AIMS: Our aims were to isolate and cultivate mesenchymal dental pulp stem cells (DPSC) in various media enriched with human blood components, and subsequently to investigate their basic biological properties. METHODS: DPSC were cultivated in five different media based on α MEM containing different concentrations of human plasma (HP), platelet-rich plasma (PRP), or fetal calf serum (FCS). The DPSC biological properties were examined periodically. RESULTS: We cultivated DPSC in the various cultivation media over 15 population doublings except for the medium supplemented with 10% HP. Our results showed that DPSC cultivated in medium supplemented with 10% PRP showed the shortest average population doubling time (DT) (28.6 ± 4.6 hours), in contrast to DPSC cultivated in 10% HP which indicated the longest DT (156.2 ± 17.8 hours); hence this part of the experiment had been cancelled in the 6th passage. DPSC cultivated in media with 2% FCS+ITS (DT 47.3 ± 10.4 hours), 2% PRP (DT 40.1 ± 5.7 hours) and 2% HP (DT 49.0 ± 15.2 hours) showed almost the same proliferative activity. DPSC's viability in the 9th passage was over 90% except for the DPSC cultivated in the 10% HP media. INTRODUCTION: Leptin is an adipokine which has a direct relationship to obesity. Our aim was to measure this hormone in pregnant women at three months intervals throughout their pregnancies to determine the serum value of those who developed preeclampsia. MATERIAL AND METHODS: We followed 19 women (median age 24.8 +/- 5.7 years) with pre-gestational Body Mass Index (BMI) less than 25 kg/m2, 21 (median age 26.1 +/- 4.6 years) with BMI higher than 25 kg/m2 and 16 (median age 30.9 +/- 5.8 years) with Gestational Diabetes Mellitus (GDM) (median age 30.9 +/- 5.8 years), recruited in the 1st trimester of pregnancy. Serum levels of leptin were measured with radioimmunoassay (RIA) technique. RESULTS: In the first trimester of pregnancy leptin levels showed statistically significant differences between normal weight and overweight-obese women (p < 0.001), diabetic women (p < 0.05) and the subgroup of preeclamptic women (p < 0.001). For those women with PGBMI > or = 40 kg/m2 and leptin > or = 40 ng/ml in the second trimester, the Odds Ratio (OR) to develop preeclampsia was of 47.95% CI (4.1-527.2). Analyzing leptin values with ROC curves, the greatest area under the curve (AUC) was for leptin in the second trimester (0.773, CI: 0.634-0.911). CONCLUSION: Women with morbid obesity (BMI > or = 40 kg/m2) had significantly higher levels of serum leptin (p < 0.01) and a value of 40 ng/ml of this hormone seems to be predictive of developing preeclampsia in this group of patients. and H. Ukazuje na nebezpečí, kterými hypoglykemie pacienty ohrožují, ale všímá si také rizikových skupin pacientů z hlediska rizika hypoglykemie a také různé míry rizika, která s sebou nesou různá antidiabetika. Crohnova choroba (CD) a ulcerózní kolitida (UC) jsou chronické střevní záněty (IBD) neznámého původu, v jejichž výskytu pozorujeme zřetelný západo-východní a severo-jižní gradient. Předpokládaná patogeneze zahrnuje působení dosud neidentifikovaných vnějších vlivů u geneticky predisponovaných jedinců za účasti některých složek střevní mikroflóry. V klinickém obraze obou nemocí se střídají období aktivity s fázemi klidovými; dominujícími symptomy jsou bolesti břicha a průjmy, typickým projevem UC jsou krvavé stolice. Nemocní s CD jsou často postiženi hnisavými komplikacemi v podobě abscesů a píštělí nitrobřišních nebo perianálních. Základem léčby je dlouhodobá medikamentózní terapie aminosalicyláty, kortikosteroidy, imunosupresivy nebo biologickou léčbou. Navzdory léčbě musí být značná část pacientů během života pro střevní zánět operována. Nejčastější komplikací obou nemocí je sideropenie a sideropenická anemie, nemocní jsou ohroženi také nežádoucími účinky podávaných medikamentů., Crohn’s disease (CD) and ulcerative colitis (UC) are chronic inflammatory bowel diseases (IBD) of unknown origin with a distinct west-east and north-south gradient in their geographic distribution. Unidentified external factors in combination with genetic predisposition and intestinal microbiome are supposed to cause both diseases. Periods of clinical activity and quiescent phases are typical of the disease course. Patients suffer from abdominal pain and diarrhoea, bloody stools usually occur in UC, while CD patients frequently develop purulent complications – intra-abdominal or perianal abscesses and fistulas. Patients have to use long-term medical treatment with aminosalicylates, corticosteroids, immunosuppressive drugs or biologic therapy. Despite medical therapy, many patients undergo surgery for their IBD. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Předpoklad: Studie IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE‑IT) hodnotila potenciální prospěch na snížení velkých kardiovaskulárních (KV) příhod při přidání ezetimibu vs placebo k léčbě 40 mg simvastatinu u nemocných s akutním koronárním syndromem a nízkou hladinou low-density cholesterolu (LDL‑c) ≤ 125 mg/dl. Metodika: Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (IM), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Ve větvi se simvastatinem v monoterapii byl cílový LDL‑c < 70 mg/dl. Předpokladem bylo, že ezetimib sníží LDL‑c o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo nonST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října 2005 do července 2010. Nejvíce (40 %) bylo zařazeno v západní Evropě a Severní Americe (38 %). Pacienti se STEMI byli mladší a bylo zde více nemocných bez předchozí léčby statiny. Pacienti s nonSTEMI/NAP měli častěji diabetes mellitus, hypertenzi a předchozí IM. Medián LDL‑c při randomizaci byl 100 mg/dl u STEMI a 93 mg/dl u nonSTEMI/NAP. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %) (p = 0,016) léčených kombinací. Na záchranu jednoho primárního cíle je třeba léčit 50 nemocných po dobu sedmi let. Methods: The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, re‑hospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postran‑domization). The simvastatin monotherapy arm's LDL‑C target was < 70 mg/dL. Ezetimibe was assumed to further lower LDL‑C by 15 mg/dL and produce an estimated ~8% to 9% treatment effect. The targeted number of events was 5,250. Results: 18,144 patients were enrolled with either ST‑segment elevation MI (STEMI, n = 5,192) or UA/non‑ST‑segment elevation MI (UA/NSTEMI, n = 12,952) from October 2005 to July 2010. The STEMI cohort was younger and had a higher percentage of patients naive to lipid‑lowering treatment compared to the UA/NSTEMI cohort. The UA/NSTEMI group had a higher prevalence of diabetes, hypertension, and prior MI. Median LDL‑C at entry was 100 mg/dL for STEMI and 93 mg/dL for UA/NSTEMI patients. Primary endpoint occurred in 2,742 patients (34.7%) treated with simvastatin in monotherapy and in 2,572 patients (32.7%) (p = 0,016) treated with combination treatment. To avoid one primary outcome, treatment of 50 patients for seven years was necessary. Průměrně 2 pacienti ze 100 předešli kardiovaskulární příhodě za 7 let (Number Needed to Treat – NNT) 50/7 let. Heart attacks alone were reduced by 13 %, and non-fatal stroke by 20 %. Deaths from cardiovascular disease were statistically the same in both groups. Patients were followed an average of approximately six years, and some as long as 8.5 years. Approximately 2 patients out of every 100 patients treated for 7 years avoided a heart attack or stroke [Number Needed to Treat (NNT) = 50/7years]. Erytrocytární aloimunizace (izoimunizace, hemolytické onemocnění plodu a novorozence – HON) se v současnosti díky rutinní imunoprofylaxi anti-D imunoglobulinem vyskytuje sporadicky. Výskyt atypických aloimunizací non-RhD-antigeny zůstává neměnný, poněvadž není známa profylaxe a ani prevence nezajišťuje stoprocentní účinnost. Pokud se imunoprofylaxe anti-D imunoglobulinem neprovede nebo selže, hrozí vznik hemolytického onemocnění plodu a novorozence. Každý případ je potom unikátní (specifita a typ protilátky, blokující protilátky v séru matky, antigenní výbava erytrocytů plodu, gestační stáří při začátku onemocnění atd). Pokroky v genové diagnostice, imunohematologii a dopplerovské ultrasonografii významně snižují frekvenci nutných invazivních zákroků v diagnostice a léčbě senzibilizovaného těhotenství. Intraumbilikální přísně intravenózní transfuze pod ultrasonografickou kontrolou je první metodou volby léčby případů s těžkou anémií plodu i při rozvinutém hydropsu. Konzultace a léčba se soustřeďují do center s perfektní porodnickou a neonatologickou péčí a nepřetržitou operační pohotovostí. Klíčová slova: anti-D-imunoglobulin – erytrocytární aloimunizace – hemolytické onemocnění plodu a novorozence – imunoprofylaxe anti-D-imunoglobulinem – intraumbilikální intravenózní transfuze, The erythrocyte alloimmunization (isoimmunization, hemolytic disease of the fetus and newborn – HON) occurs sporadically at present, thanks to routine immunoprophylaxis with anti-D immunoglobulin. The occurrence of atypical alloimmunizations related to non-RhD-antigens remains unchanged, as there is no prophylaxis known and even the prevention does not provide one hundred percent efficiency. When anti-D immunoglobulin immunoprophylaxis is not undertaken or fails, the threat of hemolytic disease of the fetus and newborn arises. Every case is then unique (specificity and type of antibody, blocking antibodies in maternal serum, antigenic make-up of fetal erythrocytes, gestational age at the disease onset etc). The advancements in genetic diagnostics, immunohematology and Doppler ultrasonography significantly reduce the frequency of necessary invasive interventions in the diagnostics and treatment of sensibilized pregnancy. Intraumbilical strictly intravenous transfusion under ultrasound guidance is the first-line method of therapy for cases of severe fetal anemia as well as advanced hydrops. Consultations and treatment are provided in the centres for comprehensive obstetrical and neonatal care with continuous intervention preparedness.
