Hypertenze bílého pláště (white-coat hypertension – WCH) je velmi častý jev; vyskytuje se asi u jedné třetiny osob s patologickým krevním tlakem (TK) měřeným v ordinaci, a to typicky u žen, ve vyšším věku a nekuřáků. Prognostická data nejsou zcela spolehlivá, protože velká část jedinců s tímto typem TK je léčena na podkladě vysokého TK v ordinaci; existují práce s dlouhodobým sledováním ukazující, že kardiovaskulární (KV) riziko je zvýšené oproti jedincům s normálním TK. Opačný jev je maskovaná hypertenze (MH). V populačních studiích byl zjištěn její výskyt v 13 %. Je častá u mužů, zejména ve věku 30–50 let, kuřáků s nepříznivým rizikovým profilem, a také u diabetiků. Riziko budoucího výskytu kardiovaskulárních příhod se blíží trvalé hypertenzi. Důležité je její vyhledávání, a proto bychom měli doporučovat domácí měření TK i těm jedincům, kteří mají normální TK v ordinaci. Neexistují prospektivní studie léčby WCH ani MH. U obou typů hypertenze lze zvolit jen nefarmakologická opatření, pokud je KV riziko nízké; u těch jedinců, kteří mají zároveň další KV rizikové faktory nebo orgánové komplikace hypertenze, je nutno zvážit léčbu, při níž však můžeme postupovat pouze empiricky., White-coat hypertension (WCH) is very common: it is present in about one third of subjects with high office blood pressure (BP). A typical patient is a non-smoking female at higher age. Prognostic data are not very reliable because most patients are treated on the basis of elevated office BP; some long-term studies have shown that cardiovascular (CV) risk is increased compared to normotensives. The opposite disorder is masked hypertension (MH). Its prevalence in population-based studies is 13 %. MH is common in males, typically aged between 30 and 50 years, smokers with unfavorable risk profile, and it is also frequent in diabetics. The risk of future cardiovascular events is close to that of sustained hypertensives. It is important to search for MH actively, therefore, we should recommend home BP measurements also to those subjects whose office BP is normal. We do not have any prospective data about treatment of either WCH or MH. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Metodika: Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (IM), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Ve větvi se simvastatinem v monoterapii byl cílový LDL‑c < 70 mg/dl. Předpokladem bylo, že ezetimib sníží LDL‑c o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo nonST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října 2005 do července 2010. Nejvíce (40 %) bylo zařazeno v západní Evropě a Severní Americe (38 %). Pacienti se STEMI byli mladší a bylo zde více nemocných bez předchozí léčby statiny. Pacienti s nonSTEMI/NAP měli častěji diabetes mellitus, hypertenzi a předchozí IM. Medián LDL‑c při randomizaci byl 100 mg/dl u STEMI a 93 mg/dl u nonSTEMI/NAP. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %) (p = 0,016) léčených kombinací. Karcinom prostaty patří mezi nejčastější nádorová onemocnění u mužů. V časném a lokalizovaném stadiu je léčitelný, určité procento pacientů však relabuje nebo se diagnóza stanoví až v pokročilejších stadiích, kde jsou léčebné možnosti limitované. Imunoterapie je kromě klasické hormonální terapie, radioterapie a chemoterapie další perspektivní léčebnou modalitou v těchto stadiích. Vznik nádorového onemocnění je mnohastupňový proces komplexní povahy, v němž samotné nádorové buňky představují heterogenní populaci zasazenou do kontextu mikroprostředí nenádorový ch buněk a tkání tvořících složitý nádorový „ekosystém“. Imunitní systém je schopen kontrolovat růst nádorových buněk v raných stadiích vzniku nádoru. S pokročilým růstem nádoru jsou imunitní mechanizmy víceméně paralyzovány a mnohdy na proliferaci nádorových buněk mají paradoxně podpůrný účinek. Aby byla protinádorová terapie úspěšná, musí zohledňovat všechny komplexní aspekty interakce imunitního systému a nádorových buněk včetně mikroprostředí. It is treatable in the early and localized stage; however, a proportion of patients relapse or the diagnosis is made only in more advanced stages when the therapeutic options are limited. In addition to classic hormonal therapy, radiotherapy, and chemotherapy, immunotherapy is another promising treatment modality in these stages. The development of cancer disease is a multistage process of a comprehensive nature wherein the tumour cells themselves represent a very heterogeneous population put into the context of a microenvironment of non-tumour cells and tissues, thus forming a complex tumour "ecosystem". The immune system is capable of controlling tumour cell growth in the early stages of tumour development. With advanced tumour growth, immune mechanisms become more or less paralysed and, in fact, not infrequently have a promoting effect on tumour cell proliferation. In order to be successful, anticancer therapy must take into consideration all the comprehensive aspects of interaction of the immune system and tumour cells, including the microenvironment. Zásadní význam T-lymfocytů pro kontrolu nádoru byl ověřen jak na zvířecích modelech tak klinickými pozorováními. Adoptivní buněčná terapie dosáhla v poslední době výborné účinnosti, což podporuje zjištění, že imunitní systém může bojovat s nádorovým onemocněním. Použití buněčné imunoterapie bylo dříve omezeno technickými možnostmi izolace a expanze antigen specifických T-lymfocytů schopných selektivně zabíjet nádorové buňky. Jedna ze současných slibných strategií je založena na ex vivo manipulovaných pacientových T-lymfocytech, které jsou upraveny pomocí chimérického antigenního receptoru (CAR). Chimérické antigenní receptory obvykle kombinují vazebné místo pro antigen z monoklonální protilátky s efektorovou funkcí T-lymfocytů. Genetická manipulace dává T-lymfocytům požadovanou specificitu, ale také umožňuje ovlivnit jejich aktivační a proliferační potenciál. Takto upravené CAR T-lymfocyty mohou podstatně redukovat nádorovou masu, která exprimuje na nádorových buňkách cílový antigen. Byly testovány různé designy CAR receptorů, různé výrobní procesy a soubory pacientů. CAR T-lymfocyty představují efektivní terapeutický přístup, a to zejména u hematologických malignit. Podávání CD19 specifických CAR-modifikovaných T-lymfocytů, které se váží na B-lymfocyty, u nehodgkinovských lymfomů a chronické lymfocytární nebo akutní lymfoblastické leukémie, bylo v nedávných klinických studiích velice efektivní. Budoucí výzkum musí v oblasti designu a přípravy CAR T-lymfocytů identifikovat kritické parametry, které jsou nezbytné pro efektivní využití adoptivního přenosu T-lymfocytů v onkologické léčbě. Personalizovaná buněčná terapie se pak může rozšířit a stát se slibným léčebným postupem pro hematologické malignity i solidní nádory., A central role of T-cells in the control of cancer has been supported by both animal models and clinical observations. Adoptive cell therapy has recently achieved impressive efficacy, supporting the finding that the immune system can fight cancer. The broaden application of cell immunotherapy has been previously constrained by the technical abilities to isolate and expand antigen-specific T-cells potent to selectively kill tumor cells. One of the promising current strategies is based on ex vivo engineered patients T-cells, which are redirected by a chimeric antigen receptor (CAR). Chimeric antigen receptors usually combine the antigen binding site of a monoclonal antibody with the effector function of a T -cell. The genetic manipulation gives T-cells desired specificity but also enables to tailor their activation and proliferation potential. Such modified CAR T-cells can substantially reduce the tumor burden as long as the targeted antigen is present on the cancer cells. Various CAR designs, manufacturing processes, and study populations have been tested. CAR T-cells represent a powerful therapeutic approach, especially in hematologic malignancies. Administration of CD19-specific CAR-modified T cells, that target B-cell, in non-Hodgkin lymphomas and chronic lymphocytic or acute lymphoblastic leukemia, has been remarkably effective in recent clinical trials. The future research has to identify the critical parameters of CAR T-cells design and engineering that are necessary for effective utilization of adoptive T-cell transfer in cancer therapy. The presence of a broad spectrum of autoantibodies in Sjögren's syndrome (SjS) patients is the result of abnormal B-cell regulation that can be at least partially explained by abnormal BAFF/BAFFR regulation. The objective of this study was to determine both membrane and intracellular expression of BAFF/BAFFR in monocytes and B-cells in peripheral blood of 19 primary Sjögren's syndrome patients and 20 healthy controls using flow cytometry. We also measured sBAFF in serum. Compared to healthy controls, both surface and intracellular expression of BAFF was significantly increased in monocytes and B-cells of SjS patients. Also serum sBAFF level was elevated. Expression of BAFFR on B-cells of SjS patients was surprisingly decreased, but there was no clear increase or decrease within monocytes. Our results indicate that activated monocytes communicate with B-cells via BAFF and BAFFR, so that B-cells are stimulated, but BAFF is also produced to stimulate cells in autocrine way. The decrease of BAFFR expression in SjS patients suggests that there is the mechanism that attempts to take over in order to balance the high level of BAFF. and J. Krejsek, M. Koláčková, I. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol.
