Druhá část sdělení o problematice stopových prvků v kritických stavech je věnována mědi a selenu. Kuprémie po krátkém poklesu po traumatu stejně jako u systémové infekce stoupá. Ceruloplasmin, na který je v plazmě vázána, je reaktantem akutní fáze. Deficit mědi vyžadující úhradu vzniká při rozsáhlých popálených plochách, ztrátách sekretů zažívacího traktu a negativní bilanci při hemodialýze. Protože je měď nezbytná pro utilizaci železa při tvorbě transferinu a hemoglobinu, vede její nedostatek k hypochromní anémii. Je uvedeno doporučené dávkování mědi při výše uvedených ztrátách. Selen má řadu funkcí významných pro přežití kritických stavů, mezi které patří ochrana před organickými peroxidy, přeměna tyroxinu v aktivní trijodtyronin, zlepšení odpovědí na glukokortikoidy, kontrola glykémie, inhibice adhezních molekul a vliv na funkci T lymfocytů. Selenémie je při systémovém zánětu snížená a s tíží stavu (např. s rozvojem sepse) progresivně dále klesá. Práce referuje o řadě klinických pokusů se suplementací různě vysokých dávek selenu, podávaných maximálně po dobu dvou týdnů. Mortalita významně klesla zatím v jen jednom rozsáhlém klinickém pokusu, mezi dalšími jednotlivě uváděnými pozitivními výsledky jsou např. zlepšení skóre víceorgánové dysfunkce (SOFA), úprava nízkých hodnot cholesterolu, prealbuminu, glutathionperoxidázy, pokles C-reaktivního proteinu. Jsou diskutovány optimální terapeutické dávky. Pro měď i selen jsou uvedeny i toxické projevy předávkování, které se samy mohou stát příčinou kritických stavů., The second part of the communication on the issue of trace elements in critically ill patients is devoted to copper and selenium. After a short decrease in trauma, as well as in systemic inflammation, plasmatic level of copper increases. The copper is bound on ceruloplasmin, which is an acute phase reactant. Copper deficiency requiring substitution occurs in extensive burns, losses from the gastrointestinal tract or in negative balance during haemodialysis. Copper is necessary for the utilization of iron in the formation of haemoglobin and transferrin and therefore the insufficient level results to hypochromic anaemia. The authors present the recommended doses for copper supplementation in the aforementioned losses. Selenium possesses a number of functions important for the surviving of critical illness, including protection from the organic peroxides, the conversion of thyroxine to active triiodothyronine, improvement in response to glucocorticoids, glycaemic control, inhibition of adhesion molecules and the effect on the function of T-lymphocytes. The level of selenium is reduced in systemic inflammation and it falls progressively depending on the severity of condition (e.g. sepsis). The article reports a number of clinical trials with different regimens of supplementation with high doses of selenium, administered for maximal period of two weeks. The mortality rate has dropped significantly so far in one large clinical trial only. Some other trial have reported positive results in some outcomes: SOFA score improvement in patients with multiple organ dysfunctions, elevation of previously low cholesterol, praealbumin and glutathione peroxidase levels, and reduction in C-reactive protein values. The optimal therapeutic doses are discussed in detail. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Interventions of paediatric obstructive sleep apnea syndrome are complex, varied and multidisciplinary. The goal of the treatment is to restore optimal breathing during the night and to relieve associated symptoms. Evidence suggests that the surgical intervention with removal of the tonsils and adenoids will lead to significant improvements in the most incomplicated cases, as recently reported from a meta-analysis. However, post-operative persistence of this syndrome in paediatric population is more frequent than expected, which supports the idea of the complexity of this syndrome. Adenotomy alone may not be sufficient in children with OSAS, because it does not address oropharyngeal obstruction secondary to tonsillar hyperplasia. Continuous positive airway pressure can effectively treat this syndrome in selected groups of children, improving both nocturnal and daytime symptoms, but poor adherence is a limiting factor. For this reason, CPAP is not recommended as first-line therapy for OSAS when adenotonsillectomy is an option. It is now being investigated the incorporation of nonsurgical approaches for milder forms and for residual OSAS after surgical intervention. Althought adeno-tonsillar hypertrophy is the most common for OSAS in children; obesity is emerging as an equally important etiological factor. Therefore an intensive weight reduction program and adequate sleep hygiene are also important lifestyle changes that may be very effective in mitigating the symptoms of this syndrome. Pharmacological therapy (leukotriene antagonists, topical nasal steroids) is usually use for mild forms of OSAS and in children with associated allergic diseases. Many aspects of surgical treatment of intestinal malrotation in children remain to be debatable. In the opinion of the majority of the specialists, surgical treatment is required after the diagnosis taking into account serious complications of intestinal malrotation. Purpose. The purpose of this research was to conduct an analysis of surgical tactics and operative treatment method for isolated and associated intestinal malrotations in children. Material and methods. We observed 123 children at the age of one day to 15 years with malrotation during the period of 2002 to 2013. Results. We presented the data from observing 123 children at the age of one day to 15 years with various clinical-anatomic forms of intestinal malrotation over from 2002 to 2013. In 62 patients (50.4%), the evidences of the high intestinal obstruction were prevalent, while 61 (49.6%) showed signs of low intestinal obstruction. 116 patients (94,3%) were given operative intervention: radical – 95(81,9%) and palliative – 21 (18,1%). In 56 % of the cases, various simultaneous surgeries were required. There are proposed differential approaches in relation to anatomic form of malrotation and possibility of the fixation of large intestine in the physiological position. Conclusion. The results obtained from the operative treatment are presented. Background. Nonunion of the lateral humeral condyle are of the complex pathology of the elbow joint, occurring relatively often and resulting in disability of children. The treatment of nonunion of the lateral humeral condyle of humerus with cubitus valgus remains controversial. Purpose of this report was improvement of the results of surgical treatment of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus with use of differential approach to the surgical strategy. Material and methods. We were observing 28 children (17 boys and 11 girls) with nonunion and longstanding nonunion of lateral condyle of humerus, with various degrees of severity. There were used MRI and Xray investigations for differential study of the patients divided into 3 groups in relation to stability and nonstability of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus. Results. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarovs technique. The mean postoperative humerusulna angle was 6,0 degrees of cubitus valgus. All of reverse Tosteothomies healed uneventfully, and there was no loss of correction postoperatively. The mean duration of followup was 7 years. The overall results were excellent in 15(53,5%) patients, good in 11(39,3%) patients, and fair in 2(7,2%) patients. Autoři ve svém příspěvku uvádějí první výsledky šetření, které bylo zaměřeno na zjištění hmotností školních tašek chlapců a dívek 1. a 2. tříd základních škol v olomouckém regionu. Celkem byla zjištěna hmotnost u 502 školních tašek, změřeno 502 dětí (263 dívek a 239 chlapců). Z výsledků měření je zřejmé, že hmotnost tašky se zvyšuje s postupem žáků do vyšší třídy. Největší rozdíl mezi minimální a maximální hodnotou hmotnosti tašky byl zjištěn u 8letých dívek, činil 8,9 kg. Výsledky ukázaly, že procentuální podíl hmotnosti tašky doporučeným 10 % tělesné hmotnosti dítěte odpovídal pouze u 17,5 % dětí. 82,5 % dětí pak nosí aktovky těžké (10–20 %), přičemž 18,3 % dětí nosí aktovky o hmotnosti nad 20 % své tělesné hmotnosti, což už lze považovat za extrémně těžké. Nepřiměřená hmotnost školní tašky pak může ovlivňovat držení těla a výskyt bolesti zad., Effects of the weights of school bags carried by pupils from the first two classes of primary schools in the Olomouc region (502 schoolbags belonging to 263 girls and 239 boys) on the pupils’ posture were examined. The weight of the schoolbags of pupils from the 2nd year class was found to be higher than of pupils in the first year. The greatest difference between minimum and maximum weights (8.9 kilograms) was detected in the schoolbags of 8 year old girls. Only 17.5% of children carried bags that were within the recommended 10% of the child’s body weight. The acute destructive pneumonias (ADP) occupy up to 80% of the total number of pneumonias. They require constant improvement of treatment strategy. Nowadays the use of surfactants is a part of most treatment protocols. The aim was to study the features of the solid phase bronchoalveolar lavage in children with the ADPs in the dynamics of complex treatment with exogenous surfactant. Material and methods: We examined 39 patients of contaminated surgery. We identified 2 groups of patients. The patients of first group (n=27) had pulmonary pleural form of ADP, the second group (n=12) had pulmonary form of ADP. All patients got classical treatment and the earlier draining of pleural cavity. We used as an antiseptic reamberin 1.5% by 10 ml/kg and endobronchially injected exogenous surfactant Bl in dose12 mg/kg body weight a day, 6 mg/kg every 12 hours. All the children were made a bronchoscopy to obtain BAL to study the crystallization properties. The solid phase of BAL was studied by method of cuneal dehydration. Results: All facies before treatment were divided into two groups according to classification of facies of biological fluids. Only the facies of the second and the third types were detected there. It was revealed that the sizes of the zones of the facies were different in the comparison groups before treatment and after. And the level of crystalline structures and amorphous aggregates were different in the groups with different degrees of inflammation. Conclusion: So, we can assume that the change in surfactant system is characterized by changes in the morphological structure of solids phases of BAL. Autoři probírají kazuistiku 12leté pacientky s alergickou dispozicí, u níž v průběhu akutního horečnatého stavu s řadou komplikací byl v séru zachycen paraprotein IgM lambda. Při kontrolní elektroforéze, provedené za 4 měsíce, již nebyl prokazatelný. V článku je uvedena literatura zabývající se problematikou transientních paraproteinemií, diskutovány možné vztahy a příčiny. Transientní paraproteinemie jsou zřejmě častější, než se předpokládá., The case study of a 12 - year-old girl with allergic disposition is discussed. During an acute feverish condition accompanied with a number of complications, the IgM lambda paraprotein was found in the serum sample. In the subsequent control electrophoresis which was carried four months later, this paraprotein was not found any more. Literature dealing with the problems of transient paraproteinemias is included in this work and possible causal relations are discussed. Na 3 kazuistikách je demonstrován současný pohled na diagnostiku a léčbu trombózy esovitého splavu. Trombóza vzniká na infekčním nebo neinfekčním podkladě. Příčinou infekční trombózy je akutní mastoiditida, častá u dětí obvykle spojená s hyperkoagulačním stavem nebo dlouhodobý chronický středoušní zánět s cholesteatomem, především u dospělých s poruchou imunity nebo diabetes mellitus. Neinfekční trombóza vzniká především na podkladě poruchy koagulace a postihuje zvláště mladé ženy, kuřačky, gravidní nebo ženy užívající hormonální antikoncepci. Léčba zánětlivých otogenních trombóz esovitého splavu je založena na podávání antibiotik a chirurgické sanaci spánkové kosti; podání antikoagulační léčby a chirurgické vybavení trombu není jednoznačné. Naopak antikoagulační terapie je klíčová u neinfekčních trombóz splavu. U všech nemocných s trombózou splavu je třeba cíleně pátrat po přítomnosti vrozeného či získaného trombofilního stavu., Three case studies demonstrate current views on the diagnosing and treatment of thrombosis of the sigmoid sinus. The factors causing thrombosis are of infectious and non-infectious nature. The cause of infectious thrombosis is acute mastoiditis which is frequent in children and is usually linked with a state of hypercoagulation, or a long-term chronic middle ear inflammation with cholesteatoma, especially in adult patients with an immune system disorder or diabetes mellitus. Non-infectious thrombosis primarily develops as a result of disturbed coagulation and prevailingly affects young women, female smokers, pregnant women or women taking hormonal contraception. The treatment of inflammatory otogenous thrombosis of sigmoid sinus is based on the administration of antibiotics and surgical sanitation of the temporal bone; administration of anticoagulation medication and surgical treatment of the thrombus is still an issue. On the other hand, anticoagulation therapy is a key element in non-infectious sigmoid sinus thromboses.
