Úvod: Systémový lupus erythematodes (SLE) je chronické autoimunitní onemocnění charakterizované multisystémovým postižením. Cílem naší studie bylo ozřejmit výskyt snížených hodnot koncentrací imunoglobulinů (s-Ig) v séru v populaci nemocných se SLE. V retrospektivní studii bylo zhodnoceno 799 výsledků vyšetření s-Ig získaných od 157 nemocných splňujících revidovaná klasifikační kritéria. Výsledky: Sérová koncentrace imunoglobulinů nižší než dolní hranice normy byla zjištěna celkem u 29 ze 157 nemocných (18,5 %). Nejčetnější byl výskyt izolovaného snížení IgG 12 ze 157 (7,6 %), 2 nemocné naplňovaly kritéria selektivního deficitu IgA a v jednom případě se jednalo o vzácnou koincidenci s běžnou variabilní imunodeficiencí (common variable immunodeficiency – CVID). Článek podrobněji prezentuje 2 případy nemocných se SLE komplikované těžkou hypogamaglobulinemií a infekcemi vyžadujícími dlouhodobou substituční léčbu imunoglobuliny. V prvním případě se jedná o vysoce pravděpodobný případ běžné variabilní imunodeficience a v případě druhém pak o léky indukovanou hypogamaglobulinemii. Tato žena s anamnézou maligního lymfomu a současným závažným multiorgánovým postižením při SLE byla léčena rituximabem po selhání konvenční léčby. Závěr: S protilátkovými deficiencemi se můžeme setkat i u nemocných se SLE. Opakované měření sérové koncentrace imunoglobulinů by mělo být součástí rutinního sledování těchto nemocných. Na možnost CVID bychom měli myslet v případě opakujících se sinopulmonárních infekcí, zejména v kombinaci s absencí aktivity SLE. Léčba rituximabem může vést i u nemocných se SLE k dlouhodobé supresi B-lymfocytů s rozvojem sekundárního protilátkového deficitu, který v některých případech vyžaduje substituční léčbu imunoglobuliny., Introduction: Systemic lupus erythematosus (SLE) is a chronic autoimmune multisystem disease. The aim of our study was to clarify the frequency of decreased serum immunoglobulin levels in SLE patients. There were evaluated 799 results of serum immunoglobulin levels gained from 157 patients fulfilling revised ACR criteria in the retrospective study. Results: The immunoglobulin levels under the normal range were found in 29/157 (18.5 %) patients. The most frequent was isolated reduction of IgG 12/157 (7.6 %), two persons fulfilled criteria for selective IgA deficiency, and one case possible diagnosis of common variable immunodeficiency (CVID). Additionally we report two cases of SLE patients complicated by severe hypogammaglobulinaemia and infectious complications with necessity of long-term immunoglobulin substitution therapy. The diagnosis of CVID is highly probable in the first case. The second case presents sever drug-induced hypogammaglobulinaemia. This female with lymphoma history and multiorgan impairment due to acute SLE was treated with rituximab after convention therapy failure. Conclusion: Humoral immunodeficiency may occur in SLE patients. The monitoring of serum immunoglobulin levels could be a routine in these patients. The CVID diagnosis is possible in patients suffering from recurrent sinopulmonary infections, especially in combination with absence of lupus activity. V případu uvádíme kazuistiku pacienta s výskytem solitární metastázy adenokarcinomu prostaty do glans penis cca pět let po radioterapii pro lokálně pokročilý karcinom prostaty. Popisujeme zvolenou diagnostickou a léčebnou strategii a následné sledování. I přes špatnou prognózu a postupnou progresi onemocnění je uvedený pacient v relativně dobrém stavu, asymptomatický., We present a case of a patient with a solitary metastasis of the prostate cancer to the glans penis which occurred approximately five years after the radiotherapy with curative intent for the treatment of locally advanced adenocarcinoma of the prostate. We describe the diagnostic and treatment strategy and follow-up. Spontaneous rupture is a rare and dramatic complication of hepatocellular carcinoma (HCC), burdened by a high mortality. Here we describe a case of a 73-year-old man, who arrived at the ER because of syncope, and acute epigastric and right upper quadrant abdominal pain. He had a history of hepatitis C-related liver cirrhosis and HCC in treatment with sorafenib. The physical examination showed a state of hemorrhagic anemia with the presence of blood in the peritoneal cavity. The patient underwent an urgent liver resection. Thirty days after surgery, he was in good general condition. Sorafenib is a multikinase inhibitor recently introduced in the therapy of patients with advanced HCC. Among the various side effects reported in patients treated with sorafenib, there is a higher risk of bleeding. Je popsán případ legionelózy u staršího muže, který začal chřipkovými symptomy s následným zhoršováním a hospitalizací a při další progresi pokračoval umístěním na ARO fakultní nemocnice. Až zde byla diagnostikována legionelóza. Epidemiologické šetření se zaměřilo na vodovodní vodu v bytě nemocného a odebrané vzorky potvrdily podezření na akviraci nákazy v místě bydliště. Izoláty z pitné a teplé vody obsahovaly bakterii Legionella pneumophila s denzitou 102–103 KTJ/100 ml. Za jednoznačnou příčinu kontaminace vody byl označen nedostatečný ohřev vody v nezaregulovaném teplovodním systému. Účinným opatřením k nápravě byla termodezinfekce domovního vodovodního systému a chemodezinfekce pomocí instalovaného generátoru chlordioxidu., Described is a case of legionellosis in an elderly male, that began with flu symptoms with consequent deterioration and hospitalization progressing to the IC unit. Only then was legionellosis diagnosed. Epidemiological investigations were focused on tap water in the patient's flat; samples taken confirmed the suspicion. Isolates from drinking water as well as warm tap water contained Legionella pneumophila organisms at 102–103 CFU/100 ml. The unequivocal cause of water contamination was the insufficient heating of the water in the unregulated hot water meins system. Ledvinné angiomyolipomy (AML) jsou mezenchymové benigní tumory, řadí se mezi hamartomy mohou se vyskytovat osamoceně nebo se sdružují s komplexem tuberózní sklerózy (TSC). Pacienti s AML mají riziko vzniku spontánního, v některých případech až život ohrožujícího krvácení. V tomto sdělení jsou popsány tři případy spontánního krvácení AML: první u 69leté pacientky s prokázanou angiomyolipomatosou ledvin oboustranně, druhý u 60leté pacientky s dlouhodobě sledovaným solitárním AML a třetí u mladého 28letého pacienta s diagnózou tuberózní sklerózy., Renal angiomyolipoma is the mesenchymal benign tumor which belongs to the group of hammartomas. It can occur sporadically or in association with tuberous sclerosis complex. The possible morbidity associated with angiomyolipoma is spontaneous life-threatening haemorrhage. Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life. Advanced diagnostic and imaging techniques, a better understanding of the biomechanics and biology of Charcot neuroarthropathy, and suitable osteosynthetic material enables diabetic limb salvage. and T. Úvod: Synchronní nádory pankreatu a jiné primární nádory nejsou časté. V autoptických materiálech se tato koincidence uvádí kolem 5,6 %. Současné resekce pankreatu a jiného orgánu pro synchronní malignity jsou publikovány pouze ve zcela ojedinělých souborech případů či jednotlivých kazuistikách. Autoři předkládají tři případy řešené chirurgicky v jedné době. Kazuistiky: V sestavě 400 resekcí pankreatu (2006−2014) byli tři nemocní s karcinomem pankreatu a jinou synchronní maligní duplicitou. Dva nemocní se prezentovali symptomatologií nádoru pankreatu – bolesti břicha, úbytek na váze, obstrukce žlučových cest. Druhými nádory byly asymptomatický tumor antra žaludku a tumor ledviny. Pacienti podstoupili pravostrannou pankreatoduodenektomii (PDE) a subtotální gastrektomii s lymfadenektomií, resp. levostrannou nefrektomii. Histopatologické nálezy byly vývodový adenokarcinom hlavy pankreatu pT2N1M0, G3 a adenokarcinom žaludku pT1N1M0, G2, resp. vývodový adenokarcinom hlavy pankreatu pT3N1M0, G2 a renální papilární karcinom pT1bN0M0, G3. Pooperačně u obou následovala chemoterapie gemcitabinem. První nemocný zemřel 12 měsíců od operace s generalizací do jater. Druhý nemocný zemřel 19 měsíců od operace, s nově zjištěným karcinomem prostaty. Třetí nemocný byl vyšetřován pro bolesti epigastria a při gastroskopii byl nalezen tumor antra žaludku, při CT pak asymptomatický malý tumor hlavy pankreatu. Podstoupil pravostrannou PDE a subtotální gastrektomii s lymfadenektomií. Histopatologické nálezy byly: vývodový karcinom hlavy pankreatu pT2N0M0, G1 a adenokarcinom žaludku pT2bN2M0, G3. Následovaly radioterapie a chemoterapie 5-fluorouracilem a leukovorinem. Žije téměř 5 let od operace, bez recidivy. Závěr: Přežití nemocných operovaných pro duplicitní synchronní malignitu pankreatu a jiného primárního nádoru v naší sestavě významně ovlivnily vlastnosti pankreatického tumoru. Pokud se duplicita projevila symptomy pankreatického tumoru, bylo přežití významně kratší nežli u incidentálně zjištěného tumoru pankreatu při manifestaci jiné malignity., Introduction: The occurence of synchronous pancreatic cancer and other primary cancer is not frequent and reaches about 5.6% as reported in autoptic studies. Double resections of the pancreas with another organ due to synchronous malignancies have been published only in quite sporadic sets of cases or individual case reports. The authors present three cases of synchronous pancreatic malignancies and stomach or renal cancers treated with surgery, including results and survival. Cases: Three patients with synchronous double cancer were identified in a series of 400 pancreatic resections (2006−2014). Two patients presented with symptoms of pancreatic periampullary tumors (bile duct obstruction , weight loss and abdominal pain). The second malignancies were identified as incidental during diagnostic work-up (asymptomatic cancer of the stomach, kidney). Pancreatoduodenectomies (PDE) with lymphadenectomies were performed due to ductal adenocarcinomas (pT2N1M0 G3 and pT3N1M0 G2). The second procedures included subtotal gastrectomy with lymphadenectomy (gastric adenocarcinoma pT1N1M0, G2) and nephrectomy (renal papillary carcinoma pT1bN0M0, G3). Postoperative adjuvant chemotherapy with gemcitabine was given in both patients. Survival rates were 12 and 19 months, respectively. The third patient suffered from abdominal pain and weight loss. Diagnostic work-up revealed stomach carcinoma and early pancreatic adenocarcinoma. Double resection – subtotal gastrectomy with lymphadenectomy and pancreatoduodenectomy with lymphadenectomy – was performed. Gastric adenocarcinoma pT2N2M0, G3 and pancreatic ductal papillary-mucinous adenocarcinoma pT2N0M0, G1 were found in the specimens. Adjuvant radiochemotherapy with 5-fluorouracil and leukovorine was given postoperatively. This patient is still alive nearly 5 years after the surgery, without any reccurence. Conclusion: The survival of patients with double synchronous pancreatic malignancies and other primary tumors in our set seems to be influenced by the stage and biology of pancreatic cancer. The survival was worse when the duplicity was presented with symptoms of pancreatic cancer. Autoři prezentují případ 73leté ženy léčené v posledních 15 letech pro arteriální hypertenzi diuretiky (tiazidy, amiloridem a indapamidem) a současně antidepresivem trazodonem. Po 6 letech této léčby začala pacientka trpět manifestní osteoporózou s vícečetnými frakturami i po banálním traumatu a současně byla po celou tuto dobu dokumentována těžká hyponatremie bez adekvátní suplementace. Osteoporóza a hyponatremie u této pacientky spolu s největší pravděpodobností těsně souvisely – autoři diskutují možné patofyziologické mechanizmy na úrovni renálních tubulů, osteoblastů a osteoklastů, hypofyzárně-skeletální osy, syndrom neadekvátní sekrece adiuretinu (SIADH) a možný vliv porušené acidobazické rovnováhy., The authors present a case-report of a 73years old woman treated for arterial hypertension within past 15 years with diuretics (thiazides, amiloride and indapamide) and concomitantly with antidepressive trazodone. The patient developed severe osteoporosis after 6 years of such a treatment with multiple fractures even after minor trauma; during whole this period severe hyponatremia without adequate supplementation was documented.
