Crohnova nemoc se často projevuje čistě jako zánětlivé onemocnění, ale u mnoha pacientů se vyvine v komplikované onemocnění charakterizované tvorbou striktur nebo píštělí. Přesná etiopatogeneze tohoto závažného onemocnění není zcela jednoznačně objasněna, a to i přes veškeré úsilí výzkumu a přes poznání řady dílčích patogenetických mechanizmů. Mnoho studií naznačuje, že se zvýšeným rizikem rozvoje komplikované formy onemocnění souvisí zejména varianty v NOD2/CARD15 genu. Samotná přítomnost variant v NOD2 genu je pouze jen jedním z faktorů přispívajících k rozvoji tohoto onemocnění. U geneticky predisponovaných jedinců v kombinaci s působením faktorů prostředí dochází k porušení vrozené (např. porušená střevní bariéra, dysfunkce Panethových buněk) i adaptivní (např. nerovnováha efektorových a regulačních T lymfocytů a cytokinů, migrace a retence leukocytů) imunitní odpovědi vůči snížené pestrosti střevních komenzálů. Data dosud provedených metaanalýz neposkytují jednoznačné důkazy k podpoře top-down strategie terapie závislé pouze na jediné variantě v NOD2 genu, ale na druhé straně v případě vysoce rizikových pacientů se dvěma variantami v NOD2 genu data ukazují, že cílená a časná intenzivní terapie by mohla být prospěšná. Na tyto otázky by mohly odpovědět další prospektivní studie., Crohn’s disease is often purely inflammatory, but most patients develop complicated disease with strictures or fistulae. Specific etiopathogenesis of this severe disease is not definitely clear despite research efforts and learning of many pathogenetic mechanisms. Many studies have suggested that NOD2 mutations are associated with increased risk of complicated disease. Presence of NOD2 mutation itself is just one of factors contributing to development of this disease. Genetically predisposed individuals in combination with influence of environmental factors result in a disturbed innate (i.e., disturbed intestinal barrier, Paneth cell dysfunction) and adaptive (i.e., imbalance of effector and regulatory T cells and cytokines, migration and retention of leukocytes) immune response towards a diminished diversity of commensal microbiota. Osteoporóza je závažné onemocnění charakterizované vysokou morbiditou a mortalitou v důsledku atraumatických fraktur. Kromě vlivu vnějšího prostředí se v patogeneze osteoporózy uplatňují faktory vnitřní (hormonální nerovnováha a genetické pozadí). Článek podává přehled kandidátních genů pro osteoporózu, které klasifikuje podle metabolických okruhů regulujících minerální denzitu nebo kvalitu kosti (okruh estrogenní, RANKL/RANK/OPG, mevalonátový, okruh Wnt, geny pro kolagen a vitamin D). Zabývá se také perspektivou praktického využití farmakogenetiky (identifikace jednotlivých kandidátních genů pomocí PCR) nebo farmakogenomiky (plošné měření škály genů pomocí GWAS) při výběru optimální léčby osteoporózy. Potencionálními prediktory odpovědi na antiresorpční léčbu jsou geny pro ER, FDPS, Cyp19A1, VDR, Col 1A1 a gen pro signalizaci Wnt. Polymorfizmy genů CYP2C, ale také FDPS mohou naopak identifikovat nemocné s vysokým rizikem nežádoucí odpovědi na bisfosfonáty (osteonekróza čelisti). Limitujícím faktorem výzkumu genetiky osteoporózy je nedostatečný konsenzus výsledků asociačních studií. Vysvětlení kontroverzních výsledků lze hledat v rozdílných metodických přístupech (rozsah a homogenita souboru, etnické rozdíly nebo vazebná nerovnováha mezi geny). Klíčovým úskalím asociačních studií je nízká variabilita (7-10 %) kostních fenotypů, které souvisí se zkoumaným genem. Nicméně identifikace nových genů a ověřování jejich asociace s kostními parametry oběma metodami zůstává velkou výzvou s cílem optimalizace prevence a léčby osteoporózy., Osteoporosis is a serious disease characterized by high morbidity and mortality due to atraumatic fractures. In pathogenesis of osteoporosis, except environment, internal factors, such as hormonal dysbalance and genetic background, are also in play. In this review, candidate genes for osteoporosis are classified accorging to metabolic or hormonal pathways, which regulate bone mineral density/and or quality (estrogen, RANKL/RANK/OPG, mevalonate, Wnt circuit, genes for collagen and vitamin D). Authors discuss the perspectives of practical utilization of pharmacogenetics (identification of single candidate genes using PCR) or pharmacogenomics (using genome wide association studies) in choise of optimal treatment of osteoporosis. Potentional predictors of effectivity of antiresorption therapy are genes ER, FDPS, Cyp19A1, VDR, Col1A1 and gene of Wnt pathway. Moreover, polymorphisms of CYP2C gene, but also FDPS may identify patients with high risk of undesirable effects of bisphosphonates (osteonecrosis of jaw). Unfortunately, results of the most association studies has not been confirmed by other investigators. The controversial results could be explained by different methodic approches in individual studies (different sample size, homogenity of investigated groups, ethnic differences or linkage disquilibrium between genes). Key cliff of association studies is low variability (7-10 %) of bone phenotypes associated with investigated genes. Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem., Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. Cieľ: Cieľom našej práce bolo identifikovať kauzatívne germinálne mutácie u suspektných NF1 pacientov za účelom diferenciálnej diagnostiky a zozbieranie čo najväčšieho súboru pacientov pre prípadné genotypovo-fenotypové korelácie. Súbor a metodika: Náš súbor pozostával zo 107 nezávislych pacientov zo Slovenska, ktorý boli v období rokov 2008-2013 klinickými genetikmi posúdení ako suspektní pre NF1. Na identifikáciu mutácií bolo použité sekvenovanie cDNA NF1 génu, ktoré nám umožnilo úspešnú identifikáciu zostrihových mutácií, ako aj MLPA analýza umožňujúca identifikáciu rozsiahlejších delécií v géne. Výsledky: V súbore 94 nepríbuzných slovenských pacientov, ktorí spĺňali základné diagnostické kritériá pre neurofibromatózu, sme za použitia našich metód identifikovali germinálne mutácie v NF1 géne u 83 z nich (88,3 %). Zaznamenali sme vysoký podiel mutácií, ktoré boli zatiaľ identifikované len v slovenskej populácii (41/83, 49,4 %), a u 27/42 testovaných rodinách sme potvrdili mutáciu de novo (64,3 %). Genotypovo-fenotypové korelácie odhalili zvýšený výskyt očných gliómov u pacientov s mutáciou na 5’ konci NF1 génu. Závery: Kombinácia cDNA analýzy a MLPA je efektívnou pre identifikáciu mutácií v NF1 géne. Na základe tejto diagnostickej metódy sme odhalili aj často nesprávne klasifikované netypické zostrihové mutácie. Naše výsledky využili klinickí genetici pri diferenciálnej diagnostike ochorenia, hlavne v sporných prípadoch. Kľúčová slová: NF1 gén – kauzatívna germinálna mutácia – RNA zostrih – genotypovo-fenotypové korelácie – de novo mutácie –diferenciálna diagnostika, Aim: The aim of our study was to identify causative germline mutation in suspected NF1 patients, in order to help differential diagnostics, as well as to collect as large as possible group of patients for a possible genotype-phenotype correlations. Material and methods: Our set of patients consisted of 107 Slovak patients, recruited between 2008 and 2013, who were considered for a diagnosis NF1 by clinical genetics. In order to identify mutations, we employed NF1 cDNA sequencing that enabled us to also detect splicing mutations, as well as MLPA analysis that enables identification of larger deletions. Results: By employing the selected methods in our set of 94 unrelated Slovak patients who fulfilled the basic NF1 diagnostic criteria, we uncovered germline mutations in the NF1gene in 83 of them (88.3%). We observed a high proportion of mutations identified in Slovak population only so far (41/83, 49.4%), and we confirmed de novo mutation in 27/42 tested families (64.3%). Genotype-phenotype correlations revealed an increased incidence of optic pathway glioma in patients with a mutation in the 5´end of the NF1 gene. Conclusion: Combination of cDNA analysis and MLPA provides an effective method for identification of mutations in the NF1 gene. By employing these methods, we were able to also identify frequently incorrectly classified atypical splicing mutations. Clinical geneticists used our results in the differential diagnosis of the disease, especially in contested cases. The purpose of study was to analyze clinical and genetic polymorphism of Duchenne/Becker progressive muscular dystrophies among patients with neuromuscular diseases in Uzbekistan. 106 male patients with progressive pseudohypertrophic forms of muscular dystrophy were retrospectively and prospectively analyzed in the period from 2004 till 2014: 93 patients with Duchenne PMD aged from 3 years to 18 years and 13 patients with Becker PMD aged from 10 years to 25 years, who had been examined in the medico-genetic consulting department of the Republican Center “Mother and Child Screening” of Tashkent city. Interventions of paediatric obstructive sleep apnea syndrome are complex, varied and multidisciplinary. The goal of the treatment is to restore optimal breathing during the night and to relieve associated symptoms. Evidence suggests that the surgical intervention with removal of the tonsils and adenoids will lead to significant improvements in the most incomplicated cases, as recently reported from a meta-analysis. However, post-operative persistence of this syndrome in paediatric population is more frequent than expected, which supports the idea of the complexity of this syndrome. Adenotomy alone may not be sufficient in children with OSAS, because it does not address oropharyngeal obstruction secondary to tonsillar hyperplasia. Continuous positive airway pressure can effectively treat this syndrome in selected groups of children, improving both nocturnal and daytime symptoms, but poor adherence is a limiting factor. For this reason, CPAP is not recommended as first-line therapy for OSAS when adenotonsillectomy is an option. It is now being investigated the incorporation of nonsurgical approaches for milder forms and for residual OSAS after surgical intervention. Althought adeno-tonsillar hypertrophy is the most common for OSAS in children; obesity is emerging as an equally important etiological factor. Therefore an intensive weight reduction program and adequate sleep hygiene are also important lifestyle changes that may be very effective in mitigating the symptoms of this syndrome. Pharmacological therapy (leukotriene antagonists, topical nasal steroids) is usually use for mild forms of OSAS and in children with associated allergic diseases. Special orthodontic treatment and oropharyngeal exercises are a relatively new and promising alternative therapeutic modality used in selected groups of children with OSAS. and A. Šujanská, P.
