Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Cíl: Výměnný obrázkový komunikační systém (VOKS) je na podmínky České republiky (ČR) modifi kovaná metoda vycházející z komunikačního systému PECS (The picture exchange communication system), který byl vytvořen v Chicagu v USA. Výzkum měl za cíl zmapovat spokojenost uživatelů s touto metodou, zhodnotit obtížnost osvojení metody VOKS a zmapovat dodržování doporučených postupů. Uživatelem autoři rozumí, pracovníka, který využívá metodu VOKS při komunikaci s klienty s PAS. Ve výzkumné části jsou prezentovány výsledky šetření, které se uskutečnilo v pěti vybraných speciálních zařízení a čtyřech školách na území celé ČR. Metody: Výzkumné šetření proběhlo v období od září 2010 do prosince 2010. Byl vytvořen dotazník vlastní konstrukce, který obsahoval 24 otázek. Kritériem pro zařazení respondentů do výzkumu, byla práce s metodou VOKS v zaměstnání, které respondenti aktuálně vykonávali. Výzkumný soubor tvořil 54 respondentů. Výsledky: Analýzou výsledků bylo zjištěno, že většina respondentů je spokojena s metodou VOKS. Rozdíly jsou patrné v době osvojení metody VOKS v praxi. Na tomto konkrétním vzorku respondentů se nepodařilo určit obecně platný čas potřebný k osvojení metody VOKS. Jako problematické se jeví i přesné dodržování postupů. Hlavními důvody nedostatečného dodržování postupů v praxi je potřeba individuálního přizpůsobení klientovi, nedostatek personálu a nedostatek času na nácvik. Velkým problémem je aplikace metody v době dovolených proškoleného personálu. Závěr: Výzkum naznačil, že metoda VOKS se prozatím, z pohledu individuálního hodnocení respondentů, jeví jako vhodná pro komunikaci s klienty s různorodými poruchami řeči. Metoda VOKS může přispět pracovníkům ve speciálních zařízeních ke zkvalitňování ošetřovatelské péče a k lepšímu uspokojování potřeb klienta., Aims: Czech picture exchange communication system called VOKS is a modifi ed method of communication system PECS (Picture Exchange Communication System) which was created in Chicago in the USA. The research aim was to assess user satisfaction with this method, to evaluate diffi culties with acquiring the VOKS methods and to map compliance with recommended practice. A “user” is understood as somebody who uses VOKS method to communicate with clients suff ering from ASD. The research section presents results of the survey which took place in selected fi ve specialised facilities and four schools all over the Czech Republic. Methods: The survey covered the period from September 2010 to December 2010. We designed our own questionnaire which contained 24 questions. The criterion for inclusion of respondents in the research was current professional working experience with the VOKS method. The research sample consisted of 54 respondents. Results: The results of the analysis revealed that most respondents are satisfi ed with the VOKS methods. Differences are evident in the time when they adopted VOKS methods in practice. With this particular sample of respondents the time necessary to master the VOKS method could not be established. Accurate compliance with recommended procedures proved to be problematic. The main reason for poor compliance with the procedures in practice is the necessity to adapt it for an individual client, staff shortages and insuffi cient time for training. A major problem is the application of the method when the trained staff is on holiday. Conclusion: The research indicates that for the time being the respondents consider the VOKS method to be a suitable method for communicating with clients with a variety of speech disorders. Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. Cíl: Cílem příspěvku je literární analýza dosavadních nejvíce využívaných nefarmakologických metod tlumících bolest u novorozencůpro nejlepší praxi založenou na důkazech. Metodika: Pro získání dat za období 2000–2012 byly použity elektronické licencované a volně přístupné databáze. Hodnocení nalezených důkazů bylo realizováno dle metodiky Fineout-Overholt, Johnstona (2005), kteří řadí studie dle specifi ckých kritérií do sedmi kategorií. Do výběru předkládaného příspěvku byly zařazeny důkazy úrovně I, II, III. Zvolená kritéria splňovalo 42 studií (4 meta-analýzy, 6 systematických přehledů, 1 multicentrická studie a 31 randomizovaných kontrolovaných studií). Výsledky: Nefarmakologické metody využívané k tlumení bolesti u novorozenců jsou mnohem více efektivní, pokud jsou použity v kombinaci s dalšími nefarmakologickými metodami, jako jsou muzikoterapie, zavinutí, usnadněné zasunutí, multisenzorická stimulace, klokánkování a non-nutritivní a nutritivní sání. Závěr: Nefarmakologické postupy jsou účinné a vedou k efektivnímu zmírnění bolesti především při drobných procedurálních výkonech jako je odběr krve z patičky pomocí lancety, venepunkce, apod. Léčba bolesti a řízení její prevence by měla být nezbytnou součástí standardní i intenzivní zdravotní péče o novorozence., Aim: The aim of study is a literary analysis of the current most used non-pharmacological methods of relieving pain in neonates for the best evidence-based practice. Methods: For obtain data for the period 2000–2012 were used e-licensed and freely accessible database. Evaluation of the evidence found was carried out according to the methodology Fineout-Overholt, Johnston (2005), who put the study according to specifi c criteria in seven categories. The selection of the present study were classifi ed evidence level I, II, III. Selected criteria met the 42 studies (4 meta-analyzes, 6 systematic reviews, 1 multicenter study and 31 randomized controlled study). Results: Non-pharmacological methods used to control pain in neonates are much more eff ective when used in combination with other non-pharmacological methods, such as swaddling, facilitated tucking, multisensory stimulation, kangaroo care and non-nutritive and nutritive sucking. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%)., F. The author reported clinical characteristic features and risk factors for acute intestinal infections in 225 children (140 boys and 85 girls, mean age 1.5 years old). 90 (40%) patients aged up to 12 months, 101 (44.9%) were from 1 to 2 years old, and 34 (15.1%) were more than 2 years old. Most children were hospitalized in the first week of illness and had moderate severe course of disease. Severe forms of illness reliably more often were noted in children being on artificial feeding. Toxicdystrophic condition, severe forms and perinatal encephalopathy were prevailed in boys. Factorial analysis revealed 20 most significant factors with factorization fullness 70.8%. The most significant factors were F1( factor of hyperthermia duration) with 8.5% effect, F2 (factor of pain syndrome) 5.8%, F3 (factor of normal enzymatic activity of colon bacillus) 4.9%, F4 (etiological factor) 4.6%, as well as F5( factor of opportunistic flora of the intestine) 4.4%, respectively., Mukarram Shadjalilova, and Literatura
Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Cieľ: Výskum bol zameraný na identifikáciu drţania diéty u adolescentných respondentov a niektorých príčin jej drţania vo vzťahu k pohlaviu a veku. Metodika: Na získanie empirických dát boli pouţité vybrané poloţky sebaposudzovacieho meracieho nástroja s názvom Mapovanie stravovacích návykov medzi adolescentmi (The Project EAT Survey, Neumark- Sztainer et al.). Výskumnú vzorku tvorilo 387 adolescentných respondentov, toho času študentov prvých a štvrtých ročníkov stredných škôl. Výsledky: Podľa výsledkov výskumu ţeny častejšie drţia diéty (redukčné alebo pre jej udrţanie) či uţ v minulom roku (p = 0,002) aj v terajšej dobe (p = 0,000). Sú nespokojné so svojou hmotnosťou (p = 0,000) a postavou (p = 0,009). Respondenti vo veku 18 - 20 rokov uviedli, ţe si ich členovia rodiny doberali alebo si robili posmech z ich hmotnosti (p = 0,000). Vyššie percento ţien zaţilo negatívne hodnotenie ich hmotnosti zo strany členov rodiny (27,5 %) a vrstovníkov (33,5 %). Záver: Sestra v klinickej praxi môţe byť nápomocná pri identifikovaní drţania diét u adolescentov a jej príčin z hľadiska prevencie dopadu drţania diét na telesné a duševné zdravie adolescenta a pri realizácii uţ cielených intervencií, napr. edukácie v rámci takto obsahovo zameranej prevencie., Aim: The research was aimed at identifying dieting among adolescent respondents and some of the reasons for its keeping in relation to sex and age. Methods: For obtaining empirical data selected items of self-assessing measuring tool called Mapping dietary habits among adolescents were used (The Project EAT Survey, Neumark-Sztainer et al.). The research sample consisted of 387 adolescent respondents, previously high school students (of the first and fourth study year). Results: According to results of research women more frequently keep a diet (in order to lose weight or keep it the same) in last year (p = 0,002) as well as at present-day period (p = 0,000). They are not satisfied with their weight (p = 0,000) and figure (p = 0,009). Respondents aged 18-20 years reported that their family members have teased or made fun of their weight (p = 0,000). A higher percentage of women had experience a negative evaluation of their weight on the part of the family members (27,5 %) and peers (33,5 %). Conclusion: A nurse in clinical practice can be helpful in identifying dieting among adolescents and its causes in order to prevent the impact of diets on physical and mental health of adolescent and in the implementation of already targeted interventions, e.g. education in the context of the following content targeting prevention., Michaela Miertová, and Literatura 40
Cíl: Posoudit efekt samoléčby a autodiagnózy dysmenorey u studentek univerzity v Maiduguri. Metody: 289 studentek trpících dysmenorhoeou z celkového počtu 25 000 studentek univerzity v Maiduguri ze státu Borno, Nigerie, bylo randomizováno k léčbě dysmenorey. Studentky byly vybrány na základě dotazníku, který vyplnily v lednu 2011. Výsledky: Střední věk ± směrodatná odchylka (SD) účastnic byl 22.50 ± 3.12 roku. 112 (38.8 %) užívalo analgetika a nefarmakologické prostředky k tišení bolesti, buď současně, nebo následně (p < 0.001). Nejčastějšími nemedikamentózními prostředky byly horká lázeň (15.2 %) a ohřívací dečka umístěná na podbřišek (5.5 %). Nejčastějšími léky byly nesteroidní antirevmatika (NSAID) užívané při nástupu menstruace. V 72 % byla léčba účinná, ve 4.2 % nebyla. Většina účastnic se medikovala sama a nezabývala se možnými vedlejšími účinky (p < 0.001). Většina účastnic (63 %) utratila 300 Nigerijských Naira a méně (1.9 US Dolarů a méně) za léčbu dysmenorey. Závěry: Výsledky potvrdily, že NSAID byly nejčastěji užívané medikamenty k léčbě dysmenorey, ale většinou bez znalosti potencionálních vedlejších účinků. Proto doporučujeme zapojení profesionálních zdravotníků do tohoto procesu., Objective: This study was performed to evaluate self-medication for the treatment of self-diagnosed dysmenorrhoea among a group of University of Maiduguri female students. Methods: The population of this randomized study was 289 female students suffering from dysmenorrhoea out of over 25 000 students attending a college of medicine and eight non-medical faculties of university of Maiduguri in Borno state, Nigeria. The students were interviewed using a self-administered pre-tested structured questionnaire in January, 2011. Results: The Mean ± Standard Deviation (SD) of age of the participants was 22.50 ± 3.12 years. Significant number 112 (38.8%) of participants use drugs to remedy menstrual pains followed by non-drug therapy while few combine both the drug and non-drug therapy for the pain management (p < 0.001). The most common non drug remedies employed by the participants were taking a hot bath (15.2%) and placing a heating pad on abdomen (5.5%). However, the most common drugs used by the participants were Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) which were usually taken orally at the onset of menstruation. Among the participants that used drug, 72% found it effective while 4.2% reported ineffectiveness. Significant majority of the participants usually self-prescribed their drugs, and were ignorant of the common side effects of these drugs (p < 0.001). Most participants (63%) spent 300 Nigerian Naira and below (1.9048 US Dollar and below) on drugs for dysmenorrhoea every month. Conclusion: The results confirmed that NSAIDs were the most common drugs used for dysmenorrhoea treatment, but frequently used without knowledge of potential side effects due to self-medication. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
Cieľ: Cieľom štúdie bolo identifikovať úroveň emočnej inteligencie (EI) u sestier a zistiť rozdiely v úrovni EI vzhľadom na vek, dĺžku praxe a vzdelanie sestier. Metodika: Výskumný súbor tvorili sestry (n = 280) poskytujúce starostlivosť pacientom štyroch nemocníc Trnavského kraja. Priemerný vek sestier bol 39,64 (SD = 9,44), dĺžka ich profesijnej praxe bola 18,7 (SD = 10,64). Na meranie úrovne EI boli použité dotazníky, ktoré reprezentujú súčasné prístupy chápania EI. Dotazník SITEMO (Situational Test of Emotional Understanding), zisťuje úroveň EI ako schopnosti, výsledkom je celkové skóre EI, ktoré je dané súčtom správnych odpovedí. Dotazník SEIS (Schutte EI Scale) zisťuje úroveň EI sestier ako črty. Sestry vyjadrovali mieru súhlasu resp. nesúhlasu prostredníctvom 5bodovej Likertovej škály. Pri spracovaní dát boli využité metódy deskriptívnej štatistiky a induktívnej štatistiky. Údaje boli spracované v štatistickom softvéri SPSS 15.0. Výsledky: Zistili sme štatisticky významný rozdiel (p < 0,05) v úrovni EI sestier vzhľadom k ich veku. Pri ostatných sociodemografických znakoch (dĺžka profesijnej praxe, vzdelanie) nebol zaznamenaný štatisticky významný rozdiel (p > 0,05). Sestry s Mgr. stupňom vzdelania dosiahli najlepší výsledok v priemernom poradí mier EI v oboch metodikách. Záver: Zistené výsledky umožňujú upriamiť pozornosť na rozvoj EI u sestier prostredníctvom možností vzdelávacích inštitúcií. Môžeme podporovať zmeny, ktoré budú prispievať ku zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti. Využívanie EI je nutnou a zároveň modernou požiadavkou ošetrovateľstva., Aim: The aim of this study was to identify the level of emotional intelligence (EI) of nurses and to identify differences in the level of EI in relation to their age, length of work experience and education. Methods: The sample consisted of nurses (n = 280) providing care to patients in four hospitals in the Trnava Region. The mean age of the nurses was 39.64 years (SD = 9.44) and the length of their work experience was 18.7 years (SD = 10.64). The level of EI was measured using questionnaires representing current approaches to understanding of EI. Emotional intelligence as an ability was determined by the Situational Test of Emotional Understanding (SIT-EMO) questionnaire, yielding the overall emotional intelligence score given by the sum of the correct answers. EI as a feature, on the other hand, was assessed by the Schutte EI Scale (SEIS) in which the nurses expressed the extent to which they agree or disagree with each statement on a five-point Likert scale. To analyze the data, methods of descriptive statistics and inductive statistics were used. The data were processed using the SPSS 15.0 statistical software. Results: A statistically significant difference (p < 0.05) was found between the level of nurses’ EI level and their age. In terms of other social demographic data (length of work experience, education), a statistically significant difference (p > 0.05) was not found, although the nurses with a master’s degree ranked highest as for their mean level of EI, using both methods. Conclusion: The results enable to focus attention on the development of nurses’ EI through educational institutions. Changes may be encouraged that will contribute to an increase in the quality of nursing care provided. ABSTRAKT Cieľ: Zámerom štúdie bolo identifikovať štatisticky významné rozdiely v úrovni emočnej inteligencii (EI) u študentov ošetrovateľstva vzhľadom k demografi ckým údajom (vek, dĺžka štúdia) a zároveň zistiť či úroveň EI študentov ošetrovateľstva ovplyvňuje ich záujem o problematiku geriatrického ošetrovateľstva. Metódy: Výskumu sa zúčastnilo 86 univerzitných študentov denného bakalárskeho štúdia odboru ošetrovateľstvo. Na meranie úrovne EI boli využité dotazníky reprezentujúce rôzne chápania EI. Prostredníctvom SIT-EMO (Situational Test of Emotional Understanding) bola zisťovaná EI ako schopnosť. EI ako črta bola zisťovaná dotazníkom SEIS(Schutte Emotional Intelligence Scale). Dotazníkom vlastnej konštrukcie ESE-GP (Emotional self-effi cacygeriatric patient) bola zisťovaná úroveň EI študentov v procese starostlivosti o geriatrických pacientov. Výsledky: Nezistili sme štatisticky významné rozdiely v úrovni EI študentov z hľadiska demografi ckých údajov (vek a ročník štúdia). Medzi úrovňou EI (SEIS)a perspektívnym záujmom študentov venovať sa geriatrickému ošetrovateľstvu bola zistená štatisticky významná súvislosť (p < 0,05). Závery: Zistené výsledky môžu uplatniť najmä inštitúcie poskytujúce ošetrovateľské vzdelávanie, ktorým odporúčame zamerať pozornosť na rozvoj EI v procese vzdelávania študentov ošetrovateľstva., Aim: The aim of the study was to identify statistically significant differences in levels of emotional intelligence (EI) of nursing students in relation with demographic data (age and year of study) and also determine whether the EI level of nursing students affects their interest in the issue of geriatric nursing. Methods: The sample consisted of 86 full-time undergraduate university students of nursing. To measure the level of EI, questionnaires representing diff erent understanding of EI were used. EI as an ability was determined by the SIT-EMO (Situational Test of Emotional Understanding). EI as a trait was evaluated by the questionnaire SEIS (Schutte Emotional Intelligence Scale). The questionnaire of own construction ESE-GP (Emotional self-effi cacygeriatric patient) assessed the level of EI of students in the process of caring for geriatric patients. Results: There were no statistically signifi cant diff erences in the students’ level of EI in terms of demographic data (age and year of study). A statistically signifi cant association (p < 0.05) was found between the students’ level of EI (SEIS) and their promising interest to pursue geriatric nursing. Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. Conclusion: Majority of ethical problems can be prevented by mastering effective communication and sufficient awareness, and this can be achieved before the hospitalization of child in the ambulance of GP for children and adolescents, or in specialist clinic., Žaneta Beránková, and Literatura
Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS.
