In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. The significance of ABO blood system groups antigens in development of some malignant tumors is already established. The alteration of hormonal homeostasis must also be taken into account. Hence the aim of the investigation was to study ABO and Rh blood system antigens and hormonal status among reproductive age women with benign and malignant breast tumors. Methods: The determination of hormones was made by the enzymatic analysis method (ELAIZA), provided by proper ELAIZA kits. For the study of ABO and RhHr system antigens, internationally recognized immunoserology methods were used. Results: High index of the breast gland tumors were revealed in patients with A(II) phenotypic group, according to the ABO system. The frequency distribution of O(I) phenotypic group was low among women with breast tumors. Among D, C, E, c and e antigens of the Rh system, the frequency of D and E antigens were increased in benign and malignant breast tumors patients. The study of hormonal balance revealed thyroid gland hypofunction and increased level of estradiol on the background of increased testosterone and decreased progesterone levels. Such hormonal imbalance and excess production of estradiol creates conditions for malignant tumor formation in reproductive age women. Conclusion: The highest frequency of breast cancer in reproductive age was revealed in A(II) group patients. Backround: Worldwide, approximately 1 billion people, from which 200250 million are women, smoke. Based on the Estonian Medical Birth Register data, in 2010, 7,5% of pregnant women were smokers, and in 2012, 7% of pregnant women were smoking during their pregnancy. Thus, smoking rates among pregnant women fell only 0,5%. The objective of the research was to find out the views of pregnant women who were smoking during pregnancy of the effect of smoking on the health of them and their babies, and experiences in quitting smoking, changing indicators, e.g. smoking and socioeconomical status (initial exposure to smoking, employment, longterm partnerships etc.) in 20092013. Methods: This research is a phenomenological study, which was being carried out from October 2009 until January 2013. Data collection methods were semistructured interviews with 45 pregnant women smoking during their pregnancy and being registered for antenatal care in three health care institutions of Republic of Estonia. An inductive approach for qualitative analysis was used. Results: Most women smoking during pregnancy started smoking in their teens, their parents smoked, they were under 30 years old housewives or unemployed. On an average of 1120 cigarettes were consumed each day, the pregnancy of the research group did not influence frequency and tobacco intake. The participants in the research group reported that tobacco consumption helped them relax, gave them a chance to spend time in a good company, and it was not regarded as a bad habit but a social addiction, being caused by availability of tobacco products. Although most of them had an opinion about harmful effects of smoking, e.g. a cough, asthma and rapid fatigue, on the health of them and less on their expected babies, they continued smoking. Conclusions: The smoking and socioeconomical status of women smoking during pregnancy in 20092013 has not changed, health awareness of them is low and has not improved. Support groups with close relatives are needed to motivate cessation. In the first place, prevention needs a good example, purposeful cooperation, which starts in daily life context at homes, in educational institutions and in the community. Limiting the availability of tobacco products, postponing and preventing an initial exposure to smoking are thought to be of great importance. Infantilní hemangiomy jsou nejčastějšími benigními vaskulárními nádory novorozeneckého a kojeneckého věku. Jedná se o nádory s velmi dobrou prognózou. Většina infantilních hemangiomů spontánně zcela regreduje v prvních deseti letech života. Jen deset procent infantilních hemangiomů vyžaduje nějakou terapii. V současné době je preferováno použití b-blokátoru propranololu. Ostatní typy benigních cévních nádorů je zapotřebí diferenciálně diagnosticky odlišit pro zahájení vhodné terapie a určení prognózy. Předložený článek shrnuje základní poznatky o hemangiomech u novorozenců. V závěru článku je zmíněna krátká kazuistika, kdy byla léčba rozsáhlého hemangiomu zahajována po konzultacích na našem pracovišti za kontroly kardiology., Infantile hemangiomas are the most common benign vascular tumors of neonatal and infancy. These are tumors with a good prognosis. Most infantile hemangiomas spontaneously regress completely in the first decade of life. Only ten percent of infantile hemangiomas require some therapy. Currently it is preferred to use b-blocker propranolol. Other types of benign vascular tumors are needed to distinguish the differential diagnosis for initiation of appropriate therapy and prognosis. Inversion of uterus is very rare and serious complication of childbirth. It is a situation with very high maternal morbidity and mortality, whitch can lead to developement of very serious periparthal bleeding, DIC, neurogenic and haematologic shock. Onemocnění lidským papilomavirem (HPV) patří mezi jednu z nejběžnějších sexuálně přenosných infekcí a HPV působí spektrum nemocí, od genitálních bradavic ke karcinomům. Důležitým cílem očkovacích programů v mnoha zemích je snížení incidence karcinomu děložního hrdla použitím kvadrivalentní (Silgard/Gardasil) nebo bivalentní (Cervarix) HPV (human papillomavirus) vakcíny. Randomizované kontrolované studie prokazují robustní protilátkovou odpověď a vysokou účinnost HPV vakcinace také u mužů. V roce 2009 Úřad pro kontrolu potravin a léčiv (FDA) v USA schválil kvadrivalentní HPV vakcínu u mužů k prevenci anální prekancerózy a karcinomu. The important goal of immunization programs in many countries is the reduction of the incidence of cervical cancer using either the quadrivalent (Silgard/Gardasil) or the bivalent (Cervarix) HPV (human papillomavirus) vaccine. Randomized controlled trials demonstrate robust antibody responses and high efficacy also in men. In 2009 the Food and Drug Administration in the USA approved the quadrivalent HPV vaccine for young men to prevent anal precancerosis and cancer. Detailed cost effective modeling has preceded this decision showing that when the burden of disease in men is included in the models then, depending upon vaccine price, coverage of a vaccinated population, and other factors male vaccination can become cost effective. The purpose of this study was to investigate the content neurospecific markers protein S-100 and neuroenolaza in blood serum and tear fluid of patients with ocular ischemic syndrome. Material and methods. We observed 43 patients aged 57 to 79 years, mean age 67.3 ± 2.7 years. Control group consisted of 11 volunteers without ophthalmic symptoms. The main group consisted of 32 patients with OIS. The neurospecific proteins S100 and NSE were investigated in blood serum and tear fluid. Results. The study found that in patients of the control group the content of protein were within the normal range: S -100 in the tear fluid – 0,0662 ± 0,00335 mkg/l, in the blood serum 0,0508 ± 0,00241 mkg/l. In patients of the main group the indicators of protein in the tear fluid were elevated in all patients - 3,12 ± 0,246 mkg/l ( p<0.005). The normal evels in blood serum of marker S-100 was in 30 patients - 0,0589 ± 0,00303 mkg/l, while, in 2 patients protein S-100 were raised and averaged 0,2175±0,00725 mkg/l. It was found that in patients of the control group content of protein NSE in the tear fluid and blood serum were within normal values - 15,86 ± 0,148 Ng/ml, 15,60 ± 0,202 Ng/ml respectively. In the main group the amount of protein NSE tended to increase in the tear fluid in 23 patients and averaged 33,012 ± 3,2626 Ng/ml (p<0.005), a significant decrease the quantity of protein was observed in 9 patients, which amounted to 5,166 ± 0,8301 Ng/ml. At normal levels in the blood serum protein NSE detected in 30 patients and averaged 14,48 ± 0,263 Ng/ml, whereas, in 2 patients there was a significant increase of content of protein NSE and was 27,47 ± 3,068 Ng/ml. Conclusions. Autoři předkládají kazuistiku „antibiomanie“ u 55leté pacientky s emočně nestabilní poruchou osobnosti a mentální anorexií v anamnéze, nyní dlouhodobě ambulantně léčenou pro úzkostnou a depresivní symptomatologii, související se závažnou somatickou nemocí manžela. Přesmyk do mánie nastal po druhé 500mg dávce klarithromycinu. Nicméně ten byl již třetím podaným antibiotikem v intervalu posledních čtyř měsíců. Manické symptomy dosáhly takové intenzity, že byla nutná nedobrovolná hospitalizace v psychiatrické nemocnici. Zde příznaky odezněly do týdne. Autoři nabízejí hypotézu patogeneze manického zvratu. Předpokládají, že přesmyk mohl nastat primárně v důsledku alterace střevního mikrobiomu antibiotiky. Další patogenetická cesta může vést buď přes přímé vagové dráždění, nebo přes zvýšenou propustnost střevní bariéry pro falešné neurotransmitery., The authors present a case of antibiomania in 55years woman with borderline personality, anorexia nervosa in personal history, and long lasting anxious and depressive symptomatology associated with husband´s serious illness. Switch to mania occured after two 500mg doses of clarithromycin. However, clarithromycin was the third antibiotic administered in the last four months. Manic symptoms were so severe that involuntary hospitalization was required. Signs of mania disappeared within a week on psychiatric ward. V předložené kazuistice je zachycena práce rodinného terapeuta s rodinou, kde matka trpí bolestmi zad a dítě (3,5) je chronicky nemocné. Trpí kašlem nejasného původu. Autorka podrobně popisuje průběh šesti sezení, během nichž se v rodině postupně měnila atmosféra. Konflikty, které pár značně zatěžovaly, měly své kořeny v minulosti obou rodin a v mylných nevyslovovaných předpokladech na obou stranách. Narativní terapie s respektem ke struktuře a genderové vyváženosti, kterou se autorka učila v rámci čtyřletého výcviku v institutu rodinné terapie psychosomatických poruch v Liberci, byla zde hlavním nástrojem léčby. Léčba se nemůže zaměřit přímo na symptom dítěte. Jde o symptom celého rodinného systému. Zjednodušeně řečeno, bylo třeba, aby se všichni mohli zase volně nadýchnout., In the presented casuistry there is given a description of a family therapist’s work with a family, where the mother suffers from backache and the child (3.5) is chronically ill suffering from cough of unknown origin. The author describes in detail the course of six sessions, during which the family atmosphere was gradually changing. The conflicts that significantly burdened the couple, had their roots in the past of both families and in wrong untold assumptions on both sides. Narrative therapy with respect to the structure and gender balance, which the author learned in the four-year training in the family therapy institute of psychosomatic disorders in Liberec, was the main tool of treatment. Nedostatečná informovanost o využití praxe založené na důkazech může vést k nekvalitní a neplnohodnotné péči o klientku. Cíl: Hlavním cílem bylo zjistit informovanost porodních asistentek o Evidence Based Midwifery. Mezi dílčí cíle patřilo zjištění, zda porodní asistentky znají pojem Evidence Based Midwifery a zkratku EBM a zda s Evidence Based Midwifery ve své praxi pracují. Metody: Byl vytvořen nestandardizovaný dotazník. Celkem obsahoval 15 otázek. V úvodu dotazníku byly respondentky seznámeny se záměrem výzkumu a byl jim předložen informovaný souhlas. Respondentkami byly porodní asistentky z České republiky pracující na gynkologických, porodnických lůžkových a ambulantních pracovištích. Rozdáno bylo celkem 160 dotazníků. Návratnost činila 93,8 %. Celkem se pracovalo se 150 dotazníky. Výzkumné šetření probíhalo v měsících září–listopad roku 2013. Výsledky: Zkratku EBM uvedlo, že nezná celkem 100 (66,7 %) respondentek. Znalost pojmu Praxe založená na důkazech potvrdilo celkem 46 (30,7 %) respondentek. Zájem o studijní materiál věnující se praxi založené na důkazech v porodní asistenci má 93 (62,0 %) respondentek, zájem o školící kurz o EBMid uvedlo 110 (73,4 %) respondentek. Závěry: Cíle výzkumného šetření byly splněny. Úroveň informovanosti porodních asistentek o Evidence Based Midwifery jsme stanovili jako nedostatečná (méně než polovina respondentek nezná pojem EBMid). Většina respondentek s Evidence Based Midwifery ve své praxi nepracují. Bylo by vhodné informovat porodní asistentky o využití EBMid v praxi a poté provést další výzkum, díky kterému bychom mohli zhodnotit zlepšení dané situace či nikoliv., Background: Evidence-based practice is very useful in every profession, in midwifery exception. Methods: Respondents were midwives working in the gynecological-obstetric wards and clinics in the Czech Republic selected by purposive sampling, there were 150 respondents. e research was carried out by a non-standardized questionnaire. Results: 100 respondents don´t know EBM: 93 respondents are interested in the study material about EBMid and 110 respondents are interested in a training course. Článek je věnován právně teoretickým (i právně realizačním) aspektům aktuální právní úpravy v oblasti takzvaných „specifických zdravotních služeb“ (viz pojmosloví zákona č. 373/2011 Sb.). Text článku je konkrétně zaměřen na dispozici paragrafu šest jmenovaného zákona, která kogentně stanovením věkového limitu omezuje možnost skupiny žen podstoupit konkrétní zdravotní výkony. Přehledový článek pojednávající na téma možných komplikací páskových operací běžně užívaných v léčbě stresové inkontinence moči. První metodou byla retropubická tahuprostá páska TVT, která s sebou nese zejména riziko poranění močového měchýře. V literatuře byly dále popsány komplikace typu závažného krvácení, poranění orgánů v malé pánvi, jako je střevo, nebo velkých cév. Další generace tahuprostých pásek se vyhýbá retropubickému prostoru vedením pásky skrz foramen obturatum. Tato generace má popsané riziko vzniku bolesti v třísle/tříslech. Avšak tato bolest většinou spontánně ustupuje, případně rychle odeznívá po podání nesteroidních antiflogistik. Poslední generace tahuprostých pásek jsou „minipásky“. Zavádí se z jednoho drobného řezu a jejich délka je menší než u předešlých generací. Rizikovost „minipásek“ spočívá v nedostatečné fixaci a tedy riziku selhání. Přestože se páskové metody používají celosvětově v klinické praxi již přes patnáct let, doposud nebyla vyvinuta metoda, která by byla jak naprosto bezpečná, tak úspěšná a jednoduchá k zvládnutí operatérem., This review focuses on possible complications during tape procedures carried out due to stress urinary incontinence. The first method was retropubic tension-free vaginal tape TVT that held mainly risk of bladder perforation. Further complications previously described in literature were severe bleeding and injuries of organs in lesser pelvis like bowel or great vessels. The next generation eliminated retropubic space by insertion through the obturator foramen. However another complication had been witnessed in presence of groin pain. Fortunately, this is usually transient and quickly passing either spontaneously or after use of non-steroid anti-inflammatory drugs. The last generation of tension-free vaginal tapes are “minislings” that are inserted from a single incision and are shorter in comparison with the previous generations. The risk of use of this “minislings” is in insufficient fixation and therefore failure of the procedure. Objective: To determine risk factors in different pathogenic subtypes of ischemic stroke in Tashkent hospital registry and to compare with the data of other stroke registers. Design: Prospective observational study. Participants: 300 adults aged 18 years or over who suffered first-ever acute ischemic stroke, of them, the data of 100 patients with atherothrombotic, lacunar and cardioembolic strokes were analyzed. Main outcome measures: Mathematically significant differences and not significant differences but interesting from other point of view. Categorical variables are expressed as frequencies and percentages. Differences were regarded as statistically significant when the P value was less than 0.05 (P<0.05). Results: Of 100 patients with acute first-ever ischemic stroke at the median age of 62.63±4.68 (interquartile range 25-92) years, 57% were men and 43% were women. Depending on aetiology and stroke subtype, all the patients were divided into three groups: 42 patients with atherothrombotic stroke, 41 – with lacunar stroke, and 17 patients with cardioembolic stroke. Atherothrombotic stroke developed often in people over 60 years old, while lacunar stroke prevailed at the younger age. Stroke cases increased in people over 51 years old. Although men were more likely to suffer strokes than women, stroke incidence was equal in both sexes with increasing of age (over 60 years old), especially in atherothrombotic stroke, testifying to that women live longer. Combination of cerebral atherosclerosis and hypertension was the most significant risk factor for stroke. Atherosclerosis was the main aetiological factor for ischemic stroke, independently of its pathogenic heterogeneity. Diabetes mellitus was an independent risk factor for all stroke subtypes, which in most cases worsened the European Medical, Health and Pharmaceutical Journal ISSN 1804-5804 course and outcome of stroke. Conclusions: This study found simultaneous influence of many risk factors (multifactorial disease) in the development of ischemic stroke. The more course and influence of the underlying disease and risk factor, the higher stroke risk and severity, especially in the absence or inadequate previous therapy of underlying disease before the stroke index. We have noted a high prevalence of under-diagnosis of vascular risk factors at levels of primary and secondary care. We recommend for improved public awareness of vascular risk factors and better diagnostic and treatment facilities aimed at addressing those factors at levels of primary and secondary healthcare. Úvod: Vrozené srdeční vady jsou nejčastější vývojovou vadou plodu. Detekce vrozených srdečních vad u plodu je založena na propojení screeningového a specializovaného echokardiografického vyšetření. Metodika: Pro posouzení úspěšnosti prenatálního záchytu srdečních vad používáme srovnání s prospektivní studií BOSS o postnatální prevalenci vad v ČR. Prevalence všech srdečních vad byla 6,16 na 1 000 živě narozených dětí a kritických 2,36/1 000 živě narozených dětí. Výsledky: V České republice se v období 1986-2012 intrauterinně odhalilo 2 996 plodů se srdeční vadou. V posledních 5 letech se záchytnost vad srdce pohybuje v rozmezí 39-47 % a detekce kritických vad přesáhla 80 % z očekávaného počtu VSV. Z celkového počtu fetálně detekovaných VSV se 1 612 (54 %) těhotných rozhodlo pro ukončení gravidity, z nichž 49 % mělo další extrakardiální vadu. Narodilo se 1 296 dětí s prenatální diagnózou VSV (43 %) a 90 plodů (3 %) zemřelo intrauterinně. Vzhledem k vysoké četnosti ukončení gravidity se mění postnatální spektrum srdečních vad. Snižuje se počet dětí s diagnózou syndromu hypoplázie levého srdce, atrioventrikulárním defektem, atrézií plicnice, společným arteriálním trunkem, společnou komorou a Ebsteinovou anomálií. Závěr: Prenatální echokardiografie prováděná v celé České republice umožňuje v posledních letech detekci až 47 % všech a více než 80 % kritických srdečních vad. Vysoký počet ukončených těhotenství je nejspíše důsledkem závažnosti srdečních vad a četných přidružených vývojových abnormalit. Četnost ukončení gravidity má vliv na postnatální spektrum srdečních vad, kdy ubývá závažných nebo komplexních srdečních onemocnění (syndromu hypoplázie levého srdce a dalších)., Introduction: Congenital heart defects (CHD) represents the most frequent congenital malformation. Prenatal detection of heart lesions is based on cooperation of screening and specialized echocardiographic examination. Methods: To assess the success of prenatal detection of heart defects (CHD) we compared the number of prenatal diagnosis with a known prevalence of CHD at birth as determined by prospective Bohemian „BOSS“ study. The study established prevalence of all CHD at birth 6.16 per 1 000 liveborn newborns and 2.36 of those with critical forms. Results: Between 1986 and 2012 were evaluated 2 996 of foetuses with congenital heart defects. A detection rate of CHD reached 47% during recent five years and detection of critical forms exceeded 80%. 1 612 (54%) mothers of foetuses with CHD opted for termination of pregnancy, 49% of them had an extracardiac heart malformations. 1 296 (43%) newborns were born with prenatally detected heart lesion and 90 (3%) foetuses died in utero. Due to high termination rate of foetuses with CHD the number of children with several complexed heart lesions declined (hypoplastic left heart syndrome, pulmonary atresia, single ventricle, persistent arterial trunk, atrioventricular defect and Ebstein anomaly). Conclusion: The nationwide prenatal ultrasound screening programme enabled detection of 43% of all CHD in recent years. Owing to severity of lesions and associated extracardiac malformations and cultural background, termination rate of foetuses with CHD remains high. Našim cieľom je poukázať na dôležitosť podpory popôrodného bondingu, čiže bezprostredného kontaktu matky s dieťaťom po pôrode, odhaliť túžby, pocity žien a zistiť ich prežívanie vo vzťahu k bondingu. Štúdie sa zúčastnilo 2001 žien (vek 29,96 rokov ± 5,34 rokov), ktoré rodili spontánne v období za posledné tri roky. Metódou zberu údajov bol dotazník vlastnej konštrukcie. Pre vyhodnotenie sme použili deskriptívnu štatistiku. Zistili sme, že väčšina žien (93,90 %) túžila po bondingu a mala silu (82,86 %) byť bezprostredne po pôrode v kontakte so svojím dieťaťom. Po separácií dieťaťa matky prežívali strádanie (36,53 %), strach o dieťa (22,04 %), smútok (11,29 %). Zo strany pôrodných asistentiek/sestier podpora bondingu by mala patriť medzi priority. Podporou bondingu pôrodé asistentky/sestry podporujú fyziologickú, imunologickú a psychologickú adaptáciu dieťaťa a matky a dávajú zdravý základ vzťahovej väzby., Our aim is to point out the importance of postpartum bonding support, thus immediate contact of mother with her newborn baby straight after giving birth, as well as to expose desires and detect their experience regarding bonding. 2001 women (age 29.96 ± 5.34 years) who have given birth during the last three years participated this study. Own questionnaire was used for the method of data gathering. For its evaluation we used descriptive statistics. We found out that most of women (93.90 %) aspired bonding and were vigor enough (82.86 %) to be in contact with their newborn baby straight after giving birth. After motherś separation from a child, the mothers experienced child lacking (36.53 %), worries about the child (22.04 %), sadness (11.29 %). Bonding support should be one of the highest importances of midwifes /maternity nurses. V České republice je od roku 2008 zaveden celoplošný screening gestačního diabetes mellitus u těhotných žen. Od roku 2014 jsou však výsledky orálního glukózového tolerančního testu (oGTT), který je součástí screeningu, hodnoceny podle dvojích kritérií. Zatímco diabetologové a biochemici nově postupují podle doporučení Mezinárodní asociace pro studium diabetu v těhotenství (IADPSG), přejatého v řadě zemí a také WHO v roce 2013, čeští gynekologové zůstávají u původních diagnostických kritérií z roku 2008. Článek rozebírá nejdůležitější argumenty proti přijetí nových kritérií a vysvětluje důležitost jednotného postupu při screeningu a sledování gestačního diabetes mellitus., Since 2008 there have been screened all pregnant women for gestational diabetes in the Czech Republic. Nevertheless since 2014 the results of the oral glucose tolerance test (which is a part of the screening) have been evaluated by two different sets of diagnostic criteria. While diabetologists as well as biochemists newly use the criteria recommended by the International Association of Diabetes and Pregnancy Study Groups (IADPSG), accepted by various national medical societies and WHO in 2013 as well, czech gynecologists still use the former criteria accepted in 2008. Východiska: Východiskem výzkumného šetření byla Psychologická studie motivů pozdního rodičovství autorky Ireny Bímové. Cíl: Hlavním cílem výzkumného šetření bylo zjistit důvody oddálení narození prvního dítěte u matek po 35. roce. Metody: Výběrovým kritériem pro zařazení do výběrového souboru byl věk primipary 35 let a více. Jako technika sběru dat byl použit polostrukturovaný dotazník vlastní konstrukce. Počet respondentů byl 92. Data byla získávána na gynekologicko-porodnických a novorozeneckých odděleních v období od ledna 2013 do ledna 2014. K analýze získaných dat byl použit Fischerův přesný test. Výsledky: Nejčastějším důvodem oddálení narození prvního dítěte u matek po 35. roce byl výběr vhodného partnera. Tuto variantu uvádí 42,4 % matek. Druhým nejčastějším důvodem byly problémy s otěhotněním u 32,6 % matek. Součástí výzkumu bylo zpětné pozitivní a negativní hodnocení věku matek při narození prvního dítěte po 35. roce. Zpětně hodnotilo pozitivně věk porodu 45,7 % žen, negativně hodnotilo věk 54,3 % žen. Ženy, které zpětně hodnotily věk porodu 1. dítěte negativně, uvedly signifikantně častěji jako důvod oddálení porodu „Problémy s otěhotněním“ (p = 0,014). Ženy, které zpětně hodnotily věk porodu 1. dítěte pozitivně, uvedly signifikantně častěji jako důvod oddálení porodu „Preferenci nezávislého způsobu života“ (p = 0,039). Závěry: Důvody oddálení narození prvního dítěte v pozdějším věku spadají zejména do oblasti sociální, psychologické a zdravotní. Výzkum může vést k zamyšlení nad problematikou trendu pozdního rodičovství. A semi-structured questionnaire of an original design was used to collect the research data. e number of respondents was 92. Data were obtained on obstetric and neonatal departments from January 2013 to January 2014. To analyze the data was used Fisher's exact test. Results: e most frequent reason for birth postponement amongst mothers aged 35 and more was the search for a suitable partner. is option was selected by 42.4% mothers. e second most frequent answer involved issues with conception, this being reported by 32.6% mothers. Additionally, the survey research included a retrospective positive or negative evaluation of the mothers’ age when they had their first child aer the age of 35. In retrospect, the age for the first birth was seen positively by 45.7% women and negatively by 54.3% women. Women evaluating the age for the first birth negatively significantly more oen reported the reason „Problems with conception“ (p = 0.014). Many aspects of surgical treatment of intestinal malrotation in children remain to be debatable. In the opinion of the majority of the specialists, surgical treatment is required after the diagnosis taking into account serious complications of intestinal malrotation. Purpose. The purpose of this research was to conduct an analysis of surgical tactics and operative treatment method for isolated and associated intestinal malrotations in children. Material and methods. We observed 123 children at the age of one day to 15 years with malrotation during the period of 2002 to 2013. Results. We presented the data from observing 123 children at the age of one day to 15 years with various clinical-anatomic forms of intestinal malrotation over from 2002 to 2013. In 62 patients (50.4%), the evidences of the high intestinal obstruction were prevalent, while 61 (49.6%) showed signs of low intestinal obstruction. 116 patients (94,3%) were given operative intervention: radical – 95(81,9%) and palliative – 21 (18,1%). In 56 % of the cases, various simultaneous surgeries were required. There are proposed differential approaches in relation to anatomic form of malrotation and possibility of the fixation of large intestine in the physiological position. Conclusion. The results obtained from the operative treatment are presented. The condition of the protective barrier of the stomach in patients with duodenal ulcer associated and non-associated with H. pylori infection has been studied. The state of gastric mucosal barrier was studied in 72 patients with peptic ulcer disease association with H. pylori and 26 patients without H. pylori by determining the content of insoluble glycoproteins and its fractions in the basal portion of the gastric juice. In a comparative perspective the state of these indicators were studied depending on the association with H. pylori and the severity of the disease. In patients with H. pylori, damages of protective mucus barrier are characterized by the significant shortage of fucose, which is considered to be the main component of the insoluble glycoproteins. In patients without H. pylori, status of mucosal barrier is characterized by uniform and less significant reduction in the content of fractions of insoluble glycoproteins. In this patients the reduction of insoluble glycoproteins is not dependent on the frequency of exacerbation. The Purpose of this study is to demonstrate the effectiveness of combined treatment for III AB stage of nonsmallcell lung cancer. Nonsmallcell lung cancer spread form treatment is disputable. Objective: to analyze main clinical-statistical indices of spine and spinal cord injuries (SCI) and system of organization of medical aid to the victims. We conducted a cohort retrospective study of materials of medical institutions of Tashkent city. Medical records of patients and records of forensic examinations were analyzed. 242 cases of spinal cord injuries were studied. Over the last 10 years in Tashkent the frequency of SCI have been tend to increase, males and working-age people are dominated. In 60.9% of cases the injury was associated, in 36.2% - isolated, and in 2.9% - combined. Fallings from height and road traffic accidents were the main reasons for getting SCI. Lesions at cervical level registered in 45% of cases, thoracic level - 27.3%, at lumbar level - 27.7%, respectively. According to ASIA/IMSOP, full injuries were noted in 40.1% of cases, incomplete injures were in 59.9%. V devadesátých letech minulého století dochází k rozvoji laparoskopické chirurgie. Cestu ukázali chirurgickým oborům gynekologové. Vývoj metody započal simultáně na třech místech. V USA, Francii a Německu. V roce 1989 provedl Reich v USA první LAVH. Querleu v roce 1991 první laparoskopickou lymfadenektomii a v roce 1992 kombinoval Dargeant Schautovu radikální vaginální hysterektomii s laparoskopickým přístupem. Systematická pánevní a alternativně paraortální lymfadenektomie je součástí radikálních laparoskopických výkonů. Stanovili jsme novou nomenklaturu parametrií, snažíme se šetřit autonomní inervaci. Zpočátku byli operatéři autodidakté, v poslední době jsme svědky standardizace indikací a techniky. Bezpečnost výkonu a komfort operatéra zvyšuje vyspělá technika, 3D zobrazení, vysokofrekvenční bipolární technika, konstrukce trokarů omezujících možnost cévních poranění. Postupně byl téměř na všech specializovaných pracovištích opuštěn vaginální přístup a kompletní operace je prováděna pouze laparoskopicky pod mezinárodně srozumitelným označením TRLH (totální, radikální, laparoskopická hysterektomie). Systematicky se budeme ve svém sdělení věnovat anatomickým aspektům, aplikaci trokarů, základním preparačním krokům a postupům, které omezí možné komplikace, zejména ze strany močového měchýře a ureterů. Zmíníme nutné technické vybavení, základy bezpečné práce s vysokofrekvenční koagulací. The method was simultaneously developed in three countries - the United States, France and Germany. From the United States the vaginal laparoscopically assisted procedures have came to Europe. The French and German schools were based on the historical knowledge of the Schauta radical hysterectomy. In 1989 Reich was the first who operated the LAVH, followed by Querleu in 1992 with lymphadenectomy and Dargeant who combined the Schauta procedure with the laparoscopic lymph node dissection. It was the laparoscopy that taught us the proper functional anatomy. A new nomenclature of the parametries was defined. Careful attention is given to the preservation of the autonomous inervation. In the beginning all laparoscopic surgeons were self-taught. Nowadays a standardized laparoscopic techniques are implemented for a radical laparoscopic operations. The safety of this method is enhanced by using advanced technologies like 3D imaging, high frequence bipolar tools, safety trocars. More often the vaginal approach is being abandoned and the whole operation is performed laparoscopically. A generally accepted term for this operation is TRLH which stands for Total Radical Laparoscopic Hysterectomy. We will systematically describe the anatomical aspects of the dissection technique regarding the save preparation of the ureters and the bladder. Východiska: Pohybový projev zdravého organismu je charakterizován vysokou mírou variability provedení pohybového úkonu. Za situace vzniklé poruchy řízení, zejména v důsledku neurologických onemocnění, dochází v různé míře k fixnímu pohybového projevu, projevujícímu se omezením schopnosti pružné reakce na změnu zevních či vnitřních podmínek, působících v průběhu provádění cíleně zaměřené pohybové akce. Cíl: Zjistit rozdíly ve svalové aktivitě a variabilitě svalových souher akrálních a proximálních svalů během dosahu a úchopu u zdravých jedinců a u pacientů s hemiparézou. Metody: Experimentální soubor tvořilo 24 probandů po prodělané ischemické cévní mozkové příhodě (iktu) s pravostrannou hemiparézou. Kontrolní soubor tvořilo 30 zdravých probandů. Svalová aktivita byla zaznamenána pomocí povrchové elektromyografie v průběhu dosahu, úchopu a oddálení válcového a kulového předmětu. Rozdíly v hodnotách plochy pod křivkou záznamu jednotlivých svalů mezi experimentálním a kontrolním souborem byly statisticky vyhodnoceny pomocí T-testu pro nezávislé skupiny. Vztah mezi aktivitou svalů předloktí a svalů pletence ramenního byl hodnocen pomocí Spearmanova korelačního koeficientu pro neparametrické hodnoty. Výsledky: Byly zjištěny významné rozdíly v míře a počtu statisticky významných korelací akrálních a pletencových svalů. U souboru probandů s hemiparézou byla zjištěna statisticky významně nižší aktivita m. pectoralis major a naopak vyšší aktivita m. trapezius. Současně převažovala vysoká korelace m. pectoralis major a extenzorů zápěstí a prstů, na rozdíl od souboru zdravých probandů, kdy se vzájemné korelace akrálních a proximálních svalových skupin objevovaly nahodile, v menší míře a četnosti. Závěry: Výsledky ukazují na snížení variability svalové aktivity u pacientů s hemiparézou v průběhu úchopové funkce. Náhradou nedostatečné funkčnosti akrálních svalových skupin se stává relativně fixní „ko-aktivace“ s vybranými proximálními svalovými skupinami., Background: Movement of a healthy organism is characterized by a high degree of variability performance of a movement task. In a situation of a motor control lesion, mainly due to neurological disease, occurs in varying degrees to a fixed musculoskeletal manifestation. ere are manifesting in a limiting responsiveness to changing external or internal conditions during the task oriented physical action. Objective: To determine differences in muscle activity and variability between a muscle synergy of acral and proximal muscles during reach and grip in healthy subjects and stroke patients. Methods: e experimental group consisted of 24 probands aer ischemic stroke (ictus) with right-sided hemiparesis. e control group consisted of 30 healthy subjects. Muscle activity was recorded using surface electromyography during the performation of selected types of grips of cylindric and spheric objects. e differences in the area under the curve record of individual muscles between the experimental and control group were statistically evaluated using the t-test for independent groups. e relationship between the activity of the muscles of the forearm and shoulder girdle muscles was assessed using Spearman‘s correlation coefficient for non-parametric values . Results: ere were the significant differences in the size and number of statistically significant correlations distal and proximal arm muscles. In stroke group was found significantly lower activity m. pectoralis major and higher activity m. trapezius. At the same time the prevailing high correlation m. pectoralis major and extensor wrist and fingers, unlike the group of healthy subjectss, when the correlation distal and proximal muscle groups appeared at random, in a lesser extent and frequency. Conclusions: e results indicate a reduction in variability of muscle activity during grasp function of stroke patients. Background: Obstetric complications developing at antiphospholipid syndrome are serious medical-social problem. Seeking of the treatment methods sets conditions for urgency of the article reviewed. Enzyme preparations are widely applied for treatment of women with fecundity disorder and hemorheology changes. Research objective was in clinico-laboratorial evaluation of Serrata drug impact in antiphospholipid syndrome in women with reproductive losses of Andijan state. Methods: Data of 43 women in the non-pregnancy state with reproductive losses (RL) in past history with revealed APS had been studied by us. Results: Thus, conducted therapy with Serrata was conductive to haemostasis. Východiska: Validizační studie v ošetřovatelském procesu umožňují vyjádření míry platnosti komponent NANDA-I pro různé soubory osob a v různých oblastech péče. Bolest jako ošetřovatelská diagnóza v péči o rodící ženu je jedním z nejčastějších problémů, které porodní asistentky v praxi posuzují, řeší a hodnotí. Validované definující charakteristiky ošetřovatelských diagnóz jsou rozhodující pro správnou diagnostiku typu a intenzity bolesti a pro navržení efektivní léčby. Cíl: Cílem práce bylo validizovat definující charakteristiky ošetřovatelské diagnózy 00132 Akutní bolest vybraným souborem expertek – porodních asistentek a zjistit, které definující charakteristiky (určující znaky, související faktory) porodní asistentky považují za hlavní a které definující charakteristiky za vedlejší. Metody: Pro obsahovou validizaci byl využit Fehringův model validity diagnostického obsahu (Diagnostic Content Validity Model). Expertky – porodní asistentky splňovaly kritérium bakalářského stupně vzdělá- ní v oboru porodní asistence a klinické praxe na oddělení porodního sálu. Výzkum probíhal současně ve Fakultní nemocnici Olomouc, v Nemocnici Šternberk a v Nemocnici Uherské Hradiště. Měřící nástroj pro posouzení významnosti diagnostických prvků byl sestrojený pomocí jednotlivých definujících charakteristik ošetřovatelské diagnózy z NANDA-I, Taxonomie II. Výsledky: Z celkového počtu 22 definujících charakteristik (18 určujících znaků, 4 související faktory) považují porodní asistentky za hlavní (majoritní) 9 definujících charakteristik. Mezi hlavní definující charakteristiky (určující znaky) s váženým skóre (VS) vyšším než 0,75 porodní asistentky zařadily: úlevová poloha z důvodu vyhnutí se bolesti (VS 0,82); expresivní chování (VS 0,77); pozorované známky bolesti (VS 0,80); slovní vyjádření bolesti (VS 0,84); změny dýchání (VS 0,80); změny krevního tlaku (VS 0,82); změny srdeční- ho rytmu (VS 0,86). Mezi hlavní související faktory zařadily tyto charakteristiky: biologické faktory – porodní mechanismus (VS 0,94) a faktory psychogenní (VS 0,80). Závěry: Z výsledků tohoto šetření vyplývá, že dotazované porodní asistentky považují z celkového nabízeného počtu 22 definujících charakteristik (18 určujících znaků a 4 související faktory) 9 za hlavní – majoritní pro celkové posouzení stavu klientky – rodičky přicházející na porodní sál k porodu., Background: Validation studies allow expression of the degree of validity of components NANDA-I for different sets of people and in different areas of care. Pain is one of the most common problems that midwives in practice assess, evaluate and solve. Validated defining characteristics of the nursing diagnoses are crucial for correct diagnosis of the type and intensity of pain, and for proposal of effective treatment. Aim: The aim of the study was to validate defining characteristics of the nursing diagnosis Acute pain 00132 by a selected file of experts-midwives, and see which characteristics they consider major and minor. Methods: The study followed Fehring Diagnostic Content Validity Model. Experts-midwives met the criterion of bachelor degree in midwifery and clinical practice in the delivery room. The research was conducted simultaneously in Olomouc region. Measuring tool for assessing the significance of diagnostic elements was constructed using individual defining characteristics of the nursing diagnoses of NANDA-I Taxonomy II. Results: Out of the 22 defining characteristics considered by experts-midwives nine characteristics were described as major with weighted score (WS) greater than 0,75: antalgic position due to the avoidance of pain (WS 0,82); expressive behavior (WS 0,77); observed signs of pain (WS 0,80); verbal expression of pain (0,84); changes in breathing (0,80); changes in blood pressure (WS 0,82); changes in heart rhythm (WS 0,86). Among the major related factors were selected the following characteristics: biological factors - birth mechanism (WS 0,94) and psychogenic factors (WS 0,80). Období vstupu do rodičovství je významným přechodovým obdobím v psychické ontogenezi ženy. Důležitým faktorem předvídajícím rodičovské kompetence a emoční pohodu je rodičovská self-efficacy (vnímaná rodičovská účinnost). Nedávné studie ukazují, že rodičovskou self-efficacy matky může ovlivnit i spokojenost s porodním zážitkem. Ta je zase predikována sociálně psychologickými aspekty porodního zážitku, především oporou ze strany zdravotníků a pocitem osobní kontroly. V longitudinálním dvoufázovém výzkumu jsme proto sledovali faktory ovlivňující rodičovskou self-efficacy v období kolem narození prvního dítěte a jednotlivé determinanty porodního zážitku. S využitím postupů mnohonásobné lineární regrese jsme zjistili, že ačkoli je rodičovská self-efficacy z velké míry založena už před porodem, opora ze strany zdravotníků při porodu a snadná utišitelnost dítěte ji mohou významně pozitivně ovlivnit. Potvrdili jsme také rozhodný vliv opory ze strany zdravotníků při porodu na spokojenost s porodním zážitkem i v našem souboru, reprezentativním pro prvorodičky v ČR., Transition to parenthood is a significant transition period in a woman’s psychological ontogenesis. An important predictor of parenting abilities and well-being is parental self-efficacy. Recent studies show that mothers’ parental self-efficacy can be influenced by childbirth satisfaction, which, in turn, is predicted by social-psychological aspects of childbirth experience, especially support by caregivers. In a longitudinal 2-phase research, we have followed parental self-efficacy determinants working around 1st childbirth as well as childbirth experience determinants. Using multiple linear regression analysis we determined that although parental self-efficacy is mainly prenatally determined, caregivers‘ support and baby’s soothability can impact it positively.
