roce 2013 zůstalo bez hypolipidemické léčby 43,6 % diabetiků z ambulancí diabetologů a 51,3 % diabetiků z ambulancí praktických lékařů!). V doplňující analýze dat za rok 2014 jsme zjistili, že v tomto roce bylo léčeno z celkového počtu osob s farmakologickou léčbou diabetu nějakým hypolipidemikem 57,3 % osob. Nedostatečné využití zaznamenává rovněž kombinační hypolipidemická léčba, dosud zastoupená zejména kombinací statinu s fibrátem. Nicméně v následujících letech očekáváme na základě pozitivních výsledků subanalýzy studie IMPROVE-IT nárůst kombinační hypolipidemické terapie, resp. kombinační léčby statiny v kombinaci s ezetimibem. To influence cardiovascular risk there is enormous important not only positive influence of glycemia, but also the treatment of diabetic dyslipidemia and hypertension. The present work provides an analysis of lipid-lowering and antihypertensive therapy for all diabetics registered with General Health Insurance Company in the period 2010–2013. In this time 866 570 patients with diabetes mellitus registered with General Health Insurance Company were treated, the majority of them were diabetics independent on insulin. Approximately half of the patients were observed by dialectologists and half of them by the doctors of other specialization. Out of antihypertensive medication, patients were most often treated by drugs that affect the renin-angiotensin system, as well as beta-blockers and diuretics. Prescription of the lipid-lowering therapy, especially prescription of statins, in accordance with the guidelines, is increasing, but remains insufficient (at 2013 43.6% diabetics treated by dialectologists and 51.3% diabetics treated by GP´s didn´t have lipid lowering therapy). Inadequate use of combination lipid-lowering therapy was recorded too, still represented mainly by combination of statin and fibrate, but in coming years we expect (based on the positive results of the subanalysis IMPROVE-IT study), an increase of combination therapy statin and ezetimibe. Cíl: Cílem našeho článku je předložit normativní data pro fonemickou verbální fluenci (VF), pro hlásky K, P, S, i sémantickou VF, pro kategorie zvířata a zelenina. Úvod: Testy verbální fluence jsou jedna z nejrozšířenějších metod hodnocení kognitivní výkonnosti. V českém prostředí dosud neexistuje kvalitní normativní studie testů VF se zaměřením na populaci starších osob, u kterých se tato metoda používá nejčastěji. Metodika: Testy VF jsme administrovali v rámci neuropsychologické baterie 540 zdravým osobám (292 žen, 248 mužů) nad 59 let. V části fonemické VF jsme použili hlásky K, P, S a celkový skór všech hlásek (K + P + S) jako analogii anglického originálu F, A, S v Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). V rámci sémantické VF jsme hodnotili výkonnost ve dvou kategoriích, zvířata a zelenina. Výsledky: Věk má statisticky významný (p < 0,001) slabý až středně silný vliv na výkon ve VF, reprezentovaný součtem skórů K + P + S (r = –<span class=""></span>0,236), zvířata (r = –<span class=""></span>0,359) a zelenina (r = –<span class=""></span>0,264). Rovněž počet let vzdělání ovlivňuje slabě až středně silně výkon ve VF. Statisticky významně (p < 0,001) pouze ve skórech K + P + S (r = 0,297) a zvířata (r = 0,357). Mezi věkem a skórem zelenina vztah není signifikantní (r = 0,028; p = 0,523). Ve skóru zelenina existuje signifikantní (p < 0,001) rozdíl mezi muži a ženami. Dále předkládáme normativní data pro věkové skupiny 60–<span class=""></span>75 let, 70–<span class=""></span>85 let a 80–<span class=""></span>96 let. Závěr: Výsledky naší studie prokazují statisticky významný vliv věku na všechny uvedené zkoušky VF. Fonemická VF a kategorie zvířata jsou závislá i na výši vzdělání, v kategorii zelenina není výkonnost závislá na vzdělání, ale na pohlaví., Aim: The aim of the study was to report normative data on letter (LF; letters K, P, S) and semantic fluency (SF; animals and vegetables). Introduction: Verbal fluency (VF) is one of the most frequently used neuropsychological methods for the assessment of cognitive performance in clinical and experimental neuropsychology. However, representative normative data for the Czech population of older and very old adults are so far lacking. Methods: We administered VF as part of neuropsychological battery to 540 (292 women, 248 men) healthy older adults (60–<span class=""></span>96 years of age). In LF, the letters K, P, S and their total score were used as analogous to the original Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). In SF, we evaluated performance in two categories –<span class=""></span> animals and vegetables. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Pavlova 6, 775 20 Olomouc, Czech Republic; Hospital Písek, Karla Čapka 589, 397 23 Písek, Czech Republic. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases. Současné studie naznačují možnou důležitou úlohu melatoninu v Huntingtonově nemoci (HN) a jeho možné terapeutické využití při léčbě této nemoci. HN je dědičné neurodegenerativní onemocnění, které doprovází snižování hladiny melatoninu s postupem onemocnění. U normálních (nenádorových) buněk působí melatonin antiapoptoticky díky svým antioxidačním vlastnostem a schopnosti zabránit aktivaci proteinu p53. Dále melatonin zvyšuje expresi BDNF (brain derived neurotrophic factor) a dalších neuroprotektivních faktorů. Cílem této studie bylo stanovit netoxickou dávku melatoninu pro primární kožní fibroblasty izolované z transgenních miniprasat pro N‑koncovou část lidského mutovaného huntingtinu (TgHD) a popsat efekt tohoto ošetření na tyto buňky vystavené genotoxickému stresu. Buňky byly kultivovány v médiu obohaceném různými dávkami melatoninu. Analýzou proliferačních křivek získaných mikroskopováním živých buněk v pravidelných časových intervalech jsme stanovili efekt různých koncentrací melatoninu.Ukázali jsme, že vyšší dávky melatoninu jsou pro primární prasečí buňky toxické. Je zajímavé, že TgHD buňky byly oproti kontrolním buňkám více citlivé k tímto dávkám melatoninu. Stanovili jsme efektivní dávku melatoninu a současně jsme ukázali její efekt na proliferaci u buněk vystavených genotoxickému stresu. HD, the inherited neurodegenerative disorder, is accompanied by gradual melatonin reduction as it progresses. Melatonin in normal cells (non‑tumor) has the anti‑apoptotic ability due to its antioxidant property and its ability to prevent the activation of p53. Furthermore, melatonin increases the expression of BDNF (brain derived neurotrophic factor) and other neuroprotective factors. The aim of this study was to evaluate the nontoxic dose of melatonin for primary skin fibroblasts isolated from minipigs transgenic for the N‑terminal part of human mutated huntingtin (TgHD), and the effect of melatonin treatment to these cells exposed to genotoxic stress. Cells were cultured in medium supplemented with different doses of melatonin. Using time lapse microscopy, we estimated the effect of decreasing melatonin concentrations by analyzing the proliferation curves. We show that higher doses of melatonin are toxic for primary porcine fibroblasts. Interestingly, TgHD cells were more sensitive to these doses of melatonin treatment than wild type cells. Large animal models to explore the safety and tolerability of novel therapeutic approaches for Huntington’s disease (HD) are in exploration to achieve higher translational reliability in future studies. Recently, a Libechov minipig has been established as one new transgenic (Tg) large animal model for HD. We here discuss the advantages and limitations in using this model in HD with regards to breeding, housing, handling, and with respect to homology to humans and ethical considerations. A group of TgHD and wild type (WT) female minipigs (n = 36) was used to gain first evidence about abovementioned aspects. Východiska: Implementace mezinárodní klasifikace NIC v podmínkách intenzivní péče v ČR. Cíl: Identifikace intervencí NIC Klasifikačního systému, které všeobecné sestry označí jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Metody: Kvantitativní analýza četnosti intervencí NIC. Dosáhla-li konkrétní intervence NIC 75% hranice (po sečtení v kategorii denní a týdenní péče), byla identifikována jako intervence NIC, která je použitelná minimálně jednou týdně v klinické péči prostředí intenzivní péče. 386 záznamových archů, ve kterých všeobecné sestry označovaly frekvence použití vybraných 184 intervencí Klasifikačního systému NIC. Získaná data byla získána z klinické praxe intenzivní péče u 12 poskytovatelů zdravotnických služeb v České republice (ČR). Ke statistickému porovnání získaných dat z jednotlivých klinických pracovišť byl použit Pearsonův chi kvadrát, na hladině významnosti 5 % (p ≤ 0,05). Výsledky: 46 intervencí z vybraných intervencí NIC, bylo identifikováno jako intervence, použitelné minimálně jednou týdně v klinické péči prostředí intenzivní péče ČR. U 14 intervencí NIC nebyly shledány signifikantní rozdíly v označení použitelnosti jednou týdně na pracovištích ARO a JIP interního a chirurgického typu. U 32 intervencí NIC byly shledány signifikantní rozdíly v označení použitelnosti jednou týdně v klinické praxi mezi výše uvedenými pracovišti. Použitelnost těchto intervencí NIC minimálně jednou týdně častěji označily všeobecné sestry na pracovišti ARO. Závěry: Ze 184 mapovaných intervencí Klasifikačního systému NIC bylo identifikováno 46 intervencí, které všeobecné sestry označily, jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Higher frequency of presence was recorded by Anesthesiologic Resuscitation ward nurses. Conclusions: Analysis from 184 intervention NIC confirms the possibility 46 of serviceability of classification system NIC in our country. V devadesátých letech minulého století dochází k rozvoji laparoskopické chirurgie. Cestu ukázali chirurgickým oborům gynekologové. Vývoj metody započal simultáně na třech místech. V USA, Francii a Německu. V roce 1989 provedl Reich v USA první LAVH. Querleu v roce 1991 první laparoskopickou lymfadenektomii a v roce 1992 kombinoval Dargeant Schautovu radikální vaginální hysterektomii s laparoskopickým přístupem. Systematická pánevní a alternativně paraortální lymfadenektomie je součástí radikálních laparoskopických výkonů. Stanovili jsme novou nomenklaturu parametrií, snažíme se šetřit autonomní inervaci. Zpočátku byli operatéři autodidakté, v poslední době jsme svědky standardizace indikací a techniky. Bezpečnost výkonu a komfort operatéra zvyšuje vyspělá technika, 3D zobrazení, vysokofrekvenční bipolární technika, konstrukce trokarů omezujících možnost cévních poranění. Postupně byl téměř na všech specializovaných pracovištích opuštěn vaginální přístup a kompletní operace je prováděna pouze laparoskopicky pod mezinárodně srozumitelným označením TRLH (totální, radikální, laparoskopická hysterektomie). Systematicky se budeme ve svém sdělení věnovat anatomickým aspektům, aplikaci trokarů, základním preparačním krokům a postupům, které omezí možné komplikace, zejména ze strany močového měchýře a ureterů. Zmíníme nutné technické vybavení, základy bezpečné práce s vysokofrekvenční koagulací. Na závěr probereme pooperační péči., In the nineties we experienced a massive development of laparoscopic surgery. Gynecologists were the first who paved the way. The method was simultaneously developed in three countries - the United States, France and Germany. From the United States the vaginal laparoscopically assisted procedures have came to Europe. The French and German schools were based on the historical knowledge of the Schauta radical hysterectomy. In 1989 Reich was the first who operated the LAVH, followed by Querleu in 1992 with lymphadenectomy and Dargeant who combined the Schauta procedure with the laparoscopic lymph node dissection. It was the laparoscopy that taught us the proper functional anatomy. A new nomenclature of the parametries was defined. Careful attention is given to the preservation of the autonomous inervation. In the beginning all laparoscopic surgeons were self-taught. Nowadays a standardized laparoscopic techniques are implemented for a radical laparoscopic operations. The safety of this method is enhanced by using advanced technologies like 3D imaging, high frequence bipolar tools, safety trocars. More often the vaginal approach is being abandoned and the whole operation is performed laparoscopically. A generally accepted term for this operation is TRLH which stands for Total Radical Laparoscopic Hysterectomy. We will systematically describe the anatomical aspects of the dissection technique regarding the save preparation of the ureters and the bladder.
